JM Saadiah was on her way to her patients house for compulsory post natal home
visit which located in Belemang Estate. Around 08:39 am, she saw a big tractor
coming from her opposite side. Since the road is small and narrow, she stop at the
left side of the road to give way to the tractor.

When the tractor pass by her, the tractor accidentally hit over her motorcycle which
causing her falling over her right side. Her right knee sustained bruise and swelling.
JM Saadiah took the drivers phone number for further management and she
continued her home visit

During JM Saadiahs home visit session, she can feel throbbing pain over her right
knee and the affected knee becomes more swelling. She called up her on call Sister,
KJK Norhayati Amat and informed her regarding the Motor Vehicle Accident. KJK
Norhayati Amat noted and will meet her at KK Sagil as soon as possible. KJK
Norhayati Amat informed Dr Zulaini Jamalludin regarding the motor vehicle
accident.

After her home visit session was done, she drove back to KK Sagil in pain and met
her sister in charge at KK Sagil. She told her what actually happened and the sister
in charge brought her to nearest police station for police report

KJK Norhayati Amat and JM Saadiah went to Balai Polis Sagil to lodge a police report
but was told by the Sagils Police Officer to lodge the police report at Balai Polis
Tangkak since it involved traffic report. Since JM Saadiahs right knee pain
increasing, KJK Norhayati decided to bring JM Saadiah to KK Payamas for proper
check up and further treatment

At KK Payamas, JM Saadiah was seen by Dr Ahmad Nizal Bin Mohd Ghazali and PPP
Muhammad Syazwan Bin Samsudin. The diagnosis given was Alleged Motor Vehicle
Accident with Soft Tissue Injury over Right Knee. Medical Certificate was issued for 2
days ( CJBH 008310 / 16.10.16-17.10.16 ). The treatments given was IM Sodium
Diclofenac 75 mg and pressure bandage was applied over right knee to reduce
swollen. Brought home medications are tablet sodium diclofenas 50 mg three times
per day, tablet prolase 2 tablets, three times per day and Methylsalicilate mentol
(LMS) ointment to be apply over affected area.

KJK Norhayati Amat and JM Saadiah arrived at Balai Polis Tangkak. Police report was
lodge against the tractor driver.
