    Dear Sir,
    We would like to highlight our Safety Violation findings for this week for immediate action by our Project Management
    for securing our Safety and Health compliance at our work site. Our safety findings are as per attached below:-

    Issue

    Unsafe Act (Working at height with lifeline and full safety harness)

    Location

    STB 1

    Recommended Action Plan

    Lifeline must be installed before workers can start working at height. Site supervisor
    incharge of the job must make sure lifeline is installed and all workers are wearing full
    safety harness before working at height starts every day.

    Issue

    Oil drums is not kept and used in proper procedure that may result in oil spills.

    Recommended Action Plan

    Oil drums must be kept in proper chemical storage that has drip tray and roofing. Oil
    drums used at site must at least have drip tray underneath at all time.

    Issue

    All workers doing hacking activity is not wearing face shield and ear plug.

    Recommended Action Plan

    No hacking work shall be allowed without workers provided with the right PPE.
    Workers must wear face shield and ear plug at all time during hacking activity.
