Specifically known as ASAP (At Source Ang Processing). This program aims to
facilitate accessibility in the filing and processing of request for Individual Medical
Assistance Program (IMAP) for indigent patients confined in the affiliated hospital.
Patients NEED NOT TO GO TO PCSO office to file their request for assistance.
They can immediately file their request and submit the necessary requirements at
the PCSO Help Desk.

ANDRES SORIANO FOUNDATION
Andres Soriano Foundation offers assistance to patients with BREAST CANCER
STAGE I-II or better. Patients must prepare the following:
1. Profile of patient, diagnosis (original or certified true copy of medical abstract
and histopathology or biopsy results), history, plan of treatment/doctors
report with his name, signature and license number.
2. Names of medication needed (prescription of all drugs signed by the medical
officer with his/her license number) and prices (include source of price
information).
3. Letter of request for financial assistance from the patient.
