INFORMACIN CLNICA IMPORTANTE

CUIDADO DE LA PIEL
La piel sirve como barrera protectora contra todo ataque qumico, mecnico y biolgico.
Tambin es importante para la regulacin de la temperatura corporal y acta como va para la
excrecin de agua, especialmente en los neonatos pretrminos. Se ha establecido una relacin
entre la introduccin de nuevas tcnicas de cuidado intensivo y la mayor supervivencia de
neonatos pretrminos de muy bajo peso. La inmadurez drmica de estos neonatos de bajo peso,
sumada a la manipulacin e instrumentacin excesivos de que son objeto, presentan problemas
anteriormente no reconocidos para su atencin en las unidades neonatales.
Srvase leer, evaluar y poner en prctica las recomendaciones siguientes, segn corresponda:

Cuando vaya a utilizar este dispositivo con un recin nacido, vea la siguientes
recomendaciones para el cuidado de la piel, que guran en el material informativo2. Deber
prestarse atencin especial a la higiene e integridad de la piel.
Observe el color y compruebe si hay erupciones o excoriacin

Limpie la regin perineal despus de una deposicin

Informacin general
Este manual describe cmo vericar, hacer funcionar y mantener el sistema de fototerapia
BiliBlanket Plus High Output de Ohmeda Medical. Adems describe los procedimientos de
servicio de reparaciones de la BiliBlanket Plus High Output para aquellas personas que estn
tcnicamente preparadas para prestarlos.
Descripcin
En la unidades con opcin de transiluminacin, la luz no ltrada en el espectro visible pasa
por un tubo de luz exible y aparece en la punta del cable transiluminador. La luz del cable
se usa para facilitar inyecciones o pinchazos vasculares. Tambin se usa para encontrar
los neumotrax. Unidades son disponibles con almohadillas bra-ptico de dos tamaos;
almohadilla de tamao regular Alta Potencia y la almohadilla ms grande Max XL Bilipad.
El sistema de fototerapia BiliBlanket Plus High Output consiste en una unidad de reector y
una almohadilla luminosa con un cable broptico de 1,20 m de largo. La unidad del reector
contiene una lmpara, ltros para la luz, un suministro de energa variable para el reector, un
sistema de refrigeracin y una proteccin contra recalentamiento cerca de la lmpara.
La lmpara del reector consta de una bombilla halgena de tungsteno de alta intensidad con un
reector incorporado. Este est revestido de una supercie dicromtica que reduce la energa
infrarroja que se transmite. La bombilla se fabrica especcamente para usarse con el BiliBlanket
Plus High Output.
Aunque habr una ligera degradacin con el tiempo, la bombilla mantendr aproximadamente la
misma intensidad de luz durante todo su perodo de vida til.

1/Informacin general
Un ltro de luz, ubicado frente a la lmpara, rechaza la luz que est fuera de la gama nanomtrica
de 400 a 550. Este ltro bloquea casi toda la luz ultravioleta e infrarroja; solamente se permite
que pase la luz azul. Esta luz ltrada se centra en la entrada del cable broptico.
Para la almohadilla regular el rango de control es desde 21 +7.35/-5.25 uW/cm2/nm cuando el
selector est al mximo a la izquierda y 50 +17.512.5 uW/cm2/nm cuando el selector est
al mximo a la derecha. Para el Max XL Bilipad el rango de control es de 15 +5/-4 uW/cm/nm
cuando el selector est al mximo a la izquierda y 35 +12/-9 uW/cm2/nm cuando el selector est
al mximo a la derecha.

precisar la medicin de la intensidad de luz.
Seleccin de modo
Selecciona el modo de operacin: fototerapia o transiluminacin. Usando el tubo de luz
transiluminador de Ohmeda Medical, el sistema BiliBlanket Plus High Output puede usarse como
un transiluminador.
Suministro de energa
Se puede suministrar energa a la unidad del reector mediante cualquier fuente estndar de CA
ya sea a 50 60 Hz que tenga un voltaje entre 90 - 132 180 - 264. La energa entra al reector
a travs de un receptculo que tiene un interruptor de alimentacin integral.
Refrigeracin
Un ventilador enfra la unidad del reector. Al lado del ltro de luz hay un disyuntor trmico
que sirve para proteger la unidad del reector y el cable broptico o transiluminador contra el
recalentamiento.
Cable broptico
El cable broptico consta de 2400 bras plsticas individuales que transmiten la luz del
reector a la almohadilla luminosa, la cual fue hecha entretejiendo estas bras hasta formar
una malla. Este proceso patentado produce una almohadilla que tiene luz en toda su supercie.
Estas bras estn mezcladas intencionadamente en el cable para eliminar cualquier gradacin
de intensidad debida a zonas calientes en la bombilla, polvo en el ltro, polvo en el extremo del
cable, etc. Esto permite que haya una capa de luz casi continua y uniforme. Se proporciona una
cubierta protectora desechable para la almohadilla para reducir los riesgos de contaminacin
cruzada y aumentar la comodidad del paciente.

Cubierta desechable
La cubierta desechable est diseada tanto para los lactantes pretrminos como para los que
nacen a trmino. Se coloca al lactante directamente encima de la almohadilla con la cubierta
desechable, sin usar ningn tipo de jacin para mantener al lactante sobre la almohadilla.
Chaleco desechable
Se ha diseado un chaleco desechable para asegurar la almohadilla broptica al lactante.
Con el chaleco desechable es posible sostener y alimentar al lactante mientras se sigue con el
tratamiento de fototerapia. Para los lactantes pretrminos y los que nacieron a trmino pero
no pueden tolerar el chaleco ajustado alrededor de la regin abdominal, se deber emplear la
cubierta desechable.
Transiluminador
El tubo de luz del transiluminador contiene bras plsticas que transmiten la luz desde el
reector hasta la punta.

Indicador de energa elctrica
1. Cuando el aparato est encendido, se ilumina el diodo que emite una luz verde (LED).

Lmparas no recomendadas
El uso de una lmpara que no recomiende ni distribuya Ohmeda para su sistema BiliBlanket
Plus High Output afectar el rendimiento del mismo y puede ocasionar daos en el reector, la
almohadilla broptica o el transiluminador.

montaje) con el n de colocar el reector cerca del lugar de tratamiento.
2. Verique que los oricios de ventilacin para la circulacin de aire en los lados del reector no
estn obstruidos. El ltro del aire no debe tener una cantidad excesiva de pelusa.

w No raye ni manche el extremo de la entrada de la luz del cable conector. No ponga objetos
alados o pesados sobre la almohadilla, chaleco o cable conector bropticos.
w Cuando se desconecta el cable broptico del puerto de entrada del reector, queda al
descubierto una supercie caliente. No introduzca los dedos ni coloque ningn objeto
extrao en el puerto de entrada del reector, est o no conectada la lmpara.
w Cubra los ojos del paciente cuando use el sistema de fototerapia BiliBlanket Plus

El lactante, junto con la almohadilla luminosa, pueden cubrirse o envolverse en la manta. El
lactante continuar recibiendo tratamiento de fototerapia efectivo siempre y cuando la seccin de
la almohadilla desechable cubierta que emite la luz est en contacto con la piel.
La funda desechable debe ser el nico material entre el lado de la almohadilla que emite luz y
la piel del lactante. Si la funda desechable se mancha, debe cambiarse por una nueva. La funda
desechable tambin debe cambiarse con cada paciente.

Importante: La almohadilla debe estar forrada con la cubierta o con el chaleco desechable tal y
como se ha descrito anteriormente. No use la almohadilla sin el chaleco desechable o la cubierta.

Monte el sistema BiliBlanket Plus High Output en un calentador de radiacin, una incubadora
o un soporte o colquelo de manera que est sobre una supercie plana y slida cerca del
beb. El reector puede colocarse de lado o derecho.

Asegure la cubierta o el chaleco alrededor del cable de la almohadilla con las lengetas
adhesivas. Vase la gura 2-1.

Coloque rmemente el conector del cable broptico en el puerto del reector. Asegrese
de que no haya presin o tensin en el cable. El cable se desconecta fcilmente del reector
como medida de seguridad para ayudar a evitar que el reector se vuelque cuando se estira
el cable ms de lo posible durante el tratamiento.

Coloque la almohadilla luminosa forrada en un colchn u otra supercie plana con el lado
de emisin luminosa (el lado sin etiqueta) hacia arriba. Coloque al beb con la espalda o el
abdomen directamente sobre el lado blanco de la almohadilla con el extremo de sta en los
hombros del beb y el cable a los pies del beb. Vase la gura 2-2. Asegrese de que:
la mayor cantidad de piel del lactante que sea prcticamente posible est en contacto
directo con la seccin de emisin de luz de la almohadilla (puede llevar paales)
no haya nada entre la piel del beb y la almohadilla luminosa que no sea la cubierta
desechable (se puede llevar ropa encima de la almohadilla).

El beb puede estar vestido o envuelto en una manta y continuar recibiendo tratamiento de
fototerapia ecaz siempre que la seccin luminosa de la almohadilla permanezca en contacto con
la piel (la luz proveniente de la almohadilla que penetra algunos milmetros en la epidermis es la
que produce la isomeracin de la molcula de bilirrubina). Usando el chaleco, es posible sujetar
y alimentar al beb a la vez que se contina el tratamiento.
Importante: Asegrese de que el rea mxima de iluminacin est en contacto con la piel del
paciente.

Limpie el exterior del reector con una esponja o pao humedecido con una solucin
de detergente suave. Pueden usarse soluciones acuosas que se usen a la vez como
desinfectantes para hospitales y micobactericidas.
No permita que los lquidos se ltren en la caja.
Seque la supercie del reector con un pao suave y limpio.

w No use un producto de limpieza que contenga fenol. Se ha comprobado que existe una
relacin entre los compuestos fenlicos y los niveles elevados de bilirrubina en los bebs.

Nota: No toque la lmpara de vidrio central ni la supercie de espejo con los dedos. La
contaminacin de la lmpara puede reducir el rendimiento de la misma. Si se toca la lmpara
durante la instalacin o si se notan manchas, limpie la lmpara con alcohol y squela con un
pao limpio y suave. Adems, tenga cuidado de no dejar que los cables de la lmpara toquen el
reector cuando est caliente.

Limpieza del ltro del ventilador
Se debe examinar visualmente el ltro del ventilador del lado del reector y se debe limpiar si

When sending equipment to the Ohmeda Medical Service Repair Center for service, clean
the equipment. Include the light source, pad and power cord. Package it securely in the
original shipping container (if possible) and include:
1. A letter describing in detail the difficulties experienced with the unit.
2. All warranty information. A copy of the invoice or other applicable documentation
must be included.
3. Purchase order number to cover the repair of any unit not under warranty. (Contact
the Ohmeda Medical Service Repair Center for details.)
4. Return address and bill-to information.
5. Contact person (name and telephone number) for operational inquiries.
The equipment should then be shipped prepaid to the Ohmeda Medical Service Repair
Center listed on the back cover of this manual.

Functional description
The BiliBlanket Plus High Output consists of the light source and the light-emitting pad with
a flexible cable. The light source can operate from 90-132 V or 180- 264 VAC power source
at either 50 or 60 Hz.
The light source consists of a lamp, light filters, a variable power supply and a cooling
system.
The pad assembly consists of a bundle of optical fibers with an inlet and coupler on one
end and the light pad on the other end. The inlet end is located at the focal point of the
light source. Light is transmitted through the length of the cable to the pad. The cable is
covered with a protective plastic outer jacket.
Using patented technology, the pads light fibers are woven into a flat configuration. The
design distributes light over the entire surface of the pad. Light intensity is determined by
the power of the light source, the length of the light cable and the size of the light pad.
The transilluminator light pipe contains plastic fibers which transmit light from the light
source to the tip.

Light filtering
The lamp is a tungsten halogen bulb with an integral reflector. A light filter positioned
between the bulb and the fiber optic bundle essentially restricts light output distribution to
the 400 to 550 nanometer range. This filtered light is focused on the end of the fiber optic
cable that transmits the light to the pad.
Power supply
The power supply in the BiliBlanket Plus High Output is a universal input (90-132 V~, 180264 V~ @ 50/60 Hz) 15V DC output supply. There is a potentiometer to allow adjustment of
the output voltage. This DC voltage is connected to the small PCB and the bulb circuit.
Air Flow Failure
The bulb circuit consists of the bulb, the source and drain of the field effect transistor (FET),
Q1, and the thermostat. When excessive heat opens the thermostat, the current is diverted
from the bulb to the Air Flow Failure LED. See Figure 4-8 for the printed circuit board
schematic.
Light output control
The small PCB consists of a light output intensity control circuit. The LM556 dual timer (U1)
provides a pulse train to the gate of Q1. Light output is controlled by the duty cycle of this
pulse train, which controls the on time of the bulb. The duty cycle is varied by the intensity
potentiometer (Brightness Knob). The photodiode, D2, turns on the hour meter when the
bulb is on. See Figure 4-8 for the printed circuit board schematic.
System temperature
Air flow from a DC fan cools the system. Incoming air is filtered to remove lint. There is an
over-temperature thermal cutout switch near the light source end of the fiber optic cable.
This thermal cutout switch monitors the temperature and shuts off the light source if the
cooling system malfunctions.

Replacing a fuse
Refer to figure 4-1.
To replace a fuse
1. Switch the light source off and disconnect the power cord from the power source.
2. Place the tip of a small flat head screw driver under the tab next the power switch on
the power receptacle, and pry the fuse door open.
3. To replace either fuse, place the screw driver tip between the end one of the fuse
holder and the power switch and pull out the holder.
Note: the arrows on the end of the fuse holder should match those on the inside of the fuse
door to ensure correct fuse polarity.

8. Install the replacement fan by reversing steps two through six. Use Loc-tite No. 222
on the four screws that secure the fan to the chassis. When installing the replacement
fan, replace any cable ties which were removed. Be careful about wire replacement
and fan orientation.
9. Perform the electrical safety procedures detailed near the end of this section.
10. Perform the checkout procedure detailed in section 2/Operation before placing the
BiliBlanket Plus High Output in service.

Replacing the thermal cutout switch
1. Disconnect the power cord.
2. Place the light source in its vertical position. Remove the four cover screws and
washers .
3. Gently slide the cover off, pulling it back away from the bezel, being careful not to
scratch the finished surfaces.
4. Place the light source in its horizontal position. The thermal cutout switch bracket is
toward the front of the light source (between the front bezel and the lamp).
5. Remove the mains wire harness (it connects the power supply to the PCB).
6. Remove the thermal cutout switch holder screw and washer (this can be best done
through the bulb access door). Slip the switch out of its holder and through the bracket
hole grommet.
7. Remove the connectors (P9 and P2, the long connector on the power supply board)
from their circuit board sockets.
8. Install the replacement thermal cutout switch by reversing steps two through seven
and replacing any cable ties which were removed. To mount the switch properly in the
holder, the clip should be located at the very base of the metal portion of the switch.
9. Perform the electrical safety procedures detailed near the end of this section.
10. Perform the checkout procedure detailed in section 2/Operation before placing the
BiliBlanket Plus High Output in service.

Replacing a bezel/hour meter
1. Disconnect the power cord.
2. Place the light source in its vertical position. Remove the four cover screws and
washers.
3. Gently slide the cover off, pulling it back away from the bezel, being careful not to
scratch the finished surfaces.
4. Place the light source in its horizontal position.
5. Remove the four screws that secure the fan and remove the fan.
6. Remove the two screws on the bezel and the screw on the mode select slide lever, and
remove the bezel.
7. Loosen the two set screws on the brightness control knob and remove the knob.
8. Remove the connector (P7) from the printed circuit board.
9. Remove the heat shrink that surrounds the potentiometer.
10. Remove the nut on the potentiometer and remove the potentiometer.
11. Remove connector (P8) from the PCB.
12. Remove the hour meter by removing its two mounting screws.
13. To reassemble, reverse steps 1 through 12.
14. Perform the electrical safety procedures detailed near the end of this section.
15. Perform the checkout procedure detailed in section 2/Operation before placing the
BiliBlanket Plus High Output in service.

Electrical safety procedures
Use approved equipment and techniques to test the units ground continuity and leakage
current.
Ground continuity
Use a low range ohmmeter or electrical safety analyzer to measure the resistance between the
ground pin on the line cord plug and exposed metal surface of the unit. The ground resistance
must be less than 0.2 ohm*.
Leakage current
Follow the test equipment manufacturers directions to verify:
1. Less than 300 microamperes at 100/120 VAC (less than 500 microamperes at 220/230/240
VAC) measured with the ground intact for normal and reverse polarity at any exposed
metal surface.
2. Less than 300 microamperes at 100/120 VAC (less than 500 microamperes at 220/230/240
VAC) measured with the ground wire open for normal and reverse polarity at any exposed
metal surface.
* If measured from the ground pin of the power inlet module on the unit, it must be less than 0.1 ohm.

Measurement Procedure
There are two methods to check the irradiance level a center point check, which provides
approximate radiance values, and the more comprehensive 6-point check, which provides
more accurate radiance values.
To perform the center point check, select maximum light output using the switch on the
BiliBlanket light source front panel, place BiliBlanket Light Meter at the approximate center
of the pad and observe the measured irradiance:
For the BiliBlanket High Output Pad the value should be > 53 W/cm2/nm
For the MaxXL Bilipad the value should be > 36 W/cm2/nm
If the meter readings correspond to these values, you may use the unit without performing
the comprehensive 6-point check.
If the meter readings are lower than these values, perform the six-point check to ensure
adequate irradiance levels. If the irradiance levels are still lower than these values, see the
troubleshooting section of this manual for low output, or have the unit serviced.
6 Point Check
1. Be certain the checkout procedure for the Ohmeda Medical BiliBlanket has been
completed.
2. Position the BiliBlanket pad on a flat surface with the light-emitting side up.
3. Switch on the BiliBlanket light source and allow to warm up approximately 15 minutes.
4. Install the template on the light pad as shown in Figure 4-2, step 1, making sure that
the pad slides all the way into the template. Align the standard size pad with the
straight line cut through the universal template. Align the MaxXL Bilipad with the edge
of the universal template.
5. Using the brightness selector switch on the light source front panel, select the high
light intensity setting.
6. Turn on the BiliBlanket Meter.
7. Set the Hold/Run switch to the Run (j) position. The radiometer now performs
continuous irradiance measurements.
8. Using the template, place the radiometer's receptor dome in contact with the surface
area of the Illuminator pad, as shown in Figure 4-2, step 2.
9. After a few seconds, press the Hold/Run switch push button to set it to its inward Hold
(J) position. This freezes the display reading at the current spectral irradiance level
received from the pad. The radiometer may now be removed. The measured spectral
irradiance data is shown in the display window. Repeat the measurement for the five
locations as shown in Figure 4-2, step 3. To obtain the average irradiance value, add
the five readings together (A+B+C+D+E) and the center reading again (E), then divide
the sum by six [(A+B+C+D+E+E)/6].

10. Switch off power to the BiliBlanket light source.
11. Switch off the radiometer power if no additional measurements are to be made.
Note: For the High Output pad, meter readings should be between 37.5 and 67.5 w/
cm2/nm. For the Max XL Bilipad readings should be between 26 and 47 w/cm2/nm.
Important: The Ohmeda Medical BiliBlanket Light Meter is calibrated with a traceable
tungsten lamp source. This calibration assures the accuracy of the meter with that
lamp standard. When measuring other types of light sources, the meter may not
accurately measure the specified spectral irradiance, as it is not calibrated for the
spectral distribution of that lamp. Refer to the recommendations provided by the
manufacturer on measuring technique and light meter specifications.
Over-range Error
If the measured irradiance exceeds the measuring range, "-0-" will blink to inform you that
an over range error has occurred.
With the Light Source set on the High, the transilluminator tip should be located 2 1/2
inches (6.35 cm) from the lux meter input. The reading should be 9000 +2250/-3150 lux.
