University Supervisors Evaluation Form
The criteria evaluated on this form are based upon and reflective of the
Knowledge, Skills and Dispositions mentioned on pages 4-5 of the
Internship Guide. They are coded by their appropriate K, S, and D numbers
below.
Please check the appropriate box indicating which evaluation form you are
filing. Make copies of this evaluation form for each supervisors as well as
the interns evaluation.

Mrs. Campion was and continues to be an asset to our building. She is in charge of
running many committees and events in the building. The biggest school event is
the Ready Freddy/Back to School which was held in August. This was a major
undertaking and required Elizabeth to be organized, efficient and planned. Mrs.
Campion also attended a three day workshop for leadership in which she was an
active participant and contributor with other current Buffalo Administrators.

A) 5 LEADERSHIP ABILITY (K1, K2, K3, S3, D1, D4, D6 - e.g. ability to inspire
other, set and reach goals, create positive action, solve problems, etc.)
COMMENTS:
Mrs. Campion has proven that she has excellent leadership ability. She has started
a Math Committee which will introduce school wide initiatives such as problemsolving, fluency, and using data to drive instruction. In addition to running the
math committee, she is also part of the leadership team which conducts
instructional rounds, provides feedback and supports the staff.
B) 5 PROFESSIONAL MOTIVATION (D3, D4 - e. g. clear goals, high level of
professional interest and motivation, etc.)
COMMENTS:
Elizabeth went over and above her required hours. She attended a three day
leadership workshop with other Buffalo administrators so that she could gain
experience and valuable information to help her in her skills. Mrs. Campion is
always willing to be a mentor, and also involved in many decisions within the
building.
C) 4 PERSONAL CHARACTERISTICS (D2, D5, D6 - e.g. sense of humor, poise,
self-control, ability to handle crises and stress, honest, etc.)
COMMENTS:
Mrs. Campion has been a pleasure to work with. She is always positive, pleasant
and has a great sense of humor. Elizabeth has had to deal with many situations
involving staffing decisions, interview panels, and handling stressful situations.
She has also had to deal with many dealings of confidentiality and was always
professional.

D) 4INTERPERSONAL RELATIONS (D2, D3, D4, D4, D6 - e.g. ability to relate to


others, work with groups, acceptance of criticism, respect for others, etc.)
COMMENTS:
Mrs. Campion continues to work with many staff members on committees, and
other initiatives within the school. She has been charged with turn keying
professional development. In doing so, Elizabeth works with many teachers. She
remains open to questions, criticisms and feedback as well. She has also been
appointed as the liaison between our volunteer group, Ingram Micro. She has
organized volunteers, set up events, and communicated with this outside agency
many times. She does it respectfully, and for the good of the school. Mrs.
Campion has also worked on organized many outside consultants that come into
the building.

E) 5 PROFESSIONAL COMPETENCE (K1, K2, K3, S1, S2, S3, D1, D2, D3, D4,
D6 - e.g. successful completion of tasks and assignments, demonstrates
understanding of nature of administration, includes dependability,
demonstrates initiative, assumes responsibility, able to work
independently, punctual, etc.)
COMMENTS:
Mrs. Campion has been tasked with many things over the summer. She ran and
organized our parent event, and also our Turnaround Teacher Institute which was a
full two day event with over 50 participants. She was able to plan both events with
little direction and I could count on her to complete the task. Elizabeth worked
many hours with our administrative team.
F) 4WRITTEN COMMUNICATION (S1, S2 - e.g. fluency, accuracy, command of
the language, timeliness, grammar, etc.)
COMMENTS:
We are a school under Receivership and with that comes many reports that need
to be written and completed. Mrs. Campion has been charged with writing these
reports and will continue to take the lead on this. She writes very well and is
someone that we can feel confident regarding completing these reports.

G) 4ORAL COMMUNICATION (S1, S2 - e.g. fluency, poise ability to articulate,


enunciation, grammar, etc.)

COMMENTS:
Mrs. Campion continues to run meetings, professional development, training and
conferences. She also is not intimidated nor afraid to contribute during a
leadership conference with other Buffalo Administrators. This proves that she has
the confidence of being a leader.

H) ADDITIONAL COMMENTS (Comments/observations not covered in prior


question areas)
Elizabeth is an incredible asset to our school and administrative team. She is
someone that we will call on for advice and input. She was an excellent intern and
is ready for an administrative position. Not only does she possess the intelligence,
but she also has very good interpersonal and people skills that will make her a
successful leader. She takes ownership of her core values and continues to
transfer these into her leadership skills.
