HWP # 14923 issued to Tulsi contractor for platform

fabrication and wrapping of pipeline in the fabrication
yard. The welder who was doing welding found using
only cotton gloves. He did not use welding gloves. Also,
the wrapping job using Gas flame was done by 3
people. Only 2 of them were using welding gloves. One
person was using normal gloves. Stopped the work,
asked them to conduct a tool box again, explained the
importance of following the requirement of PPEs by
everyone involved in the job and Fire man was asked to
verify the tool box before continuing the work.

tank and normally not manned all the time. Hence, if
there is any leak, it may not be identified immediately.
As per VF requirements, all vents and blinds to be

blinds/plugs on all vents and drains

1230 hrs transporter representative went to inform the
surveyor sitting in sample sealing cabin for empty
tanker inspection. While he was peeping from the
window to call the surveyor he kept his hands on the

with droplets of phenol already present on the table.

from tk-126, leakage observed through safety valve of
1st compartment while waiting for sealing & sampling
and same was arrested.

We done surprised check by Raghu, Amol and Prakash

during travelling General shift bus from residence to
terminal (on the way) at 09:30 Hrs, 22 07 2014, we
found and surprised that there is no safety belt
compliance (by our own staff and driver) during
travelling. In general shift bus, following responsible
persons were travelling: 1. Varun Sharma - Safety

New Castor oil header is recently commissioned to load

Castor oil into Flexi containers. There are 3 tappings are
taken on this pipeline with an elbow welded on the
pipeline. A valve, NRV, Blind flange and hose is also
connected to this elbow tapping and the entire load of
valve, NRV and hose are acting on the elbow tapping.
No support is provided for the valve and NRV fitted on
the elbow.

1. Excavation certificate is issued by the authorized

engineers, but the layout for checking the underground
cable, pipes in that area is not attached and verified
before issuing the certificate. 2. Road is blocked since
26th July. Road block certificate is authorized and
marked on the layout. This blockage is not
communicated to all, especially to the Security, All shift
leaders and the fire men, etc. Alternate access for the
fire vehicle (Gate # 4 is available) is not identified and
not communicated. In case of emergency especially in
night shift, the fire vehicle may have to reach the spot
and identify that the road is blocked. 3. JSA is attached
with the permit. Few hazards are not identified like
electrical shock, chemical possibility while digging, etc.
Actions for these hazards have been taken without
identifying the hazard. 4. Nearby JB is standing on a
pole. Digging is going upto that location. This is not
identified in the risk assessment/JSA for a possibility for
tilting.

During unloading of the castor oil , spills occurred

approximately 03 liters by the tanker lorry No.GJ2Z
3804 at unloading point, Transporter is M/s:
N.K.Roadways, Driver Name : Haribhai.
spills occurred 05 liters of the cargo CDSBO from tanker
lorry No.GJ12W 9504, Driver Name: Lalit Bhai,
Transporter Name: Pooja Roadways. Gentry No.5.
Tanker no. : GJ12V 7486 Driver name : NATHALAL
Transporter : NATIONAL TRANSPORT. After completion
of the loading CDSBO, the driver started the tanker for
the gross weighment but unfortunately the air brake
was failed due to which the tanker hit the collision
guard as a result of which the collision was broken.
Tanker No. MH04DK 5393 spilled the cargo CDSBO
approx.02 liters during loading of the tanker from
Gentry No.09, Transporter Name: Ashok Bulk Carrier,
Driver Name: Baldev Singh. the incident occurred on
30.07.14 at 14:40 hrs.
Hazardous waste storage room at south east corner is
kept locked. Chemical/Oil leaks through the door
bottom from the room and goes to the soil. We were
told that the used pigs are kept in the room. Requested
the supervisor to open and check.
