AFFIDAVIT OF ARREST
We, the undersigned members of the Philippine National Police, presently
assigned at the Olongapo City Police Office Police Station 3, Olongapo City, Zambales,
do hereby depose and state THAT:
1. We are among the members of the Philippine National Police of Olongapo City
who conducted entrapment operations at HotBabe Club located along 3293, Red
Light Street, Brgy. Olongapo City, Zanbales;
2. Said operation stemmed from the complaint of MARIA HIPOLITO ARAGON for
Human Trafficking violations.
3. On the complaint dated June 20, 2016, it was alleged that Andrea Hipolito
Aragon, daughter of the complainant, was forced to engage in prostitution in the
HotBabe Club Olongapo.
4. Upon receipt of the said complaint, our office planned out a surveillance
operation against the suspect on June 23 and 25, 2016 to determine if the
allegations in the complaint has basis.
5. On the morning of 23rd of June as a first-hand information PO2 Cadavis entered
twice the said Club with regard to the prostitution activities facilitated by Mr. M
who was positively identified as one Alexander Charles Uy and from the series of
surveillance operations, our office mapped out an entrapment operation against
Mr. M and HotBabe Club.
6. On June 30, 2016, an entrapment operation was conducted led by the
undersigned, together with the Philippine National Police Olongapo City, with the
assistance of social workers from the Department of Social Welfare and
Development (DSWD) Olongapo City.
7. Upon arrival at the HotBabe Club at around 11pm of June 30, 2016, SPO3 Mark
Morelos and PO2 John Lloyd Cadavis together with the other operatives were
greeted by Alexander Charles Uy and upon recognizing PO2 Cadavis as a
returning customer, we were ushered into a table near the VIP lounge and asked
a waiter to take their orders as Alexander Charles excused himself to entertain
other customers.
8. A few minutes later, Alexander Charles Uy returned and approached PO2
Cadavis and discreetly asked if he would like to have a special service which
covers sexual intercourse with a girl of his preference in a room provided by the
club for VIP customers for an amount of money depending on the duration and
the extent of service.
9. PO2 Cadavis asked, Alexander Charles Uy how much the rate for the special
service is if he would opt to have 3 hours with the girl. Alexander Charles Uy

answered that it would amount to 10,000.00 Php and the service is all the way
referring to the sexual intercourse.
10. As PO2 Cadavis agreed, paid Alexander Charles Uy the marked money and
counted it. Then, he was led to a showroom full of girls with a one- way glass
panel which can only be seen from his side. He was asked to choose any girl he
wants and PO2 Cadavis picked Andrea.
11. Upon choosing Andrea, Alexander Charles Uy called on her to go outside of the
showroom and introduced her to PO2 Cadavis and she led him to a room in the
VIP lounge. As Andrea was taking her clothes off, she was prevented by PO2
Cadavis and apprised her of the entrapment operation and informed her that
there are operatives of the police inside the club to rescue her.
12. As PO2 Cadavis signalled SPO3 Morelos through a text message that he had
already secured Andrea and that the operation is a go, SPO3 Morelos found him
in another table talking to another customer and offering the same services
offered to PO2 Cadavis and witnessed the payment made by the customers.
Then and there SPO3 Morelos personally facilitated the arrest of one Alexander
Charles Uy and informed him of the cause and nature of his arrest, read him his
rights while cuffing him.
13. The police were able to seize and recover item(s) in the possession and control
of the arrested person were the following, to wit:

14. Suspect together with the seized and recovered evidence was brought to the
Olongapo City Police Station 3, Olongapo City, Zambales for booking and proper
disposition. Likewise, the arrested person was apprised of their rights as
provided for under the Constitution of the Philippines.
15. The report of the PNP Crime Laboratory Group Physical Identification Report No.
16-4569 dated July 1, 2016 revealed that suspect Alexander Charles Uy was
POSITIVE for the presence of Bright Yellow Ultraviolet Fluorescent powder on
both palm and right dorsal portion of the hands.
We executed this affidavit to attest the truthfulness of the foregoing facts and to
support the filing of Criminal Cases against Alexander Charles Uy for violation of Sec. 4
par. (a) and (h) of the R.A. No. 9208, Qualified by Sec. 6 par. (h) of the R.A. No. 10364.
