Student Organization Profile Officer Worksheet (Optional)

This worksheet is designed to assist student organization officers in collecting the required information for updating
officers in the PantherSync system organization profile. Once the worksheet is completed, you can login to your
organizations portal in PantherSync, select settings and update the officer information in the Organization Profile and
submit the revisions electronically.
All student organizations must have at least four officers. If additional officers are listed, their information must be
complete (all required fields) as well. All data fields are required in PantherSync unless otherwise noted.
Faculty/Staff Advisors: UW System Risk Management has stated that Faculty/Staff Advisors to student organizations are
provided liability protection from their actions when acting within the scope of their duties. The supervisor information
required below will be used to send documentation of advisor status and liability protection.
