Application ($830)

USMLE Step 3 Application - The Step 3 application fee is non-refundable and non-transferable from one
eligibility period to another or from one application to another. Your application fee must be paid by
American Express, MasterCard or Visa.
2016 Fees

Effective Dates for 2016 Fees

Three-month eligibility periods that end in 2016.

$830

The first eligibility period in which the 2016 fee will apply is November 1, 2015 - January 31, 2016.
The last eligibility period in which the 2016 fee will apply is October 1, 2016 - December 31, 2016.

Certification of Identity (CID) Form

Certification of Identity (CID) Form
(http://www.fsmb.org/Media/Default/PDF/USMLE/USMLEStep3_Certification_of_identity.pdf) - A CID is
required for all Step 3 applications unless one is already on file with FSMB and has not expired. CID Forms from
prior Step 3 applications are valid for 5 years from the date of the applicant's signature.

Name Change/Correction
Name Change/Correction Authorization Form
(https://www.fsmb.org/Media/Default/PDF/USMLE/NameChange_Form.pdf) - Use this form to change or
correct your name. Evidence of the change/correction must also be submitted.

Test Accommodations
Visit the Test Accommodations (http://www.usmle.org/test-accommodations/forms.html) page on the USMLE
website to download the forms to:
Request new test accommodations or to request a change to accommodations received previously
Request the same accommodations for Step 3 that you received on Step 1 or Step 2

Eligibility Extension ($70)

Eligibility Extension application - If you need to extend your eligibility period, you must submit an eligibility
extension request and $70 processing fee. Requests can only be submitted after your original scheduling permit is
available and must be received no later than 25 days after the end of your original eligibility period. If your request
is approved, a one-time contiguous 3-month extension will be granted. (Note: "Contiguous" means that the start
date of the eligibility extension immediately follows the end date of your eligibility period.)
Requesting an extension of your eligibility period does not cancel a scheduled exam appointment. If you have a
scheduled testing appointment during your original eligibility period and need to reschedule the appointment for an
extended eligibility period, you must cancel or reschedule with Prometric (https://www.prometric.com/enus/Pages/home.aspx). A fee may be charged if you change your appointment, depending upon how much notice
you provide when making the change. See the appointment change fee information posted on the USMLE website

Score Recheck ($80)

Score Recheck Form (https://www.fsmb.org/Media/Default/PDF/USMLE/usmle-score-recheck.pdf) A recheck of

your Step 3 score will be done upon submission of the recheck form and $80 fee. Please note that, for all USMLE
Steps, standard procedures ensure that the scores reported for you accurately reflect your performance. A change
in score based on a recheck is an extremely remote possibility.
