Please fill in your surname as stated in your international passport or other identity document.
First and middle names
Please fill in your first and middle names as stated in your international passport or other identity
document.
Any previous names
Please fill in previous names in this section. This is only applicable if you have changed your name.
If you have never changed your name, you do not need to fill in this section.
Date of birth
Please fill in your date of birth in the following order: day, month, year.
Norwegian social security number
In this section, please fill in your Norwegian Social Security number (5 digits). You do not need to
fill in this section if you do not have a Norwegian Social Security number.
Street
Please fill in your street address (or if applicable your post office box). If you are not registered at
this address, please use c/o (care of) in the following manner: Kari Nordmann, c/o Lydia Dalen,
Storgata 1, 0999 Oslo).
Phone number
Please fill in a phone number where you can be reached during daytime (office hours).
Postcode
Please fill in your postcode (4 digits).
Place
Please fill in the place where you live.
Email address
Please fill in your email address. If you do not have an email address, you do not need to fill in this
section.
Country
Please fill in the country you live in.

Please tick the box to state in what language you wish to receive letters and other written
information from NOKUT.

Please fill in the name of the degree you have been granted. Use the original language. If the
original language is not written in Latin letters, you have to translate the name of the degree into
Latin letters.
Official study length of degree programme
According to study plans, please fill in the official study length of your degree.

Name of institution
Please fill in the name of the degree granting institution.
Place and country
Please fill in the name of the place (town) and the country where the institution is situated.
Did you complete the degree?
Please tick the relevant box to let NOKUT know if you completed the degree or not. If you
completed the degree, you will most likely have received a final diploma from the institution.
If NO, how much was completed?
Please fill in how much of the degree you completed. If you completed the degree, you do not
need to fill in this section.
Did your degree include a thesis/dissertation component?
Please tick the correct box to let NOKUT know if you wrote a master thesis/dissertation.
If YES, what was the scope of the thesis?
If you wrote a thesis as part of your degree, please fill in this section and state the scope either in
weeks, months or study points. If you did not write a thesis, you do not need to fill in this section.

3. Previous education
Education
Please fill in the name of the final certificate from primary and secondary education (lower and
upper). If you have completed technical/vocational education, you can fill in the name of the final
certificate from that education.
Name of institution
Please fill in the name of the institution that granted the final certificate.
Place and country
Please fill in the name of the place (town) and the country where the institution is situated.
Year started
Please fill in the year in which you were enrolled.
Year completed
Please fill in the year in which you graduated.

order for your application to be assessed, you need to send the requested documentation
together with the application form.
Higher education you seek recognition of
This section contains information on documentation requirements concerning your higher
education credentials. In order for your application to be assessed, you need to send the
requested documentation together with the application form.
Previous education
This section contains information regarding documentation of secondary education. In order for
your application to be assessed, you need to send the requested documentation together with the
application form. We need the final certificate from secondary education that grants access to
higher education (universities or university colleges).

5. Additional information
In this section you may write down additional information that you regard as important in
your application. Please note that the section is only meant for additional information that is information that has not been stated in other sections of the application form. If
you do not have any additional information, you do not need to fill in this section

6. Important information, power of attorney and signature
