Extraocular motor neurons residing in the nuclei of the oculomotor (CN III), trochlear (CN IV) and
abducens (CN VI) nerves are spared until very late in the disease course. Patients who choose long-term
mechanical ventilation have a longer clinical course that can include progressive difficulty with ocular
motility. This may culminate in the locked-in-state, a clinical condition characterized by inability to move
any voluntary muscle. Such patients may be alert and awake but completely unable to communicate.
