Use good critical thinking skills.

Using good critical thinking skills can help you to best assess the
situation whether an incident or a CARE-related interaction.

Include all available information under


involved parties.

This helps us to identify students. XIDs in particular help us to


determine that we have the correct John Smith for students
with similar names who attend Clemson.

Always choose Conduct Matter.

Choosing another category can affect who the case is sent to for
further review. For us to properly address a report, it must be
filed under Conduct Matter.

Write down all of the steps that you have


taken in the process.

We review IRs to determine if we have followed all of the steps


in our process. Writing everything down leads to fewer
questions that we may ask later.

Include information about each person
that is documented in the IR.

Consider campus resources that may be


helpful to the student.

Is there a threat of harm (to self)?

(Do not fill out a CARE report for threat of


harm this is always an incident report.)

When someone in Advocacy & Success reviews a CARE report,


they need to determine what to do next. It really helps for them
to know what we have already done.
Someone reading the report or hearing the case needs as much
information as possible about what happened. We need your
written account of EACH person to go with their verbal account
given at the conference with staff.
Someone else will step in after you write the report, but you
have a lot of knowledge that may help them now. Feel free to
suggest on-campus resource(s) based on the concern.
Stop! If someone has threatened to harm themselves, or we
have reason to believe that they may, you must get immediate
assistance. Call the GCD on-call for assistance. (Call CUPD first if
there is an imminent threat of harm, then call the GCD on-call.)

Should someone else be called to assist you?
Look in StarRez for Name, Gender, XID, DOB, Email,
and Hall. (DL number for non-students)
Remember to select role usually alleged, victim,
or witness. An RA who is not the author of the
report, or CUPD, are witness.
If you are instructed to file the incident report under
something else, please do so. But only if you are
specifically instructed to do so by your supervisor or
on-call staff.
Did you contact a supervisor or the GCD on-call?
Did you contact CUPD? Do you know if a police
report was filed?
Did you speak to CUPD about them notifying
someone that a resident went to the hospital?
Did you discuss or consult with your supervisor?
Did you refer the student to any specific resources?
Does anyone else have more information?
Did you ask anyone a question (such as, were you
consuming alcohol) and their response?
Does the student need to go to Redfern?
Does the student know how to fill out their own
notification of absence online?
Is the student aware of CAPS drop-in hours?

Using language indicating someone may harm
themselves, to a person or on social media
Suicide attempt (call CUPD: 656-2222)
