Anecdotal Data
This chart would be used to make notes throughout class. Both the regular education
teacher and the special education teacher would be using this chart during class to make note of
any student observations. This may include behavior or academic observations. At the end of the
day, the notes would be copied onto an excel document that both teacher have access to. If there
are any behavior concerns, we would be able to communicate our findings with parents or the
academic director. Behavioral concerns or patterns would be forwarded to the school counselor
or the student supports team. Parents would also be notified by phone or email. Notes will also
be available to parents during report card conferences.

IEP Progress Monitoring Data Collection


Parents will receive a report on their childs IEP goals and progress made towards those
goals four times a year. This particular student used in the example below only had reading
goals, but if students have math or behavior goals, they would also be listed below in the chart. A
brief summary of the goal and any teacher observations will be noted. It will also be noted if the
student is making progress toward the goal, if it is too difficult at this time, or if the student has
mastered the goal. If changes need to be made to the goal, the Special Education teacher will talk
to the parents about making any edits to the IEP. In addition, graphs will be made for each IEP
goal for each probe that was given to the student. Graphs will be attached to the Progress Report
and will be able to show visually how the student is doing with each of their IEP goals.

aware of all aspects of the student and has an accurate snapshot of the students academics,
social interactions, behaviors, strengths, and needs.
