APPENDIX I SAFE WORK PRACTICES

1. Site Preparation Work


1.1 Verify the tank location with production team prior to start work.
1.2 Ensure work permit is issued for any maintenance work to be carried out.
1.3 Ensure the tank must be emptied and cleaned.
1.4 Ensure electrical source is isolated and signage is displayed.
1.5 Ensure working area is barricaded and signage is displayed.
1.6 PPE Matrix for PPE requirement for each task/activity as per below.

1.7 All isolation points with marking in P&ID should be given by related department (if
necessary).
2. Confined space entry

2.1 Operators who are to enter a confined space are properly instructed, and trained in
the use of protective equipment, if applicable.
2.2 Ensure good ventilation system is provided continuously. Use blower to suck and
blow air (at least 2 units of blowers).
2.3 Ensure sufficient lighting is provided prior to start work.
2.4 Ensure proper arrangements of electrical cables and hoses inside confined space
entry.
2.5 Beware of protruding structures, holes on the working platform. Check and review
the equipment layout / design prior to entry. Standby life line and entrant shall wear
hardness.
2.6 Ensure standby person / manhole watcher standing by at manhole during the entry.
2.7 Confined space entry log shall be maintained by stand by person to record the head
count inside the confined space.
3. Hot work (welding, cutting, etc.)
3.1 Only competent workers are allowed to use welding equipment.
3.2 All sparks should be contained by the use of sufficient fire blankets and the work
area should be barricaded with proper signage Hot Work in Progress.
3.3 Fire extinguisher should be available and accessible at working area at all times.
3.4 Check and confirm all welding set, cutter or grinding machine, welding cable is free
from defects. Isolation switches are in good condition. ELCB is working.
3.5 To properly blind flange connection for pipeline modification before hot work.
4. Working at Height and Scaffolds
4.1 Fall protection (safety harness) is required when working above 2 meters except
permanent working platform with complete railing.
4.2 Scaffolding equipped with ladder or staircase must be constructed and used as
temporary access or working platform.
5. Emergency Procedure
5.1 When any emergency case happens, standby person shall inform supervisor
immediately.
5.2 Activate ERT team to rescue, fire evacuation.
6. Housekeeping
6.1 Ensure all tools / material is removed from ATT upon work completion.
6.2 To clear all debris and waste from working area.
6.3 After housekeeping is completed, PTW must be returned to Production team for
closing.
