LIABILITY WAIVER; INDEMNIFICATION AND HOLD HARMLESS AGREEMENT

RELATED TO MY CHILDS PARTICIPATION IN THE MOVIE WITH CHILDCARE EVENT

WITH ALAMO DRAFTHOUSE CINEMAS, LLC
The undersigned, hereby acknowledges and agrees that his and/or her childs participation with Alamo
Slaughter Lane, Ltd. and Alamo Drafthouse Cinemas, LLC and/or its affiliates (Alamo) in activities on,
around or in connection with the movie with childcare event at the Alamo Drafthouse Cinema located at
5701 W. Slaughter Lane, Austin, TX (hereinafter referred to as Activity) is strictly voluntary.
In consideration of the opportunity for my child to participate in the Activity and other good and valuable
consideration, the undersigned hereby releases, acquits and forever discharges Alamo, its employees,
agents, affiliate corporations and entities and other persons, firms, companies or corporations to whom or
for whose conduct Alamo may be liable, from any and all consequences, claims, demands, and causes of
action of whatsoever nature, including wrongful death and survival causes of action, whether in contract
or in tort, or arising under or by virtue of any statue or regulation, personal injuries, property damages and
from any and all other losses and damages of any kind, including, but not limited to, loss of consortium,
ensuring death, exemplary and punitive damages, which may have accrued or may ever accrue to me or
my child, or to my heirs, executors, legal representatives, successors, or assigns or on account of injuries
or damages arising out of participation in the Activity.

IT IS FURTHER UNDERSTOOD AND AGREED BY THE UNDERSIGNED THAT IN

CONSIDERATION OF MY CHILDS OPPORTUNITY TO PARTICIPATE IN THE
ACTIVITY AND OTHER GOOD AND VALUABLE CONSIDERATION, I DO HEREBY
AGREE TO INDEMNIFY AND HOLD HARMLESS ALAMO, ITS AGENTS,
AFFILIATE CORPORATIONS AND ENTITIES, OFFICERS OR DIRECTORS OF AND
FROM ANY AND ALL DAMAGES, INCLUDING, BUT NOT LIMITED TO,
INTEREST, COURT COSTS, ATTORNEYS FEES, AND OTHER EXPENSES THAT
EITHER ALAMO OR THE OTHER ENTITIES OR PERSONS RELEASED HEREIN
MAY INCUR OR BECOME LIABLE FOR AS A RESULT OF MY CHILDS
PARTICIPATION IN THE ACTIVITY AND ANY LAWSUIT WHICH MAY BE FILED
IN CONNECTION THEREWITH WHICH MAY HEREAFTER BE ASSERTED
AGAINST ALAMO OR THE OTHER PARTIES RELEASED BY, THROUGH, OR
UNDER ME, INCLUDING BUT NOT LIMITED TO CLAIMS ASSERTING OR
ARISING OUT OF ALAMOS NEGLIGENCE.
I authorize Alamo to provide or obtain emergency medical attention for my child in the event of sickness
or injury. I realize and understand that my family insurance policy will be responsible for any accident or
medical claim. Should my child require special medical treatment, prescriptions, or hospital care, I am
responsible for all expenses.
THIS DOCUMENT IS INTENDED TO BE A BINDING LEGAL CONTRACT. THIS DOCUMENT IS
GIVEN FOR MYSELF AND ON BEHALF OF THE MINOR MEMBERS OF MY FAMILY, IF ANY.
IF ANY POSITIONS OF THIS WAIVER ARE HELD TO BE INVALID, I AGREE THAT THE
REMAINING TERMS SHALL CONTINUE TO BE IN FULL LEGAL FORCE AND EFFECT.
I hereby certify that I have reviewed and understood all guidelines concerning my childs participation in
the Activity.
