AFFIDAVIT

I, Police Officer 3 Juan Luna, of legal age, single, Filipino,

with residence at Brgy Sapang Bato, Tarlac City, after having been
duly sworn to in accordance with law do hereby depose and say
that:

1.

I am an investigating officer of the Philippine National

Police stationed at PCP 5 for 12 years;

2.

That on November 1, 2016 at around ____ in the

morning, a certain Rolly Aguinaldo called our office and
reported that his amo who is the owner of the house
named Jaimie Tan is already dead;

3.

I, together with my co-police officer went immediately to

Brgy. San Vicente where the alleged body of the victim
resides;

4.

Upon arriving at the house, I saw that the body of the

victim was lying down the bed as if she is just sleeping
and when I checked on her pulse, she is already dead;

5.

I, together with my companions immediately rushed her

to the hospital to further check on her condition;

6.

Upon reaching the nearest hospital which is CLDH, she

was already declared dead on arrival by the attending
physician, Dr. Ian Dumayas;

7.

That I am executing this affidavit in support to the

Opposition to the Petition filed by MS. MONSHER TAN.
