Introduction

Physical Fitness is a very common and non-invasive method of


treatment used in Therapeutic recreation. It helps patients improve their
flexibility, strength, and range of motion, cognitive skills, coordination and an
overall functional and mobile independence. Patients who have had an
accident, sports injury, or any other physical disability due to other acute
incidents or chronic diseases may benefit from any kind of physical fitness
according to their need. Some disabilities we treat are Spinal Cord Injury
(SCI), Traumatic Brain Injury (TBI), Neurological disabilities and more. Active
exercise therapy involves movements that are done within the patients own
power and ability. This includes leg raises, jumping on a trampoline, walking
on a treadmill or performing lateral arm raises. All these examples are
exercises performed with the patient using their own power to move and
enhance their functional skills. (Therapeutic Health Organization Blog, 2015).
Based on patients disability and needs interventions can include,
cardiovascular training, strength, aerobics, endurance training and interval
training to achieve clients overall independence. Two examples of how
outcomes can be achieved for our clients are through aqua fits and strength
training. The following pages will give more details about the Specific
Program Design for physical fitness.

Implementation Description
Population
The intended population ranges from 18 years old to elderly. The main population this
program treats is individuals with neurological impairments which include injuries that originate
in the nervous system. These are known to be injuries such as Spinal Cord Injury (SCI) and
traumatic brain injury (TBI). Spinal cord injuries involve trauma to the spinal cord that creates
disturbance of either motor function or sensation. Spinal cord injuries can be paraplegia or
quadriplegia, also known as tetraplegia. Paraplegia is injury to the sacral, lumbar, or thoracic
areas of the spinal column. Quadriplegia is injuries between the cervical areas of the neck and
beginning and thoracic. Spinal injuries can be classified as complete or incomplete. Traumatic
brain injury (TBI) is a physical insult to the brain that may cause problems with physical,
emotional, and social functioning (Datillo, 2012, p. 448).Another disability we may treat is
Cerebral Vascular Accidents (CVA), which is when the brain does not receive the correct blood
flow and usually causing a stroke to either the right or the left brain. In addition, we also treat
Guillain-Barr Syndrome (GBS), which is a disorder in body's immune system that attacks part
of the peripheral nervous system. Symptoms of this disorder are high degrees of weakness or
tingling sensations in the legs. In GBS, reflexes such as knee jerks are usually lost. To achieve
clients outcomes the activities are based on their needs. For this population their needs are
mostly physical, cognitive and behavioral approach. Clients seek to gain back skills that have
been lost or reduces in order to fit into their everyday life style and accomplish regular tasks.

Program Purpose
The exercise and fitness program provides a variety of physical activities and workouts to
enhance individuals overall health, specially their physical and functional activities. Patients
will have the ability to achieve and obtain knowledge to overcome regular environmental
barriers that they may struggle with due to their physical or functional limitations. Clients will
learn how to use gym equipment and other materials they may use in their day to day lifestyle.
Clients Benefits include improved physical adeptness, increased strength and flexibility,
improved physical fitness and health, improved athletic prowess, improved coordination. A plan
is created with a variety of activities according to the individuals interest, ability, and current
leisure and fitness awareness.
Program Length
This program is designed to have 8 sessions of each consisting of 30 to 45 minutes of
physical training. Each session will be divided into a warmup, intense or slow activity and a cool
down. Client should participate of these sessions 2 times a week. This time period would help
clients strengthen their muscles, joints, and whole body in general in order to obtain regular
functional skills.

For the group intervention, we utilize facilities that will provide a specious, save and
clean environment. Depending of the session three of the facilities used are the gymnasium, the
open basketball courts and the fitness center.

Staff

The staffs in charge of conducting these fitness activities are two Certified

Therapeutic Recreational Therapists. He/ She must obtain a National certification. Prior to
the Certification they need to have obtained a Bachelors degree in Therapeutic recreations or
any exercise degree from a 4 year University. (www. Nctrc.org). In addition our staff must
maintain current continuing education units to maintain their certification up to date. There
are opportunities for one volunteer to come and help our CTRS with the physical fitness
activities. Also, interns are welcome to join us and observe the activities and have hands on
experience during their learning to achieve a degree.

Evaluation

The evaluation conducted provides systematic information from which specialists


can improve both the program delivery and the outcomes. Both program and client
evaluations determine if all goals were achieved. The physical fitness program
evaluations involve formative and summative information obtained in the intervention.
The main purpose of both evaluations is to see the outcomes achieved and make our
program better to fit participants needs, make changes and provide quality service.
Patients are observed in the course of the innervations and Therapists are responsible to
obtain information and accurate data in order to obtain a precise evaluation.

Program Evaluation

As a formative program evaluation, APPENDIX A shows the Post- session report


Form. Therapists are responsible to fill each section accurately. In addition, APPENDIX
B shows the summative program evaluation that contains Post session Analysis Form
which helps specialist gather past information from interventions and created a summary
of the data obtained. These forms will help the program find out areas of improvement to
provide the best quality of service. Evaluation forms give key point of each intervention
assessed, it helps staff identify areas that might need improvement with clients or if
activities are adequate for participants needs. Program evaluation will be conducted at the
end of the last session once all sessions have been completed.

Client Evaluation

Client Evaluations are conducted individually after each session. This evaluation
is important due to the fact that specialist can gain knowledge of their clients progress
and outcomes achievements. The clients are required to meet most of the performance
measures of the criteria stated in the enabling objectives for each session. In this client
evaluation the information gathered will include summaries, treatments, any program
plans, notes, goals, goals achieved and discharged/ referral summaries. Also, clients will
be evaluated in their overall physical activity such as cardio, strength, endurance and
flexibility. APPENDIX D is an example of the flexibility assessment performed before
and after each intervention; they will have a pre and post assessment to determine goals
and see what outcomes were accomplished. From this reports if there is any modification
that needs to be done in order for the client to achieve the goal then it will be done. A

performance sheet (APPENDIX C) will be used to determine client criteria with enabling
objectives from each session.

The Program Plan

Statement of Purpose: To provide activities in which clients acquire, improve and


utilize physical and cognitive skills that can be used within leisure context and daily
activities through fitness training interventions.

Program Objectives: Each Terminal Program Objectives (TPOs), Enabling Objectives


(EOs) and Performance Measures (PM) are developed and created based on clients goals
and objectives.

Content and Process

The following tables develop corresponding content and process descriptions for
each of the enabling objectives outlined in the program plan. Sessions are numbered one through
nine for each EO. Appendix E will give activity outline and description for each session. All
activities can be modified to the need of the participant. Special equipment can also be used in
order for participants to be able to perform the exercises and achieve each objective.

Session 1 Why should I do it?

Purpose: The purpose for this session is to help facilitate participants functional
activity by enhancing participants knowledge on the importance of physical activity.

TPO 1: To demonstrate the knowledge of physical activity for leisure involvement.

To break the ice Who are you? is an
introduction activity where each participant
will be given a number. The instructor will
be the first one to start. He/ she will give a
small introduction about them. (NAME,
AGE, REASON FOR BEING IN THE

that enhances team communication and
its a fun activity with funny questions

machine and materials provided. The
instructor will ask some question to test
participants knowledge on this type of
facilities. The specialist will show the
exercises for this session and explain
any modifications, rules and concerns.

Activity 2 I can do it too

Purpose: The purpose of this session is to demonstrate participants that


there are no limitations for them to be able to participate in a physical
fitness activity.

Setting: Open Space outdoors or indoors if climate does not allow

Description

The session will begin with a small introduction of each adapted sport presented
in this session and each material used. After, we will start with a small warm up. The instructor
will explain the exercises to the participant and arrange any modification that needs to be done.

Before starting the exercised we will do a short warm up to move our bodies.

Have teams of two in each station.

1. Basketball and hoops will bounce the ball three times and try making it to the hoop.
Participants can do it standing up or sitting on a chair.

Purpose: The purpose of this session is to demonstrate participants that there are no
limitations for them to be able to participate in a fitness class and be part of a gym to
enhance their skills and increase leisure activities.

Setting: Fitness Center

Description

The session will begin with a small introduction of the fitness facility, the
importance and the benefits of being part of if. The instructor will introduce the machines and
equipment presented in this session. After, we will start with a small warm up. The instructor
will explain the exercises to the participant and arrange any modification that needs to be done.
They will use a resistance ball to work on balance and strengthen they back. Last part of the
activity is yoga stretches. This is a continuation of session 2 but the purpose of this session is that
participants experience new equipment and a new environment where they can build confidence
and know they are able to participate and be part of this facilities outside their intervention
session.

1) We start with a warm up in the treadmill. The video shows an example of how the clients will

2) Resistance ball This is a basis exercise char used for this intervention to choose 2
or three exercise that will help participants obtain balance and strengthen their backs.

APPENDIX IV

Activity 4 Super Muscle

Purpose: The purpose of this session is to help facilitate participants functional ability
to participation in any leisure activity by enhancing their strength and coordination.

Kettlebells

Chairs

Setting: Gymnasium

Description

This session will begin with an introduction to the importance of doing weight
training and what are the benefits of it. After, the therapist will show participant a
learning video The Benefits of Strength Training
https://www.youtube.com/watch?v=A0tY3vmZL08. This is a video that will
challenge clients and teach them why they are here. After the video the instructor will
introduce the four station set and what they will do in each station. Participants will be

working on upper body, lower body, back/ abdominals and free exercise. At the end we
will do a cool down/ stretches to relax our bodies.
Therapist will work with participants to find out with is the best weight for them in each
station.

Station 1
Upper Body
Materials: dumbbells, chairs

Participants will perform:


Note- Can be done standing or sitting

1. 2 sets of 8-10 bicep curl

1. Stand up straight with a dumbbell in each hand at arm's length. Keep your elbows
close to your torso and rotate the palms of your hands until they are facing forward.
This will be your starting position.
2. Now, keeping the upper arms stationary, exhale and curl the weights while
contracting your biceps. Continue to raise the weights until your biceps are fully
contracted and the dumbbells are at shoulder level. Hold the contracted position for a
brief pause as you squeeze your biceps.
3. Then, inhale and slowly begin to lower the dumbbells back to the starting position.
4. Repeat for the recommended amount of repetitions.

Variations: There are many possible variations for this movement. For instance, you can
perform the exercise sitting down on a bench with or without back support and you can
also perform it by alternating arms; first lift the right arm for one repetition, then the left,
then the right, etc.

2. 2 sets of 8-10 Lateral arm raises

1. Pick a couple of dumbbells and stand with a straight torso and the dumbbells by your side
at arms length with the palms of the hand facing you. This will be your starting position.
2. While maintaining the torso in a stationary position (no swinging), lift the dumbbells to
your side with a slight bend on the elbow and the hands slightly tilted forward as if
pouring water in a glass. Continue to go up until you arms are parallel to the floor. Exhale
as you execute this movement and pause for a second at the top.
3. Lower the dumbbells back down slowly to the starting position as you inhale.
4. Repeat for the recommended amount of repetitions.

Variation: This exercise can also be performed sitting down.

3. 2 sets of

8-10 Triceps
Extension

1. To begin, stand up with a dumbbell held by both hands. Your feet should be about
shoulder width apart from each other. Slowly use both hands to grab the dumbbell
and lift it over your head until both arms are fully extended.
2. The resistance should be resting in the palms of your hands with your thumbs around
it. The palm of the hands should be facing up towards the ceiling. This will be your
starting position.
3. Keeping your upper arms close to your head with elbows in and perpendicular to the
floor, lower the resistance in a semicircular motion behind your head until your
forearms touch your biceps. Tip: The upper arms should remain stationary and only
the forearms should move. Breathe in as you perform this step.
4. Go back to the starting position by using the triceps to raise the dumbbell. Breathe out
as you perform this step.
5. Repeat for the recommended amount of repetitions.

Station 2
Lower Body
Materials: dumbbells, chairs

Participants will perform:

1. 2 sets of 10 Squats

1. Position your legs using a shoulder width medium stance with the toes slightly
pointed out. Keep your head up at all times and also maintain a straight back. This
will be your starting position.
2. Begin to go down by bending the knees and hips as you maintain a straight
posture with the head up. Continue down until the angle between the upper leg
and the calves becomes slightly less than 90-degrees. Inhale as you perform this
portion of the movement. Tip: If you performed the exercise correctly, the front
of the knees should make an imaginary straight line with the toes that is
perpendicular to the front. If your knees are past that imaginary line (if they are
past your toes) then you is placing undue stress on the knee and the exercise has
been performed incorrectly.
3. Begin to go up as you exhale by pushing the floor with the heel of your foot as
you straighten the legs again and go back to the starting position.
4. Repeat for the recommended amount of repetitions.

2. 2 sets of 10 Leg Extension on each leg with ankle weigh sitting down

1. Choose your weight and sit on the chair feet pointed forward and the hands
holding the sides. This will be your starting position. Also, make sure that
your legs form a 90-degree angle between the lower and upper leg. If the
angle is less than 90-degrees then that means the knee is over the toes which
in turn creates undue stress at the knee joint. If the machine is designed that
way, either look for another machine or just make sure that when you start
executing the exercise you stop going down once you hit the 90-degree angle.
2. Using your quadriceps, extend your legs to the maximum as you exhale.
Ensure that the rest of the body remains stationary on the seat. Pause a second
on the contracted position.
3. Slowly lower the weight back to the original position as you inhale, ensuring
that you do not go past the 90-degree angle limit.
4. Repeat for the recommended amount of times.

3. 2 sets of 10 Calf Raises with ankle weights

1. Position your toes facing forward. Push up by extending your hips and knees until your
torso is standing erect. The knees should be kept with a slight bend; never locked. Toes
should be facing forward, outwards or inwards. This will be your starting position.
2. Raise your heels as you breathe out by extending your ankles as high as possible and
flexing your calf. Ensure that the knee is kept stationary at all times. There should be no
bending at any time. Hold the contracted position by a second before you start to go back
3. Go back slowly to the starting position as you breathe in by lowering your heels as you
bend the ankles until calves are stretched.
4. Repeat for the recommended amount of repetitions.

Note: Hold onto a chair, wall, or bar to obtain better balance

Station 3
Back/Abdominals
Materials: Mat

Participants will perform:

1. 2 sets of 8-10 Back Extensions

1. Lie face down on a mat


2. With your body straight, cross your arms in front of you or behind your head. This will be
your starting position.
3. Slowly raise your torso back as you inhale. Tip: Do not swing the torso at any time in
order to protect the back from injury.
4. Repeat for the recommended amount of repetitions


2. 2 sets of 8-10 Curl up

1. Lie flat on your back with your knees bent and feet flat on the floor,
hip width apart
2. Place your hands slightly either side of your head, do not lock your
fingers or pull the head up
3. Push the small of your back into the floor to engage the abdominals
4. Tilt your chin slightly, leaving a few inches space between chin and
chest
5. Begin to roll your shoulders off the floor
6. Your shoulders should leave the floor by about 4 inches and your
lower back should always remain on the floor
7. Hold for a moment at the top then slowly move back down


3. 2 sets of 8-10 on each side Arm and leg raise

1. Sit on your hands and knees on an exercise mat.


2. The hands are directly beneath the shoulders. The knees are directly beneath the
hips.
3. The core is tight and the spine is neutrally aligned.
4. Extend one leg and lift the opposite arm until they are in line with the body.
5. Return to the starting position and repeat for the prescribed number of repetitions.
6. Switch sides.

Station 3
Free Exercise
Materials: Mat, kettlebells , dumbbells

Participants will perform


1. Any exercise they wish to perform with materials provided. They can also choose

Purpose: The purpose of this session is participants learn and perform full range of
motion moves using machines in a fitness facility to strengthen their muscles.

Setting: Fitness Facility

Description

The session will begin with an introduction to the multi station machine. The
instructor will use a video to show participants how to use the machine and what are
some common exercises done with it. After all instructions and rules have been given we
will begin with a 10 min warmup in the static bikes. Once 10 min have passes by,
participants will choose one of the sides of the machine. They can chose to work arms,
legs, back, ect. Participants do 3 sets of 8 and rotate to the next side until they have
completed all the repetitions and machine sides.

Note: Always remind Participants about breathing techniques

Therapist will test participants strength to set the appropriate weight level for each
exercise. This is done to avoid injuries and protect each participant. Also, it is important
in order for participant to perform each exercise with the correct range of motion.

The session will end with a small cool down and stretches

Activity 6 Imagination Jog

Purpose: The purpose for this session is to help facilitate participants functional
activity by enhancing participants cardiovascular system and gain knowledge of the
importance of cardio exercise.

Equipment:

None

Setting: Open Space Outdoors

Description

This session will start with a small introduction to the cardiovascular system t
teach participants the importance of doing any kind of cardio activity. This activity is a basic
exercise but works well for getting the group excited for events to come and gets them physically
warmed up. Walking, Jogging, Run, the leader will take the group through an imaginary trip.
This is a great exercise because it can change every time that you do the exercise. While walking
around the open area, Therapist will call out a verbal cue to do a certain physical activity.
Examples are when it is said log, we pretend that we jump over a log, and jump as high as
possible. When it is said duck, we pretend that we duck under a branch. You can use any physical
activity that you choose, just use your imagination to tie it into the story. After 10 minutes of

walking. begin to jog at a very slow pace. Using the same concept of imagination cues we will
jog for 10 minutes and then go into slow running for 10 min. The only limit that you have is your
imagination. Have the group come up with their own cues too.

Cardiovascular fitness enhances the ability of the heart and lungs to supply oxygen-rich
blood to the working muscles.

Cardiovascular fitness enhances the muscle's ability to use this oxygen to supply
adequate energy for movement.

What Activities Enhance Cardiovascular Fitness?

Aerobic exercise can enhance cardiovascular fitness. The words aerobic exercise defines
any type of rhythmic, continuous activity that involves the simultaneous use of multiple
large muscle groups. Aerobic activities challenge the heart and lungs, thereby causing
them to work harder than at rest. When performing cardiovascular fitness activities, your
pulse must be in a target heart range, which is usually calculated by subtracting your age

from the number 220 and then calculating 60 to 85 percent of that number. Examples of
aerobic exercises include:

Running or jogging

Brisk walking

Cycling

Swimming

Aerobic dance

Cross-country skiing

Snowshoeing

Activity 7 Kick the Ball

Purpose: The purpose for this session is to help facilitate participants functional
activity by enhancing participants coordination and motor skills.

Equipment:

Soccer Ball or dodge ball

Setting: Gymnasium

Description

This session will begin with a small ice breaker activity to make the session fun
and enjoyable. After, the therapist will introduce Kick the Ball Activity.

Kick the Ball

Participants will be partner up in a group of 2


Note: Participants can do activity standing or sitting down
1. One participant will be on the red line, the other participant will be on the blue line
standing in from of each other.
2. The participant standing in the red line will have the soccer ball while the participant
on the blue line is standing straight.

3. The participant with the soccer ball will through it to his/ her partner
4. Participant in the blue line will kick the ball for the other participant to catch it
5. They will repeat this 5 times then turn roles. Now, the one kicking will throw and
catch.
6. After each has done it 5 times, the participants on the red line will move to their right
and perform the same exercise with their new partner.
7. Continue to do this exercise until participants on the red line have worked with all
participants in the blue line.

Optional Challenge: Participant who throws the ball can move one step back in every turn
theey throw.

Activity 8 Tennis Time

Purpose: The purpose of this session is to facilitate participant functional abilities to


participation in leisure activities by increasing fine motor skills through fitness activity

Equipment:

Tennis Racket
Tennis Balls

Target Number: Group of at least 3-5people


Setting: Gymnasium

Description

The session will begin with an introduction to the importance of developing fine
motor and coordination skills. Therapist will teach participant why these skills need to be
developed in order to perform regular daily tasks. After the participant will introduce the

activity and explain how tennis can help them develop fine motor, coordination and focus
skills.
This activity will consist of two sessions: First part will consist of participant
experimenting playing with the tennis racket and a tennis ball individually. Second part
consists of all participants playing together. In addition before beginning this activity
Therapist will introduce the materials. He will teach participants how to grab the racket
and how to swing it to hit the ball.
To start this activity will do a dynamic warm up.

Part 1

Participant will be given a Racket and a tennis ball. Against a wall they
will hit the ball back and forth trying not to miss the ball. They will
begin close to the ball and after 5 hits they will move three steps back.

Repeat this for 10- 15 minutes

Part 2

Participants have now gained some experience on how to hold the


racket and how to hit the ball we will do an interaction game.
Participant will do a circle giving each other enough space so they
would not hit each other. Therapist will start serving the ball then
participant will hit the ball trying to keep it inside the circle.
Participants will hit the ball when it comes their way using the
techniques learned from the individual activity.

Activity 9 You Choose

Purpose: The purpose of this session is to facilitate participant functional abilities to


participation in independent leisure activities obtaining overall good physical fitness.

Equipment:

Any Available equipment participant will want to use.

Target Number: Group of at least 3-5people

Setting: Fitness Facility

Description

This session will begin with an introduction of the importance of maintain a


healthy and active physical fitness lifestyle. Therapist will also explain how maintaining a
good overall physical fitness helps individuals develop and enhance many skills needed
to perform day by day task and to have th ability to join any type of leisure activity.

This session will consist of giving participant the opportunity to choose machines
and materials they would like to use to enhance their overall physical fitness. They need to
choose machines and materials they will increase their motor, coordination skills, strengthen
muscles and cardiovascular. After attending past session participants should be familiarized in
what some exercises they can perform are based on their need. Therapist will oversee and help
any participant.

Before we start any exercise we will do a dynamic warmup to prepare muscles


and joints for the exercises that will be performed.
