Permission Form

I am an A2 media student who has been asked to film a music video for The
Crafts The End song. To ensure that I meet legal requirements please read the
following terms and conditions and sign and date bellow to show that you give
permission and that you understand the information given.
I understand and agree the terms and conditions as
follows;
1. That I will be recorded and featured in a music video
2. If for any reason youre not happy with the footage that youre featured in
that you have the right to withdraw without reason
3. That the production will be edited and manipulated to fit the genre of
music.
4. That the music video will be made public and will be distributed onto many
different media platforms.
I give my full consent to being featured in Emma Jane Laffoleys A2 media
production and I fully understand the terms and conditions listed above.
