The Form of application for Associate Membership by an individual,
who desires to hold shares jointly with another person, who is
already admitted to membership of the Society.

Mr. Subraya Devappa Hegde is already a member of the TriveniAmbika Co-operative Housing Society Ltd., having address at Flat
No. 13, 3rd Floor, Ambika, 353/B-11, Triveni-Ambika CHS Limited,
Vallabhbaug Lane, Ghatkopar (East), Mumbai-400077, holding 5
fully paid up shares of Rs. Fifty each being Share Certificate No. 13
(Distinctive Nos. 61-65).
The said Mr. Subraya Devappa Hegde desires that I, Mr.
Satyanarayana Subraya Hegde, his son, join him as an Associate
Member for the purpose of holding the shares jointly with him, i.e.,
Subraya Devappa Hegde. Accordingly, I, Satyanarayana Subraya
Hegde make this application for admitting me as an Associate
Member to Mr. Subraya Devappa Hegde and remit herewith the
entrance fee of Rs. 100/-. On my admission as an Associate
member by the Society, we shall be jointly and severally liable to
discharge the present and future liabilities of the Society.
The right of attending General Body Meetings and voting at them
will ordinarily be exercised by Mr. Subraya Devappa Hegde, whose
name stands first in the said share certificate No.13. My name shall
stand second in the share certificate and I (the Associate member)
shall have no rights or privileges of a member, and if the member
whose name stands first in the said share certificate is absent at
such meetings, then as mentioned in Section 27 of Maharashtra
Cooperative Societies Act, 1960, I shall have a right to attend such
meeting and to vote in such meetings pursuant to the section 27 of
the Maharashtra Cooperative Societies Act, 1960.
I request you to admit me as an Associate member of the Society
and include my name as an Associate Member in the said share
certificate just after the name of the first member. The consent of
the first member for my admission as an Associate member is
given below.

and conditions set out in the above application. The name of Mr.
Satyanarayana Subraya Hegde be added in the said share
certificate No.13 (enclosed herewith) after my name.
