Notes: BrynnLe played with the baby doll by washing it in the tub and pushing it down the slide. BrynnLe sang Tiny Tim the Turtle with Mallory and did the actions
to the song with Mallory, too. BrynnLes mom brought out candies and helped BrynnLe scoop them with a spoon using full and partial physical assists.

Impression:
BrynnLe was happy and cheerful today. She saw the baby bath and signed for and said bath. BrynnLes personality has been more evident as
she has vocalized and spoken more. She is walking everywhere without prompting and is able to more fully express herself because of this
independence.
Objective Description of Data and Observations:
BrynnLe spontaneously said out when taking the baby out of the case. BrynnLe responds positively when prompted to say words that she is
familiar with. When prompted through a model to use the spoon to scoop a candy she was not completely successful. When prompted with more
intrusive acts including physical assists BrynnLe responded positively. Before these physical assists from her mom, BrynnLe attempted to use the
spoon to scoop spontaneously (on her own) with a close approximation to success. BrynnLe did not respond positively when prompted verbally to
suck from a straw. On her own and spontaneously, BrynnLe would suck from a straw with complete success. BrynnLe walks independently and
spontaneously the majority of the session. When prompted verbally to walk, BrynnLe would respond with success half of the time. When singing
Tiny Tim the Turtle with Mallory, BrynnLe sang along with mumbles and signed the actions including eat, turtle, drink, bubbles in his
throat, and bubble, bubble, bubble.
Assessment and Evaluation:
BrynnLe is largely successful at when doing so spontaneously. BrynnLe does not consistently respond positively when given verbal prompts. This
may be because she does not understand what we are wanting her to do or she simply wants to do that on her own time. For example, BrynnLe
will not always drink when we ask or tell her to through a verbal prompt, but will drink when she is thirsty and expresses a desire to drink by
saying and signing juice. BrynnLe may not always respond positively to verbal prompts to say or sign a word because she may not originally
know the word. She is more successful when prompted to say a word she already knows. When Mrs. Larsen modeled for BrynnLe using the spoon
to scoop the candies, BrynnLe attempted to scoop with the spoon on the second prompt. She then spontaneously attempted to scoop using the
spoon because she knew her mom expected her to use the spoon to eat the candy. When BrynnLe tried to eat the candy without the spoon, the
majority of the time her mom would take the candy and say: No, use the spoon to eat your candy. Scoop. BrynnLe seemed to want to use the
spoon to please her mom and attempted to use it spontaneously following her mothers instruction. When assisted physically, BrynnLe was
completely successful. After being completely successful using the spoon to scoop a candy and eat it, BrynnLe attempted to use the spoon to
scoop another candy independently and spontaneously and was successful! Our level of intrusiveness was appropriate for all these activities.
BrynnLe needed physical assists to learn how to scoop the candy with the spoon and then was successful independently. BrynnLe is successful
when verbally prompted to say or sign words when singing a song. Cassie and I worked well together to provide intervention to BrynnLe and to
record the notes and data for analysis. BrynnLes goals are appropriate as the % correct based on IEP criteria are near her zone of proximal
development. BrynnLe needs further help to learn to respond positively when prompted verbally. Our activities were appropriate because BrynnLe
is familiar with the routine of washing her baby, pushing baby dolls down her slide, and loves to sing songs and do the associated actions.
Plan:
For our last intervention, we will continue to work on BrynnLes goals of sucking from a straw, walking 7-10 steps, using a spoon, and saying or
signing 50 words. BrynnLe has been very successful in these goals. We will continue to work with her on responding positively when only given a
verbal prompt. We will play with the baby doll in the bath, push her down the slide, sing, move, and dance to songs, and scoop candies with the

spoon. We will also prompt her to say and sign words throughout the entire intervention session.
