JOINT AFFIDAVIT OF ARREST
WE, PO2__________________ AND PO1 _______________ OF LEGAL AGES,
MEMBER OF PHILIPPINE NATIONAL POLICE, PRESENTLY ASSIGNED AT STATION
ANTI-ILLEGAL DRUGS SPECIAL OPERATIONS TASK GROUP. AFTER DULY
SWORNED TO IN ACCORDANCE WITH LAW DO HEREBY DEPOSED AND STATES
THAT;
That, on or about 2:00 PM, 19 July 2011 inside open space parking area
of ___________________, We, the undersigned arrested __________34 years old,
married, elementary under graduate, birth date unknown at___________ and a
resident of ______________________.
A confidential informant (CI) came to the Barangay Security and
Development Office (BSDO) at around 8:00 oclock in the morning of 24
August 2016 to inform Barangay Captain _________________ about the
illegal drug trade activities of Lawrence Mikhail Saguisag and his
companion, _______________; that ____________ convinced the CI to buy
Methamphetamine (Desoxyn, crystal meth, crystal, speed, crank,
bennies, ice), one of the commonly known party drugs, along with Amyl
nitrates (butyl nitrate, poppers, rush, hardware, locker room, aroma of
man), Ketamine hydrochloride (Ketalar, special K, vitamin K, cat Valium),
MDMA (mehthylenedioxy-methamphetamine, ecstasy, XTC, X, STP, clarity)
and Methamphetamine (Desoxyn, crystal meth, crystal, speed, crank,
bennies, ice); that ________________ coordinated with the Chief of _________,
Police
Superintendent
_________________(P/Supt.
__________),
who
immediately formed a team to conduct a buy-bust operation against
Saguisag and and ______________ composed of a certain Police Inspector
________________ (P/Insp. Gatus) as team leader, BSDO _________ as poseurbuyer while BSDO _____________ and the rest of the members, who were
police officers, would serve as members of the back-up team; that upon
arrival of the team at the agreed meeting place in front of ________________
store, Barangay San Antonio, the CI and _____________ waited for Saguisag
and _____________ while Esguerra and the other team members monitored
the process of entrapment from a viewing distance.
Catubays testimony, however, was in stark contrast to the above
declaration.
Upon receipt of their said complaint, our office planned out an
entrapment operation against the suspect. At about ______________ of
______________, the undersigned together with the complainant proceeded at
the pre-arranged pay off _________________________ located along
____________________________, __________ to give the money being asked by the
respondent. Upon arrival thereat, complainants waited for the suspect to
arrive while we the undersigned seated near the table of the complainants.
At about ________, suspect arrived and approached the complainants and
after few conversations, complainants handed over the marked money to the
suspect that minute of prompted the undersigned to immediately effect
arrest.

Seized and recovered in the possession and control of the arrested

person were the following, to wit:
a.) Marked Money, (state the denomination and serial numbers);
b.) (Name of documents promised);
c.) (type of CP and SIM Card); and
d.) Other evidence confiscated;
The report of the PNP Crime Laboratory Group Physical Identification
Report No. ___________ dated __________, revealed that suspects
______________________ was POSITIVE for the presence of Bright Yellow
Ultraviolet Fluorescent powder on both palm and right dorsal portion of the
hands.

That, Prior to his arrest, On or about 11:00 am, 18 July 2011, one of our
confidential informant person reported to our office was able to talk with our
team leader and with us, revealing information regarding illegal drugs,____of
alyas and her cohorts, plying their illegal drug trade at Taguig City,
jurisdiction of Southern Police District and National Capital Region.
That upon receipt of such informations, Our team leader however, able
to form a group and instructed us to verify and confirm the revelations of the
said informant. A briefing was conducted also instructed to _______
remittance and possible buy bust operation on the mentioned subject. That
before the conduct of possible operation of informed subject. Our team
leader instructed me to coordinate with SPD-DAID-SOTG and PDEA on which
we were coordinate with both offices (see coordination from PDEA and DAIDSOTG-SPD dated July 18, 2011) That we _______conducted by our team
leader, I ___________ was designated as cop poseur buyer on which she give
already marked with MP. I was also instructed that the pre-arranged signal
should be fighting of ______________ upon consummation of buy bust.

That, On or about 1:20 PM, 19 July 2011, Our team decided to conduct
operation to our subject and went to proceed and reach near the mentioned
target place at about 1:50 PM, same date. That I _____________informant were
the first who alighted our vehicles and walk on foot heading the place of
alyas _______ and back ups were secretly following us and strategically
positioned themselves to see me and our informant for arranged signal.
That, before reaching the target place, Our informant told me that our
subject alyas _____________was not ________, he (informant) was able to
identify the male person (later identified as ___________ as a relative) which
he knows the companion and right hand of ________in the selling of illegal
drugs. Was _______ of the exit gate of ___________ Taguig City near open space
parking area____________ Taguig City obviously waiting for a possible
customer I (__________) and our informant was able to approach the male
_____to be the right hand of alyas ____ and they greeted each other, Our
informant asks the male person of the whereabouts of alyas _________and the
latter replied that alyas ____had a fever and he was at the same time was the
one _____of the business, During the course of the conversation , Our
informant able to convinced the male person _______ that I (PO2_____) was
friend of him and a drug dependent. That our informant able to ask the male
person _____he had stuff and we are going to buy one thousand pesos worth
of shabu since there was an occasion in________the male person however
replied that before alyas ____leave, he had stuffs left given by alyas _____and
it or one thousand two hundred pesos per sachet and he had only two sachet
left, But because he knows that our informant is the customer, the male
person __________able ro sell one sachet for one thousand pesos. As agreed
upon, _____asks me the one thousand pesos as payment for the shabu,
subsequently, Upon receipt of one thousand pesos, He ______ (later identified
as _______ give me one small heat sealed transparent plastic sack
_______yellowish crystalline substance suspected to be shabu (later marked
JEM-1) afterwhich, I immediately light cigarette as pre-arranged signal,
however, upon sensing the presence of my teams that were rushing to ____
said subject, preventing _____running away and I immediately introduced my
self as police officer while PO1 __________they _________and able to reach my
place and assited me and instructed him to hold and handcuffed said alyas
______, I (PO2____) was able to recover from him _____________the one
thousand pesos being used as marked money and confiscated from him
another sachet of suspected shabu, (later marked JEM-2) That, ______ was
able to reach the place of our informant and poseur buyer and the male
person (alyas) was instructed by PO2 ____to handcuffed said male person,
we also informed his traditional rights and the nature of his arrests.
That, We brought immediately both the said arrested person and
evidence to our office and we turned over to our investigator. That, Upon
receipt, of the evidence and suspect by our duty investigator. Arresting
officer ______investigated while the arrested suspect was adviced to remain
silent informing his constitutional rights and _____arrest. Said evidence and
suspect were brought to Crime laboratory service office for confirmality
_____found positive to the tests for metylamphetamine hydrochloride a
dangerous drug. That, upon receipt of ________ that found positive to the
tasts (see laboratory examination result) , We execute this ______to file
appropriate charges against__________________ Section 5 and 11, Article 2 of
Ra _____to the truth of the foregoing, We are affixing our signature below.
