TO WHOM IT MAY CONCERN:
THIS IS TO CERTIFY that according to our daily records of events in the Police Blotter,
particularly in Page Nr. 0206, Entry Nr. 00730 Dated 03 October 2016, Timed 6:00 PM; the following date
exist, viz:
On or about 2:35 in the afternoon of October 3, 2016, this office received a report coming from a
concerned citizen named Kendie Bermoy, of legal age, single and a resident of Purok 3, Brgy. Bagacay,
Calbayog City, Samar, informing that a hit and run accident transpired along Magsaysay Extension of
Brgy. Bagacay, Calbayog City, Samar. Upon receipt of the said report, PNP personnel of this office
composed of SPO1 Ma. Theresa Liporado Canican, PO3 Glen Iosa Panis Comilang and PO3 Jeck
Calagos Rebucas, immediately proceeded to the given place of accident to verify the veracity of the said
report. Upon arrival thereat, responding policemen saw one female lying on the ground and one
motorcycle with Plate No. HL-1633. The witness, kendie Bermoy, informed the responding policemen that
the said motorcycle was hit by one White Mitsubishi Delica with Plate No. BED-712. Traffic investigation
was conducted by the undersigned investigator and disclosed that on or about 2:30 in the afternoon of
October 3, 2016, while a black motorcycle with Plate No. HL-1633 driven by Sheen Ann Morallo Calagos,
19 yrs. Old, single, a criminology student of NwSSU and a resident of Purok 2, Brgy. Cautod, Sta.
Margarita, Samar with Student Drivers License No. H0816S00470 is running south direction bound to
Sta. Margarita, Samar coming from Calbayog City Proper. Upon reaching Magssaysay Extension of Brgy.
Bagacay, Calbayog City particularly at the ongoing construction site of the road of said barangay, was
accidentally hit by White Mitsubishi Delica with Plate No. BED-712, running fast towards same direction
also. As a consequence of its impact, the motorcycle and its driver was thrown on the left side of the road
causing the driver to sustain an injury in the head which lead to her death.
Immediately thereafter, elements of this station was in hot pursuit for the driver of the vehicle
responsible for the said hit and run.

Investigation Report Re-Reckless Imprudence resulting to Homicide

I.

MATTERS INVESTIGATED:
1. To determine the facts and circumstances surrounding the hit and run
accident that transpired on or about 2:30PM of October 3, 2016 at Purok 3,
Brgy. Bagacay, Calbayog City, Samar involving one motorcycle driven by
Sheen Ann Morallo Calagos, 19 yrs old, single, a Criminology Student of
NwSSU and a resident of Purok 2, Brgy. Cautod, Sta. Margarita, Samar and
one White Mitsubishi Delica driven by an unknown driver.

II.

FACTS OF THE CASE:


2. On or about 2:35 in the afternoon of October 5, 2016, one Kendie Bermoy y
Omila, of legal age, single and a resident of Purok 3, Brgy. Bagacay,
Calabyog City, Samar, called this office through cellular phone and informed
that a hit and run accident transpired just minutes ago along Magsaysay
Extension, Brgy. Bagacay, Calbayog City, Samar, particularly at an ongoing
construction site along Purok 3 of said barangay. Upon receipt of the said
report, PNP personnel of this office composed of SPO1 Ma. Theresa
Liporado Canican, PO3 Glen Iosa Panis Comilang and PO3 Jeck Calagos
Rebucas immediately proceeded to the given place to verify the veracity of
the report;
3. That upon arrival thereat, responding policemen saw a female victim lying on
the ground and one motorcycle with Plate No. HL-1633
4. Investigation conducted by undersigned investigator disclosed that on or
about 2:30 in the afternoon on October 3, 2016, one motorcycle with Plate
No. HL-1633, driven by Sheen Ann Morallo Calagos, 19 yrs old, single, a
Criminology student of NwSSU and a resident of Purok 2, Brgy. Cautod, Sta.
Margarita, Samar with Student Drivers License No. H0816S00470, while
running south direction bound to Sta. Margarita coming from Calbayog City
Proper, upon reaching along Magsaysay Extension, Calbayog City, Samar,
particularly along Purok 3 at an ongoing construction site where a tenwheeler truck was parked on the right side of the road, the said motorcycle
was hit by one White Mitsubishi Delica with Plate No. BED-712 causing the
motorcycle and the driver to be thrown on the left side of the road. The victim
was found dead on the spot because of the sustained injury in the head.
5. Witness Kendie Bermoy when interviewed averred that on or about 2:25 in
the afternoon of October 3, 2016, it was raining hard. While she is in front of
their house, waiting for a ride to go to Calbayog City Proper, she saw a
motorcycle approaching south direction and one White Mitsubishi Delica on
the back running towards the same direction. Upon reaching more or less 10
meters from her, she saw the White Mitsubishi Delica accidentally bumped
the motorcycle. As a result, the motorcycle and its driver was thrown on the
left side of the road, while the Mitsubishi Delica with Plate No. BED-712 fled
away from the place. She immediately rushed to the victim and then called
the Police Station.

III.

DISCUSSION:
6. That the undersigned investigator finds out that the driver of the motorcycle
do not possess or equip with standard qualification required by law to drive.
The victim is not wearing a helmet. The weather condition and the speed of
the White Mitsubishi Delica ignited the accident.

IV.

CONCLUSION:
7. The accident sparks when the driver of the White Mitsubishi Delica is on its
full speed and due to heavy rain, the road was wet and slippery, the driver
was not able to control the vehicle until it accidentally hit the motorcycle.

V.

RECOMMENDATION:
8. This office is convinced that a crime of Reckless Imprudence resulting to
Homicide should be filed against the unknown driver of the White Mitsubishi
Delica with Plate No. BED-712.
