Self-Assessment
I found The Healthy Lifestyle Questionnaire from the text book, Concepts of Fitness and
Wellness: A Comprehensive Lifestyle Approach, 7/e. I enjoyed their assessment because it was
based on your own perception, thus engaging the mind in development. In my opinion when a
questionnaire is based on your own perception, it gives the person the opportunity to truly
engage in reflection of ones self. Therefore, I decided to base my own assessment off of the one
in the textbook. The assessment I created tested each of the six aspects of health, all completed
by deep self analyst and honesty. In my findings I found it very relevant to my workplace,
community, and personal well being, because it allows you see which areas you may be lacking
in and areas of improvement. It also shows you where you are excelling, and encourages
reflection on why you are excelling in that area; thus allowing you to take those strategies of
optimal health and transfer it to the other areas in which need improvement.
According to my assessment, the areas in which I can improve is my physical and
emotional/mental health. I can improve on my physical/mental health by limiting to complete
extinguishing some of the nasty habits I engage in, as well as increasing my physical activity. I
can also work on my mental health by giving myself the time to feel and again disengage in
acquired negative habits.
The areas in which I am strong in are intellectual, social, spiritual and comprehensive
health. I do have a strong sense in each of these aspects that promote my overall health. My
lower scores do reflect on my overall health in certain aspects. However, I do not feel they are
drastically lowering or damaging my overall health, as my strong suits guide me to improving
upon my not so strong suits.

Self-Assessment

This information helped me to realize I am generally healthy in many aspects and the
areas where I am affected can easily be solved by limiting to completely quitting, taking part of.
If I were to stop those negative habits, I could very much well thrive in all of these aspects. Thus,
benefiting my life and abilities in the end. I can use this information to guide myself on a
healthier path, as well as adopt health assessments in my workplace and community to help
others as well.
