PREFACE
Dear colleagues: health economists, economists, health professionals, and all others who are
interested in knowing the Indonesian health system and contributing to improve access to quality of
care to all Indonesians.
The field of health economics is progressing rapidly in the last 50 years due to increasing
World commitment to provide universal access to essential health services. The uniqueness of health
care is often ignored in developing countries resulting in poor performance of health care and deep
inequity across income groups in particular. The three main characteristic of health care: uncertainty
of needs, wide information asymmetric, and externality make health care the most complex system.
The unique characteristics of health care make the market mechanism generally fails to produce
efficient health care system. However, there are rooms within the health care sub systems where
market mechanisms still contribute significantly.
Limited resources available for health and increasing demand for health care are the most
chal-lenging for economists and policy makers to ensure essential health care for everyone.
Increasing re-sources dedicated for health care created increasing innovations of new technologies in
health care. The advances of communication and information technologies created new human
behavior that promote and demote healthy lives. Unfortunately the ability of people, especially in
developing world, in distin-guishing good and bad information creates new public health challenges.
Poor health behaviors pro-duce higher costs of health care in the future, especially of noncommunicable diseases. On the other hand, there is increasing trend to cover essential health care.
Health economists and policy makers are challenged to maintain dynamic balance between limited
resources and the appetite to provide access of essential health care to everyone in the World.
In Indonesia, we just entered a new era of the implementation of the National Health
Insurance called Jaminan Kesehatan Nasional (JKN) in Indonesian language. The JKN promotes
massive reforms in hospital managements, drugs and medical device managements, human resource
managements, and many other aspects of the Indonesian health system. The execution of prospective
payment (capitation and case mix based groups) nation wide has ignited massive reform in health
care managements. A unit of health technology assessment has been established under the Ministry of
Health to objectively assess new health care technologies to be covered under the JKN. The war on
tobacco in which the country is divided into two factions: one promotes tobacco consumption while
the other fights for tobacco con-trol, will affect the JKN. Thorough economic analyses are required to
ensure the equitable health care in the historically low spending health care.
The ensure that all of us could communicate effectively in conducting economic analyses and
developing new policies that improve our health system, this Indonesian Journal of Health Economic
(IJHE) is introduced. Other health economists and health professionals out side Indonesia may also
use this Journal to learn from or to provide lessons to Indonesia.
We hope, everyone could benefit from and contribute to this IJHE.

Abstract
Indonesia has the highest prevalence of smokers with 67% of adult males were smokers. Smoking
prevalence among all adults increased sharply from 27% in 1995 to 36.3% in 2013. High consumption of
cigarettes has been correlated with low price and excise of cigarettes. Experiences from other countries showed
that one of the most effective way to reduce cigarette consumption is by increasing cigarette price and excise.
Burden of tobacco related diseases has increased. The health burden will increase claims of JKN or Universal
Health Coverage which currently has claim ratio of 115% and the quality of care remain low.
The difficulties in collecting contribution from non salaried workers are blamed to contribute the deficit.
Many countries have earmarked cigarette excise to supplement financing of (UHC) both in tax-funded
system or in social health insurance system. The question is do people support? This study explored the
possibility the peoples support to increase cigarette prices and excise to meet financial shortage of the JKN.
Objectives
This polling conducted to explore cigarette consumption and supports of price increase to finance
JKN or UHC.
Methods
This study used telephone polling conducted form December 2015 to January 2016. The sample
(n=1,000) was randomly selected using systematic random by the interval of 20,000 of mobile phones
numbers. Analysis is focused on how various groups support incrasing cigarette prices and excise. The
final analysis is logistic regression to assess any difference in supporting the excise increase.
Conclusion
The prevalence of cigarette smoking is high because of prices of cigarette is relatively cheap and the
excise levels have not reduced consumption. This study found that large majority (80%) of non smokers and
76% smokers supported increasing cigarette prices and excise to supplement financing for the JKN.
The potential money to supplement JKN is double of the current revenue of JKN.

Background
Smoking cigarette is the highest risk factor for non-communicable diseases (NCDs) in Indonesia resulting in 217,400 deaths annually in Indonesia. The Ministry of Health (MoH) reporet-ed
that the prevalence of smoking conutinued to rise in Indonesia from 27% in 1995 to 36.5% in 2013. It
is estimated that as many as 70 million males are currently active smokers. The prevalence of
smoking among females increased faster from 1.7% in 1995 to an estimate of 6.7% in 2015.
More alarmingly, the prevalence of smoking among youths (15-19 years) increased from 7.1% in
1995 to 20.5% in 2013 (MoHa, 2014). The prevalence of youth smoking in Indonesia is the highest in the World reaching 18.3% in 2014 (MoHb, 2014). The World Health Organization (WHO)
forcasted that the prevalence of smoking in Indonesia will increase to 45% in 2025 (WHO, 2015)
unless significant tobacco control actions are implemented. One of the most signifant factors
con-tributing to high smoking rates is the price of cigarettes is relatively cheap.
The increase prevalence of smoking has been confirmed with the increase of cigarette
production. The Fiscal Agency of the Ministry of Finance reported production of cigarettes has
increased from 222.7 billion sticks in 2005 to 348 billion sticks in 2015. Accordingly, the excise
revenues increased from IDR 32.6 Trillion in 2005 to IDR 139.6 Trillion (US$ 1 = IDR 13,500 in
2016) (FA MoF. 2016). On average, every Indonesians, including newly born baby, consumed
1,365 sticks of cigarettes in 2015. The total money spent for cigarettes in 2015 was about IDR
330 Trillions or USD 24.5 Billions. While the total government expenditure on health was only
IDR 71.1 Trillions or USD 5.3 Billions in 2015 (MoF, 2015). It means that the excise and prices
of cigarettes had not effectively controlled cigarette consumption in Indonesia. The fight to control consumption of cigarette becomes piercer when the Minister of Industry launch a roadmap of
tobacco industry to increase cigarette production from 348 billions sticks in 2015 to 524 billions
sticks by 2020 (MoI, 2015).
While to risks of tobacco related diseases are increasing and the consumption of cigarettes
increased significantly, the country launched the first implementation of the National Health Insurance (Jaminan Kesehatan Nasional or JKN) in January 2014. The insurance mechanism, instead of
tax funded system, to establish Universal Health Coverage (UHC) in Indonesia is based on the
philosophy that everyone should be responsible for financing health care using solidarity principle.
The Government has not been viewed as the sole responsible for financing health care in Indone-sia.
The JKN is financed from 5% payroll tax and the Government subsidies for the low income.
The informal sector who do not have monthly salary pay a nominal amount that they can choose one
of three premiums depending on the class of hospitalization they choose (Thabrany, 2016). The JKN
aims at covering 100% of the population by 2019 (DJSN, 2012). By April 2016, the JKN already
covered 166 million people (65% of the population) under a single database (Idris, 2016).
It is the largest single payer health care in the World, in term of population coverage.
In the last two years, the JKN experienced increasing claims from Non Communicable
Diseases, especially from tobacco related diseases. The JKN paid 22% of the total claim for four
chronic diseases (cardiovascular, renal failure, cancer, and stroke) related to cigarette consumption
(Moeloek, 2016). In the first two years, the JKN suffered from deficit due to shortage of revenues
from contributions. Hidayat (2016) reported that the revised claim ratios of JKN of 111.5% in 2014
and 115.1% in 2015, signaling significant deficits of JKN. The shortfall of the JKN funds were
mainly due to unmet adequacy requirement of contributions, as the ideal contribution calculated by
the Ministry of Health for low income in 2016 is IDR 36,000 per capita while the Government

allocated only IDR 23,000 per capita per month (Moeloek, 2016). With the shortfall is predicted
to continue, the sustainability of JKN is being questioned. One of the viable solution, as it has
been practiced in many other countries, is to mobilize fund from cigarette excises and
earmarking the revenue to finance universal health coverage (UHC) or JKN.
However, controversies about raising cigarette excises as a dual purpose, to control
tobacco uses and to raise fund for the JKN continue in Indonesia. Some politicians, Government
officials, and academics strongly recommend the governmant to fill the financial gap of the JKN
with rising cigarette taxes while some other Goverment officials and the industries against such
notion. Since, the smokers who actually pay excises this study was conducted to explore how far
the people and the smokers support raising cigarette prices and excise to finance JKN.
Objectives
The main objective of this study is to obtain information on how far the public, both
smok-ers and non smokers, support raising cigarette excise to fill the financial shortage of the
JKN. Other objetives include collection information of the profiles of current smokers by various
social and economic variables and the levels of prices of cigarettes that smokers consider to stop
smoking to expand fiscal space.
Method
This study uses survey method of telephone polling to adult Indonesians. The data collec-tion
was conducted in December 2015 through January 2016. The initial sample was drawn from a
random mobile telephone number and then the subsquent number was called with the interval of
20,000 (systematic random sampling). If the respondent was not eligible and refuse to
participate, then the interviewer dialed the subsquent numbers until 1,000 respondents were
interviewed. The interviewers were trained to conduct telephone interviews using a set of
questionnaires special-ly developed to meet the stated objectives. Respondents were asked to
choose an answer of 2-5 choices for each variable, depending on variable of interests. A pre-test
of the questionnares was conducted to ensure that the interview would not last more than 10
minuers and the respondents understood the questiones clearly. The inclusion criteria were age
above 12 years (to ensure under-standing of the questions) and agreed to participate. Data
collected then entered into a statistical database and analyzed to examine the effect of JKN
membership, smoking status, age, gender, eduation, and monthly income on the preference or
support the raising cigarette excises to supple-ment financing of the JKN. A logistic regression
was finally conducted to examine various correla-tion on the support of raising cigarette excises.
employed (26.2%), 21% self-employed, and 6.8% government employees. Although in total 59% of
respondents were members of the National Health Insurance (JKN); only 13.2% were recipients of
the Government subsidies (low income or Penerima Bantuan Iuran (PBI). So, the PBI members are
underrepresented in this poll. The majority of the JKN members responded of this poll was of the
self-employed group (PBPU members) which is blamed as the main contributors of the current
deficit. About two-third of self employed register to JKN when they were suffering from diseases
and higher proportion of them are active smokers (CHEPS, 2016). In addition, this PBPU group who
has problems in paying contribution routinely. Therefore, this finding has more signifant finding to
mobile fund through sin tax (cigarette excise) to finance the JKN. Instead of paying ad-ditional
contribution for their higher risks to coventional channels, directly pay to BPJS Kesehatan or via
ATM, the smokers pay additional contribution through a sin tax.
Overall, 41.3% of respondent were smokers with 20.3% of respondents smoked 1-2 packs
per day. Relatively lower frequency of JKN members smoked every day, but the difference was not
significant. The prevalence of smokers among PBI members of JKN was a slightly lower (36.4%)
compared to the other two groups with more than 41% are smokers. The prevalence of smokers were
relative higher among respondents age 31-50 years with 24.8% of respondents smoked 1-2 packs
daily. The highest prevalence of smoking 1-2 packs daily (29.4%) was among elder people age 61
years and above, doubling their health risks. Middle education (6-12 years) seemed to cor-relate
higher prevalence (21.9%) of smoking 1-2 packs per day compared to low and high educated
respondents. The low income respondents (with monthly income of less than IDR 1 million per
month) seemed less likely to smoke 1-2 packs daily, but they smoked less than one pack daily.
Among JKN members, only 38.6% of them perceived that current cigarette prices is expensive signaling that the current JKN members are relatively having high income. But, the proportion of PBI members who perceived prices of cigarettes were expensive was the high (61.2%)
confirming they were low income individuals. The majority of employees perceived cigarette pric-es
were not expensive, rather there were moderate expensive. Ages were correlate negatively with
perception of cigarette prices. The higher the age, the lower the proportion of respondents who
perceived cigarette prices were expensive. Similar correlations were also found with education and
income. The higher the education and the higher the income the lower the proportion of smokers who
perceived cigarette prices were expensive. Thus, the increasing smoking rates could be at-tributed to
higher income and therefore this study confirm many other studies signaling that the price cigarette
in Indonesia decreasing as the income increase (WHO, 2016).
The most smoked cigarettes were of Sampoerna brand with almost half (45.5%) smokers
mentioned the brand. The second most smoked cigarettes were of Gudang Garam brand (20.2%)
followed by Djarum (11.8%). Marlboro, the most common compared prices were smoked by
8.4%, while other international brand (Dunhill) were smoked by 3.8% of smokers. Only 1.9% of
smokers reported smoked various brands interchangeably. The distribution of the most smoked
brands is in accordance with the market report such as Sampoerna (owned by Philip Morris
International) received highest sales of IDR 80.1 Trillion in 2014 (Sampoerna, 2015), offsetting
cigarette lowere sales in Asia (PMI, 2015). Compared to the MoH budget, the sales of only
Sampoerna in 2014 was almost double of the MoH budget.
We explored the characteristics of smokers to be able to understand their behavior in consuming cigarettes and we found overall 49.1% of smokers, smoked 1-2 packs a day with the mode of
spending between IDR 450,000 IDR 600,000 per month. This amount of money spent for

cigarette could cover contribution for 5-7 persons of JKN of non salaried workers. It is highly
potential to mobilize this money to finance JKN by increasing prices and excise and earmarking
for JKN (sin tax). When coupled with their perception of cigarette is harmful and difficulties
to quit, the increasing prices could help both the smokers to quit and the JKN to improve
financial gaps and the quality of services. As high as 96.8% smokers know that cigarette is
harmful and 62% confessed that they had difficulties to quit smoking, indicating that they are
addicted. What we need is to convince legal and policy makers to provide strong regulation,
revising current excise law to increase excise rates on cigarettes and to include earmarking excise
for JKN. However, fur-ther studies are needed to know the level of supports by legal and policy
makers. If the legal and policy makers support the notion, then the deficit of JKN can be solved.
When we dig deeper, there were slightly higher proportion of JKN members who smoked
1-2 packs daily and spent up to IDR 600,000 per month. More interestingly, more than half of
PBPU (peserta bukan penerima upah) members (50.4%) who are not employed and having difficulties to pay contribution spent up to IDR 600,000 per month for cigarettes. On the other hand,
a study of PBPU reported that members of JKN who are smokers had higher probability to lapse
membership by more than 6 months (CHEPSUI, 2016). The proportion of elderly (60 years and
older) who smoked 1-2 packs a day was the highest (71.4%) and 100% of them were aware that
smoking is harmful. The elderlies have much higher risks of NCD and higher consumption of
health care. Instead of blaming the PBPU members and the elderlies of consuming more health
care, because they are relatively sickers, it is legitimate to indirectly push them to pay more.

Considering current (2016) cigarettes prices range from IDR 12,000 20,000; we have
plenty of rooms to increase prices and excise. The willingness to stop smoking due to prices is in
line with economic theory of price and income elasticities. Unanimously, findings from
economic studies everywhere in the world show that demand for cigarette is price inelastic
(Hidayat, 2011; USAID, 2013; World Bank, 2016)). Most economists, policy makers, and health
professionals agree that the inelastic demand for cigarettes, with respect to the prices and
incomes, was due to additictive natures of nicotine. Hidayat and Thabrany (2010) proved the
addictive behavior using econometric modelling that the demand for cigarettes.
We explore more on this correlation of would stop smoking due to higher prices of cigarettes by analyzing various independent variables. Boths smokers and non smokers agreed that
certain level of high prices will eventually pushed to smokers to reduce or stop smoking. Therefore, the prices of cigarettes in Singapore, Malaysia, and Timor Leste are set high by the government to increase the government revenues, to reduce smoking prevalences especially among low
income and teenagers, and the cigarette industries continue to enjoy profits. We found all groups,
JKN members and non members, across the types of JKN members, across age groups, across
education groups, and across income groups, unanimously (more than 70%) agreed that the price
of a pack of cigarette above IDR 50,000 could stop smoking (Table 1). That level of price is
about three time more than the current average price.
The economic theory suggests that if consumers are willing to pay a certain prices, setting
price below that level will cause welfare loss. The industries and the government can set double the
current prices, say to be IDR 30,000 on average100% increase, using the known price elas-ticities
of -.39 (WHO, 2011), the cigarettes sales will reduce 39%. If current volume of cigarette
consumptions (it was 341 billion sticks in 2015) than the total consumption will reduce to 208
billions sticks. But, the total volume of business of cigarette will increase. For a simple aritmatic
modeling, if the current price of cigarette per stick is IDR 1,000; the total volume of bussines is
IDR 341 Trillion (341 billion sticks x IDR 1,000). If the price is increased double to IDR 2,000
per stick, consumption will reduce to 208 billion sticks. But, the total business will increase to
208 B x IDR 2,000 = IDR 416 Trillion. If the excise rates are increased to an average of 50%
(from current 43%), then the government revenue from excise increase from IDR 139.8 in 2015
to IDR 208 Tril-lion. The industries will enjoy higher gross revenues (total volume minus excise)
of IDR 208 Tril-lion. This is a win-win solution in tobacco control that the FCTC urges member
countries to adopt raising cigarette excise. The only loosers are the smokers. But, that is the idea
of tobacco control and the principle of responsibilities. The smokers increase their health risks
then they should pay more to handle their risks, as JKN members. As mentioned earlier, almost
all smoker (more than 96%) knows that smoking is harmful for health. If they know that their
behavior is harmful, and they are willing to pay more by rasing cigarette prices, then the
government policy should simply use it to finance the deficits of JKN. Please note that the
current deficit of JKN is actually for low quality of JKN (Thabrany, 2016).
To continue exploring public support to pay higher prices of cigarettes to suplement the
JKN fund, as has been practiced in many countries, we asked the respondents (smokers and non
smokers) of their agreement on rasing cigarette prices and earmarking.

As shown in Table 2, the vast majority (above 80%) of respondents in various groups
agreed to raise cigarette prices and exice to meet financial gap of JKN. Higher proportion
(83.5%) of non smokers significantly agreed to raise cigarette prices compared to smokers
(76%), p=0.005. Even that, 76% of smokers agreed to raise cigarette prices. This finding is a
diamond for public policy in improving our health care while reducing poor health behavior. The
people support the government to improve their health and to assist them to reduce their poor
health behavior. This a win-win policy of health promotion and health prevention while
increasing fund for health care of the people. In addition, there is the third win, which is the
government will get high reputation from the public to improve the access and quality of health
care from the money mobilized through higher excise on tobacco.
In the bivariate analysis, we found differences in the level of supports in raising cigarette
exices to finance JKN between smokers and non smokers and between the middle ages (30-50
years) with other ages. However, across income and education levels, we found no significant
difference. This is a very good news for the government, for the legal makers (DPR), and for the

tobacco control activists. This finding is line with studies everywhere in the world and suggest
that the Government should not worry and should not concern with the voice of industries in
signing or accessing FCTC.
In the final model, the logistic regression, we applied the probability of supporting higher
cigarette prices and exice across various groups. As shown in Table 3, we found no diferrent at
all among all groups. This finding, again, gives strong evidence that the President pledge of
raising 200% excise revenues by 2019 (Nawa Cita, 2014) is fully supported by the public.

the world and provide evidences for earmaking (developing sin tax) to finance UHC in Indonesia.
This sin tax has quadruple win for the Government to increse revenue, for the current ruling
parties to obtain good reputations, to improve the JKN quality, and for the industries (including
labors, tabacco farmers, and clove farmers) to sustain their income.
The authors recommend that further study to know how the legal and policy makers support developing and implementing sin tax to finance JKN. Concurently, the Ministry of Finance
should start revising current Excise Law that is 10 years old to accommodate earmarking for the
JKN. In addition to finance the JKN, portions of the excise revenues should also be earmarked or
dedicated to improve income of tobacco and clove farmers, low skill workers in cigarette industries to switch to higher salaried jobs, to finance sports and art activities of youths, and to
promote healthy behaviors.

Abstrak
Telah menjadi isu umum bahwa Global Fund (GF) sebagai salah satu donor internasional terbesar
untuk Program AIDS, Tuberkulosis, dan Malaria (ATM) akan mulai menghentikan pendanaannya. Data
menunjukkan bahwa pada tahun 2009 dukungan GF ATM mencapai 88,8% dari total pengelolaan
program sementara dana APBN hanya menutupi sekitar 11,2%. Namun demikian, anggaran APBN untuk
program ATM meningkat secara signifikan pada tahun 2012 yang mencakup hampir 30% dari total
anggaran. Meski-pun kecenderungan peningkatan anggaran ATM terjadi pada level pusat, peran
pemerintah daerah akan memegang kunci dalam keberlangsungan program paska terminasi GF ATM.
Tujuan
penelitian ini dilakukan dengan tujuan untuk mendapatkan gambaran komitmen pemerintah
daerah sebagai lembaga pelaksana untuk merespon kebutuhan pembiayaan khusus untuk program TB.
Metode
Evaluasi ekonomi dalam artikel ini dilakukan dengan membandingkan besaran anggaran yang
telah direalisasikan saat ini dengan jumlah yang anggaran dibutuhkan berdasarkan Standar Minimum
Pelayanan (SPM) Program TB. Sampel pada penelitian ini ditetapkan pada dua kabupaten/kota di Jawa
Barat yaitu Kota Cirebon dan Kabupaten Garut. Komponen biaya yang dihitung dalam evaluasi ini adalah
antara lain: obat-obatan, peralatan medis, biaya pencegahan dan pemuan kasus, serta biaya administrasi .
Total anggaran yang dibutuhkan di Garut menurut SPM sekitar 2,5 Milyar Rupiah, sedangkan total
anggaran yang telah dialokasikan sekitar 2 Milyar Rupiah. Dari toral anggaran yang telah dialokasikan di
Garut, sebesar kurang lebih 90% bersumber dari Pemerintah sedangkan selebihnya merupakan dukungan
dari GF. Kecenderungan serupa terjadi di Kota Cirebon, dimana ditemukan selisih sebesar kurang lebih
700 Juta Rupiah dari sekitar 1,6 Miliar Rupiah anggaran yang dibutuhkan. Porsi pembiayaan program di
Kota Cirebon yang bersumber dari pemerintah sudah mencapai 80% dari total anggaran. Temuan penting
dalam penelitian ini menunjukkan bahwa pembiayaan kegiatan pencegahan dan penemuan kasus di Garut
masih didominasi oleh dukungan GF yaitu sekitar 65%. Lain halnya dengan Kota Cirebon, dimana
anggaran un-tuk kegiatan tersebut sudah mencapai 80% yang didukung oleh pemerintah.
Kesimpulan
Secara umum, baik Kabupaten Garut maupun Kota Cirebon menghadapi dua tantangan dalam hal
keberlangsungan pembiayaan program TB. Pertama, tingginya kesenjangan antara kebutuhan dan
anggaran yang telah dialokasikan untuk program menjadi perhatian penting untuk mengatasi penurunan
kasus TB di wilayah terkait. Kedua adalah keberlangsungan program setelah terminasi pembiayaan dari
Global Fund, terutama untuk program pencegahan dan penemuan kasus. Oleh karena itu, tentu diperlukan
keteribatan dari LSM dan berbagai pihak terkait untuk melakukan advokasi kepada pemerintah daerah
dan DPRD un-tuk meningkatkan pembiayaan dalam penanggulangan TB.

Abstract
It has become a common issue that the Global Fund (GF) as one of the largest international donors to
AIDS, Tuberculosis, and Malaria Program will immediately stop the funding. Data shows that in 2009 GF
ATM support reached 88,8% while APBN funding just cover 11,2% of the total budget needed. Howev-er,
APBN budget for ATM programs was significantly increased in 2012 which covered almost 30% of the total
budget. Eventhough the increasing trend of ATM budget seemed at the central government level, how-ever the
local governments will hold the key to the sustainability of the post- termination GF ATM Funding
Objectives
This study aimed to get a picture of the local governments commitment as an implementing institution to respond the financing needs specifically for TB programs.
Methods
This economic evaluation compared the amount of the existing budget of local governments and
the amount needed based on the Minimum Standards of Services (MSS) of TB Programs. We sampled
two district in west java that were Cirebon and Garut. The cost component calculated in these evaluation
were: medicines, medical supplies, case findings, and administrative cost.
Conclusion
In general, both Garut and Cirebon faced two common challenges in terms of financing the TB
program. First, the high shortage between needs and budget alocated of the program becomes an important
concern for addressing TB cases reduction in related district. The second is programs sustainibility after
termination of Global Fund, particularly for prevention and case detection programs. Therefore, it might be
need a support from NGO or other related institution to advocate the local government and DPRD to
allocate more budget for reducing TB cases.

Pendahuluan
Indonesia saat ini berada di urutan ketiga negara dengan beban tuberkulosis (TB) tertinggi di dunia dengan estimasi prevalensi TB semua kasus adalah sebesar 647/100.000 penduduk
(WHO, 2015). Berdasarkan perhitungan disability-adjusted life-year (DALY) WHO, kasus TB
menyumbang 6,3 persen dari total beban penyakit di Indonesia, dibandingkan dengan rata-rata di
kawasan Asia Tenggara sebesar 3,2 persen (USAID, 2008). Hingga saat ini, sebagian besar
pembiayaan AIDS, TB, dan malaria di Indonesia secara umum masih didukung oleh donor internasional terutama Global Fund (GF). Namun demikian, peran donor Internasional dalam
beberapa tahun ke depan akan berakhir, termasuk yang bersumber dari GF (Global Fund, 2015).
Sehingga isu exit strategy pembiayaan program TB terhadap terminasi dukungan donor asing
menjadi san-gat penting untuk keberlangsungan program.

Merujuk pada tabel di atas ditunjukkan bahwa pola anggaran yang bersumber pemerintah
telah mengalami peningkatan dari sekitar 11% pada tahun 2009 menjadi sekitar 30% pada tahun
2012. Komitmen tersebut direncanakan akan terus meningkat hingga 50% di tahun 2016
(Kemen-kes, 2012). Namun demikian, sebagian besar peningkatan tersebut masih fokus pada
pengadaan obat dan bahan medis terkait pemeriksaan dan pengobatan TB.
Penyelenggaraan sistem kesehatan ke depan melalui program JKN akan lebih sistematik
dengan masuknya upaya pengobatan TB dalam paket manfaat jaminan. Namun demikian yang
menjadi pertanyaan adalah bagaimana pola pembiayaan untuk Upaya Kesehatan Masyarakat
(UKM) yang merupakan salah satu peran penting dari pemerintah, dalam hal ini adalah Kementerian kesehatan dan dinas kesehatan pada tingkat daerah dalam menurunkan angka kesakitan TB.
Sehubungan dengan hal tersebut, untuk membangun political will dari para pemangku kebijakan,
dibutuhkan sebuah evidence based data terkait besaran kebutuhan biaya program sebagai bahan
dalam advokasi pembiayaan UKM. Kemudian, dilakukan analisis situasi dengan
membandingkan kebutuhan pembiayaan program tersebut dengan anggaran yang hingga saat ini
telah dialokasikan termasuk sumber pembiayaannya. Sehingga dengan mengetahui kesenjangan
tersebut, maka hasil analisis dalam artikel ini akan menunjukkan seberapa besar gap yang harus
diupayakan pemerin-tah untuk mengendalikan dampak dari penyakit TB di Indonesia.
Tujuan
Tujuan dari studi ini adalah untuk membandingkan antara kebutuhan program
berdasarkan SPM dengan anggaran yang telah dialokasikan. Sementara secara khusus ditujukan
untuk menga-nalisis kesenjangan pada masing-masing komponen biaya dan sumber pembiayaan.

Metode
Penelitian ini dilakukan melalui pendekatan kuantitatif (dengan metode survei) dan
pendekatan kualitatif. Pendekatan kuantitatif dalam penelitian ini dilakukan untuk mendapatkan
besaran biaya program TB yang sudah di alokasikan melalui metode activity based costing dan
perhitungan kebutuhan anggaran melalui pendekatan Standar Pelayanan Minimal (SPM). Semen-tara
pendekatan kualitatif dilakukan melalui proses wawancara mendalam kepada para pengambil
kebijakan untuk menguatkan argumentasi pada temuan-temuan kuantitatif yang didapatkan (Hancock et.al, 2007). Perspektif yang digunakan dalam studi ini adalah perspektif program terutama yang
terkait dengan Upaya Kesehatan Masyarakat (UKM) pada penatalaksanaan program TB baik pada
fasilitas tingkat primer maupun rujukan pada tahun 2013.
Dasar penentuan Provinsi sampel merujuk pada capaian Case Detection Rate (CDR) dan
angka Success Rate (SR) (Dirjen P2PL, 2011). Dalam artikel ini akan dipilih satu provinsi den-gan
kriteria memiliki CDR dan SR yang dapat menunjukkan besaran kasus dan tingkat efektivitas
penyelenggaraan program. Pemilihan di tingkat provinsi terlebih dahulu dibagi menjadi 4 wilayah
hingga terbentuk kluster seperti pada tabel di bawah. Oleh karena kriteria yang ditetapkan adalah
daerah dengan tingkat CDR dan SR tinggi maka kluster yang terpilih terdiri dari 5 provinsi yai-tu
Jabar, Sulawesi Utara, Maluku, Jakarta, dan Banten. Dari kelima provinsi tersebut dilakukan
pemilihan secara acak hingga didapatkan Jawa Barat sebagai lokasi sampel.

Pada tingkat kab/kota di provinsi terpilih, pemilihan sampel tidak menggunakan cara
yang sama dengan di tingkat provinsi oleh karena ketersediaan data CDR dan SR di tingkat
kab/kota. Sehingga peneliti menggunakan indeks kapasitas fiskal untuk merepresentasikan
kemampuan daerah. Peneliti menggunakan indeks kapasitas fiskal dengan mengasumsikan
adanya hubungan yang erat antara indikator tersebut dengan komitmen pemerintah daerah dalam
pembiayaan pro-gram TB pada khususnya. Klasifikasi yang dikeluarkan dalam peraturan menteri
keuangan, setiap kab/kota dibagi menjadi 4 kategori dana daerah urusan bersama (DDUB) yang
bersumber APBD yaitu dengan klasifikasi sangat tinggi, tinggi, sedang, dan rendah. Mengingat
di setiap provinsi hanya akan dipilih 2 kab/kota maka peneliti mengelompokkan menjadi 2
kategori DDUB, yaitu tinggi (sangat tinggi dan tinggi) dan rendah (sedang dan rendah). Di setiap
kategori tersebut dipi-lih secara acak, hingga terpilih Kota Cirebon yang mewakili kapasitas
fiskal tinggi dan Kab Garut yang mewakili kapasitas fiskal rendah.
Pada setiap lokasi studi di tingkat kab/kota dipilih 4 puskesmas dengan kriteria 2 puskes-

mas satelit dan 2 non satelit yang dikonsultasikan dengan pihak Dinas Kesehatan setempat.
Selain itu pada masing-masing kab/kota juga akan dilakukan pengumpulan data di satu rumah
sakit, satu laboratorium penunjang (bila ada), serta LSM yang terlibat dalam penatalaksanaan
program TB di wilayah terkait. Selain itu, untuk pendekatan kualitatif juga akan dilakukan
wawancara mendalam di Bappeda atau pemerintah daerah.

Untuk membuat kedua pendekatan tersebut menjadi layak dibandingkan, maka selain
penyetaraan komponen biaya juga diperlukan penyetaraan item biaya (cost driver) yang diperhitungkan. Terdapat beberapa item biaya yang dikeluarkan dari perhitungan antara lain obat, gedung, dan gaji rutin. Pembiayaan untuk obat dikeluarkan dari perhitungan sebab pada pendekatan
SPM tidak menyebutkan rincian kegiatan pengobatan, sehingga biaya untuk obat tidak muncul
meskipun dalam praktiknya terdapat penggunaan obat. Hal ini dikarenakan pengadaan obat dalam
penatalaksanaan program TB seluruhnya telah di supply oleh pemerintah pusat melalui APBN
sehingga tidak ada biaya yang muncul pada fasilitas kesehatan primer selain gaji dan insentif
petugas. Begitu pula untuk item biaya gedung dan gaji rutin yang bersifat given goods atau sum-ber
daya yang pasti akan keluar meskipun tidak terkait dengan program TB, dimana pembiayaan
bersumber donor tidak mencakup biaya tersebut sehingga tidak dapat dibandingkan. Lain halnya
dengan biaya insentif staf, yang muncul baik dari sumber APBN/APBD maupun Global Fund sehingga dapat dibandingkan. Untuk gaji staf tambahan yang direkrut oleh GF ATM guna membantu
pengelolaan program akan diperhitungkan dalam item insentif staf.

di kota Cirebon sebanyak 5317 kasus atau 1778/100000 penduduk. Hal inilah yang juga memicu
tingginya kebutuhan anggaran program berdasarkan SPM, sebab basis data dalam perhitungan
SPM adalah angka kasus TB dan frekuensi kegiatan.
Total perhitungan pembiayaan program TB pada tabel di atas tidak termasuk komponen
biaya obat, sebab komponen tersebut tidak tercantum dalam item Standar Pelayanan Minimum
(SPM). Namun demikian, pada proses perhitungan realisasi anggaran didapatkan bahwa total
pengeluaran biaya obat untuk penatalaksanaan program TB di Garut sekitar 1,5 Milyar. Dari total
pembiayaan obat tersebut, sekitar 94% bersumber dari pemerintah sedangkan sisanya merupakan
support dari Global Fund. Berbeda dengan pola pembiayaan obat di kabupaten garut,
pembiayaan obat di Kota Cirebon sepenuhnya di supply oleh pemerintah.

Tabel di atas menjelaskan rincian masing-masing komponen biaya dan sumber pembiayaannya, antara lain pemerintah daerah, pemerintah pusat dan global fund. Namun demikian,
rincian biaya di atas hanya menjelaskan proporsi pembiayaan dari anggaran yang telah direalisasikan di masing-masing daerah. Secara total, penatalaksanaan program TB di kabupaten garut
masih di dominasi oleh pembiayaan bersumber pemerintah, yaitu sekitar 46% pemerintah daerah,
44% pemerintah pusat, dan 10% Global Fund. Apabila ditinjau lebih rinci memang tingginya alokasi anggaran pemerintah baik pusat maupun daerah masih cenderung pada pengadaan alat dan
bahan medis serta komponen gaji.
Lain halnya dengan Kota Cirebon, secara umum peran global fund terhadap penatalaksanaan program TB tidak terlalu signifikan. Berdasarkan perhitungan alokasi anggaran program
melalui pendekatan ABC, didapatkan bahwa peran global fund dalam pembiayaan TB di Kota
Cirebon hanya sebesar 4% sementara selebihnya merupakan support dari Pemerintah. Porsi pembiayaan pada komponen alat dan bahan medis menunjukkan bahwa peran pemerintah baik pada
level pusat maupun daerah jauh lebih dominan dibandingkan GF. Begitu juga pada komponen
pencegahan dan penemuan kasus, dimana sekitar 80% pembiayaannya telah di support oleh pemerintah daerah.

Salah satu isu penting yang terdapat pada tabel di atas adalah pada komponen pencegahan
dan penemuan kasus. Aktivitas yang dilakukan pada komponen ini antara lain kunjungan rumah pada
pasien suspek TB maupun kegiatan operasional lain guna mencegah penularan aktif oleh pasien TB,
sehingga pembiayaan untuk komponen ini sangat penting. Porsi pembiayaan kegiatan pencegahan
dan penemuan kasus di kota Cirebon sebesar 80% berasal dari pemerintah daerah.
Namun demikian, pembiayaan untuk komponen ini kabupaten Garut masih cukup kecil yaitu
seki-tar 35%, sementara 65% lainnya masih di-support oleh Global Fund. Berdasarkan temuan
studi kualitatif diketahui bahwa program penanggulangan TB khususnya untuk kegiatan
pencegahan dan penemuan kasus masih belum mendapatkan porsi pembiayaan yang memadai
seperti yang disampaikan oleh narasumber berikut.

..biasanya program TB dimasukkan dalam satu RKA yang bersatu dengan kegiatan
lainnya, termasuk kegiatan penanggulangan penyakit epidemic dan pandemic. Tahun ini saya
coba dipisahkan RKA tersendiri, jadi saya masukkan program pencegahan dan penanganan
penyakit Tuberculosis. Saya coba anggarkan Rp.256.000.000 untuk tahun 2015
Hal ini menjadi perhatian penting mengingat dukungan donor asing akan segera berakhir,
sementara pembiayaan pemerintah untuk aktivitas primer masih relatif kecil. Untuk menjamin
keberlangsungan program, pemerintah daerah tentunya perlu mengupayakan kerja sama dengan
pihak swasta (Public-Private Partnership) atau berbagai pihak lain yang memiliki potensi dan
ket-erkaitan dengan penatalaksanaan program TB. Selain itu tentunya diperlukan advokasi yang
tepat kepada pemangku kebijakan yang berwenang dalam penetapan anggaran. Hal ini
dikarenakan be-ban ekonomi yang ditimbulkan oleh penyakit TB cukup tinggi, mengingat
tingginya angka preva-lensi Tuberkulosis baik di Kabupaten Garut maupun Kota Cirebon.

Abstract
It has become a common issue that the Global Fund (GF) as one of the largest international
donors to AIDHealth economic evaluation that encompasses decision analytic model is a beneficial
approach for assisting decision maker to choose the best health intervention for patients. Decision analytic
model has been increasingly applied in health economic evaluation. This mathematical approach is mostly
used for conducting cost-effectiveness of healthcare interventions.
Decision tree and Markov model has been widely applied in the past 20 years. Decision tree is the
simplest form of decision model that drawn by the series of branches and clear pathways. Meanwhile,
Markov model is one of the powerful approaches that employ stochastic process in health economic evaluation. This paper describes the applications of those two models in tobacco cessations, specifically for
pharmacological interventions.
First, decision tree for cost-effectiveness of smoking cessation program with pharmacist and
thera-pies interventions compared to no program or self-aid cessation. Second, the application of Markov
model estimates cost-effectiveness of veranicline, in comparison to bupropion. Markov model is
constructed with morbidity and mortality states that consists of: well/no morbidities, lung cancer, COPD,
stroke, myocar-dial infarction, and dead. This paper provides step by step of populating and constructing
the model-with some modification of data. Several sections discuss the understanding of transition
probabilities, costs data, cohort simulation, and the role of sensitivity analysis. Other models, despite
deterministic approach, prob-abilistic approach are also reviewed.
Both of models had both advantages and limitation that analysts should be aware of. Translating the
real world to mathematical model yields beneficial and insightful information for analysts. In addition, it
could fulfill the need of evidence-based policy by decision maker. From simulation, the model may easy
to be replicated-with appropriate context to generate evidence related health and costs.

Introduction
Health economic evaluation is a general framework to aid decision maker with best available evidence in terms of priority setting and choosing particular health technologies (Shiell et
al. 2002). This is one of pivotal aspects for decision making that commonly related to funding,
reimbursement or regulatory of health intervention, with increasing challenges of finite resources
available (Morris et al. 2007; Hjelmgren et al. 2001).
Decision analytic model can be used in health economic evaluation; it is widely applied
to estimate cost-effectiveness of healthcare interventions (Sox et al. 1988; Buxton et al.1997;
Hun-nink et al. 2001). This essential technique is defined as a systematic approach on
synthesizing information from multiple sources and applying mathematical relationship to
answer decision problems, allowing its variability and uncertainty. Expected costs and possible
consequences of all available options are compared (Briggs et al. 2006; Gray et al. 2010). In
particular situation, economic evaluation is increasingly conducted alongside clinical trial.
However, the results from this economic evaluation do not always able to inform decision related
reimbursement (Sculpher et al. 2006). Decision analytic model therefore can be beneficial to
resolve the limitations in clin-ical trial.
Generally, there are several circumstances when decision analytic modeling in health economic evaluation is very useful. First, when Randomized Clinical Trials (RCTs) could not be
undertaken due to costs, time, or other reasons. Hence, modeling techniques can be applied by
synthesizing all relevant information from various sources, not only from primary data (Karnon and
Brown, 1998). Second, RCTs remain crucial for efficacy evidence. However, it might not com-pare
all relevant alternatives and provide appropriate time horizon. In health economic evaluation, time
horizon should be long enough to capture the magnitude of costs and benefits among alter-natives.
(Drummond et al., 2005; Briggs et al., 2006). Third, final endpoints evidence is difficult to be
achieved, for instance: mortality and long-term morbidity. It is related to short time horizon in
clinical trials, mostly RCTs are concluded up to intermediate endpoints. Furthermore, by devel-oping
a model, the results can be extended (Briggs et al., 2006). Moreover, Sculpher et al. (1997) explained
four major stages in economic evaluation process, and decision analytic model assigned prominent
role in each of those steps. Therefore, decision analytic model is able to sufficiently an-swer the
resource allocation question, while all information about cost and consequences become barriers in
primary clinical research such as RCTs. As powerful tool in economic evaluation, mod-eling is
structuring evidence both clinical and economics outcomes, simplifying representation of reality. It
thus beneficial to generate relevant evidence that highly required by decision makers.
There are some alternatives modeling approach that can be used for estimating cost-effectiveness of health interventions. For example: dynamic models, discrete event simulations (DES),
and patient level simulations (Petrou and Gray, 2011). However, this paper only discusses decision
tree and Markov model application, since these two models are frequently applied in economic
evaluation (Buxton et al., 1997; Karnon and Brown 1998). Author discusses the example of tobac-co
program as well as pharmacological therapies in tobacco cessation, in this example: veranicline,
bupropion. The costs and outcomes value have been determined as instances. Models in this paper
are structured by referring from previous studies, indeed with some modifications. Author will
frequently use terms Cost-effectiveness Analysis (CEA) and Cost Utility Analysis(CUA). Illustrations of these methods can be found in publication by Drummond et al. (2005). This paper is
divided into several sections that cover stages on designing and conducting simple decision tree

Decision rules
Before going to further parts, this section covers the decision rules and general rationale
of conducting CEA/CUA for making sense the final resources after applying decision models.
Deci-sion makers in health sector often face a dilemma for choosing health intervention. New
treatment/ health technology is often more effective than current/standard treatment, however it
is inevitably more expensive. Health economists are interested in considering of how much do
we get the benefit for additional cost or the money spent? Value for money of new intervention
furthermore is summarized by Incremental Cost-Effectiveness Ratio (ICER) (Owens, 1998;
Jefferson, 2000; Cohen and Reynolds, 2008).
The concept of cost, effect, effectiveness and its linkage to ICER concepts are divided
into four main rules (OBrien et al. 1994; Briggs and Tambour 1998). From two therapies that are
com-pared, one new treatment and a comparator or familiarly named as existing treatment
represented by A and B, respectively. Mean of treatments costs denoted by_cA and_cB.
Similarly, effects are denoted by _eA and _eB. It describes below:
1. _cA - _cB< 0 ; _eA - _eB> 0 ; dominance, accept new treatment
2. _cA - _cB> 0 ; _eA - _eB< 0 ; dominance, reject new treatment
3. _cA - _cB> 0 ; _eA - _eB> 0 ; trade-off (consider magnitude of ratio of differenc-es
in cost to differences in effect; or additional costs relative to additional effectiveness)
4. _cA - _cB< 0 ; _eA - _eB< 0 ; trade-off (consider magnitude of ratio of
differenc-es in cost to differences in effect, cost-saving to it reduced effectiveness)
Those four points are in relation to four quadrants of cost-effectiveness plane. It is commonly used for presenting cost-effectiveness results that described and discussed in almost economic
evaluation results. The condition number 1 demonstrates that we accept new treatment when it is
more effective and less costly compared to existing treatment. In contrast, condition number 2, when
new treatment is more costly and less effective, so then clearly we reject it. However, when a new
treatment is more effective but also it is more expensive, then judgment should be made. This is the
condition when ICER is playing its role, providing the summary of cost-effectiveness of new
intervention compared to current intervention/treatment(s) with equation:

Defining questions
The first step for structuring the decision model is specifying the decision problem. Population, interventions, comparators, as well as clinical and economic outcomes should be clearly
identified (Briggs et al. 2006; Drummond et al. 2005; Gray et al. 2010). In this case, for instance, we
will discuss modeling method for cost-effectiveness/cost-utility analysis of smoking cessation
program with pharmacological therapies such as: varenicline and bupropion. In addition, perspec-tive
of evaluation should be considered, why the intervention is being evaluated, costs incurred, and for
whom the result required and may support the decision. The perspectives reflect the pur-pose of
evaluation (Byford and Raftery, 1998; Jonsson, 2009; Hjelmegren et al. 2001).
Appropriate decision model should be structured and developed by clear understanding
about the problems. Several guidelines provide details of good practices in constructing the model (Weinstein et al. 2003; Roberts et al. 2012). Parameters choice, sources of data and evidence
synthesis important as model input also available in standing alone publication. (Caro et al.
2010; Briggs et al. 2012). In this paper, all the data has been given and we directly head to
explanation of model structure. However, author provides the schematic of design and
conducting decision analytic model in figure 1. It is a complete visual illustration of decision
analysis process. This illustration is summarized and modified according several publications
(Briggset al. 2006; Marti-kainen, 2008; Gray et al. 2010; Roberts et al. 2012; Briggs et al. 2012).

The average cost-effectiveness ratio (ACER) is derived from expected costs per
effective-ness, in this case per quit smoking with no persistent cough. We can conclude that
ACER of no program is higher than providing program $2,937.5. Furthermore, to estimate
ICER, as formulat-ed above, the ICER will be (222.5-117.5) / (0.21-0.04) resulting $617.65 per
quit attempt (with no persistent cough). To conclude whether program is potentially effective,
analysts can use their acceptable maximum of cost-effectiveness ratio ().
Applying decision tree allows the less complicated scenarios, simplicity, as well as
trans-parency (Karnon and Brown, 1998; Petiti, 1999). We can build a set of alternatives with
clear pathways. However, there is a limitation to deal with time dependent in economic
evaluation such as applying discounting. The decision tree above looks simple because it is
applying short-term outcome and short time horizon (less than one year), with unnecessary
discounting appli-cation. The smoking cases often have outcome that tend to be recursive, or
patients will survive with a particular condition that may take longer than one year. In terms of
recurrent events such as chronic diseases, the analysis will be more complicated. Decision tree
become bushy and having long-complex pathways. (Karnon and Brown, 1998; Briggs et al.
2006; Petrou and Gray, 2011)
Markov model
The Markov model was first developed by Russian mathematician, Andrey Markov
(1856-1922). It is a random process that encounters transition from one state to another state.
This model is beneficial for ongoing events over time and risks exposure (Stahl, 2008). For
instance, we will discuss lung cancer as clinical conditions that are experienced by patients, risks
may change over time and Markov model can be useful for this case.
As a model alternative in health economic evaluation, the Markov model can deal with more
complex decision problems and is able to be applied for longer pathways/sequences, partic-ularly for
recurrent outcomes (Stahl, 2008; Sato and Zouain, 2010). Markov state refers patients are staying in
one health state and can move from their current state to another state over discrete time period. The
movements among states are attributed by transition probability. Number of states in Markov model
are determined depend on decision problem. Moreover, time horizon is represented by distinctive
cycles, it could be monthly or annual cycle. (Sonneberg and Beck, 1993; Briggs and Sclupher,
1998; Briggs, 2006; Nikfar, 2012). The first state in Markov process

is commonly of well condition or without morbidity, then progression states and finally death,
or familiarly known as absorption state.
We will discuss the application of Markov model for estimating the cost-effectiveness of
Veranicline compared to Bupropion. This is based on condition that Varenicline has higher effectiveness to support smoking cessation compared to another pharmacological intervention (Mahmoudi et al, 2012). Structure of this model is adapted and modified according to publication that
apply BENESCO (Benefits of Smoking Cessation on Outcomes model) and Quit Benefit Model
(QBM) (Hurley and Matthews 2007; Bolin et al. 2009).
The first stage in constructing Markov model after specifying decision problem is de-fining
the states. States must represent clinical and economic effects of a disease. In this model, states
consist of: no-morbidity/well, COPD (Chronic Obstructive Pulmonary Disease), lung can-cer, stroke
and MI (Myocardial Infarction). Four states in progressive states assume that model will be
simulated for 20 years, and post-morbidities state transition probability assumed similar for first and
second year onwards. The Markov model is presented in Figure 3, and to translate and to understand
the transition probabilities, author provide the transition matrix on table 2.

the intervention. If the model having extended structure such as post-stroke or post MI, it means
that we could add new input parameters for second year onwards.

The total sum of transition probabilities from initial until end of model must be 1.0.
There-fore, when we calculate the probability of patients who remain in the same states (loop
symbol) with 1-probabilties. Value 0 simply representing that there is no transition probability
from a state to another state (see figure 2.)
Sometimes, when it is intended to input parameters such as transition probabilities, for
in-stance from systematic reviews, the confusion between rate and probability exists.
Transition probabilities gathered from literature reviews do not exactly reflect the models cycle
that we con-struct (Sato, 2010). Rates are potential occurrence of an event in defined population
and defined time. Meanwhile, probability is assigned values ranging from 0 and 1, representing
the likelihood of an event happening over a specific period of time. To deal with this, rate is
possible to be con-verted to a probability and vice versa (Briggs et al. 2006) where p is the
probability, r is the rate and t is the time period of an interest:

In the cohort hypothetical simulation, the author desribes how Markov model is running,
without assigning all input parameters. Details are provided in appendix 1. We only incorporated
transition probability for every health states, only for familiarizing audience of basic Markov application. Unlike a decision tree, Markov model is applicable for applying discounting, which is an
important factor in health economic evaluation. Additionally, for chronic diseases, the life table
and survival data are often attached into the model. Quality Adjusted Life Years (QALYs) also can be
estimated as final results, since in calculating QALYs, valuing quality of life encompasses the length
of time spent in health states (Briggs, 1998). The cost analysis also has similar method,
costs are represented in treatment events. With computer application, this complex simulation can
be handled. After simulating decision analytic model, the final results are ICER, derived from
differences for both costs and effects/utility, in this case after 20 years cycle. Thus, decision rules
should be followed.

Although Markov model application is helpful in decision analytic modeling, there are
several limitations that we should aware of. Markov has a characteristic of memory less (Sonnenberg, 1993; Briggs, 1998). It means when patient move to another states, the model will have
no historical memory where patients has come from previous cycles. The population equal in this
model also with constant risk. Adding additional states and time dependency into transition probability may possible for the model. Besides, Markov model has greater complexity compared to
decision tree, however computer software nowadays could help for running this model.
Handling uncertainty
Variability, uncertainty and heterogeneity influences the results of decision analytic
model, these should be handled (Barton et al. 2004). The results of evaluation are depending on
uncertain-ties that related with various factors. This section will focus on short-review for
handling uncer-tainty, before going further, that will be useful to define several types of
uncertainty in modeling (Briggs et al. 2012). Decision makers needs evidence beyond point
estimate of the outcome. The uncertainty-surrounding outcome should be explored and reported.
Uncertainty may categorized as methodological uncertainty, parameter uncertainty, structural
uncertainty and generalizability (Briggs and Sclupher, 1995). Thus, decision maker can receive
more information what factors that influence the model and its findings, and increasing the
confidence level where they read and intend to accept the evaluation results.
Uncertainty can be handled by performing appropriate sensitivity analysis. There are several types of sensitivity analysis (Andronis et al. 2009, Briggs et al. 2012). First, a one-way sensitivity analysis. It is the simplest form of sensitivity analysis, examines impact of the changes in
model results by varying plausible values range. Tornado diagram is used to visualized this,
presented with baseline value and range variation in the right and left wings. Second is multiway sensitivity analysis, including two-way sensitivity analysis. This examines relationship of
two or more parameters, not single parameters like one-way sensitivity analyses. Another type is
extreme scenario, simply when the parameters or input value of favorable intervention are
established to provide best and worst case scenario.
Probabilistic Sensitivity Analysis (PSA) is one type of sensitivity analysis that runs randomly by large number of Monte Carlo simulation (for instance: 10,000 iterative). It is examine
each input parameters with their probability distributions. It resulted mean cost as well as effectiveness. This approach is preferred to generate cost-effectiveness acceptability curve (Claxton et
al. 2005; Ades et al. 2006).
Finally, model result and its sensitivity analysis should be presented clearly. It is advisable to
see guidelines related to full health economics study reporting (Husereau et al. 2013).
Conclusions
Models are chosen, construct and developed based on decision problems and must be appropriate with decision makers purpose. All evidence as input parameters are assigned and performs by mathematical operations that translating into model. Furthermore, to decide the set of
alternatives that should be chosen, the final results of decision analytical models have to follow
decision rules for cost-effectiveness of health intervention. The explanation in this paper not providing best practice yet simple example of modeling application, further details and international
guidelines are available elsewhere (Sclupher et al. 2000; Hjelmegren et al. 2001; Weinstein et al.

Abstrak
Sejak dimulainya JKN, pengadaan obat di fasilitas pelayanan kesehatan (faskes) publik dilakukan dengan e-Purchasing melalui e-Catalogue. Didasarkan pada RKO dan HPS, penyusunan e-Catalogue
dilakukan melalui proses lelang dan negosiasi harga. Rantai proses tersebut akan berdampak pada jenis
(molekul) dan jumlah obat yang tayang dalam e-Catalogue maupun jumlah dan volume permintaan oleh
faskes publik (e-Order).
Tujuan
Penelitian bertujuan untuk mengevaluasi e-Order menurut kategorisasi obat, yaitu generik (OGB)
dan dengan merek dagang (OMD), pada data e-Catalogue 2014-2015.
Metode
Pada penelitian ini dilakukan pula wawancara dengan pegawai LKPP yang berwenang dalam
peny-usunan e-Catalogue. Evaluasi dilakukan dengan menganalisis profil penawaran obat JKN (eCatalogue atau RKO) dan kesenjangannya dengan permintaan oleh fasilitas kesehatan publik, baik pada
kelompok OGB maupun OMD.
Kesimpulan
Guna mengatasi masalah mendasar ini, perlu dilakukan penyempurnaan dalam penetapan RKO
dan HPS serta dibuat kesepakatan terkait alur dan jadwal penyusunan e-Catalogue.

Abstract
Since National Health Insurance (JKN) has been implemented, drug procurement in public health
care facilities use e-Purchasing via e-Catalogue. Based on the RKO and HPS, drafting e-Catalogue have
gone through a tender process and price negotiations. The process chain will effect the type (molecules)
and the amount of drug through the e-Catalogue as well as the number and volume of demand by public
health care facilities (e-Order).
Objectives
This study evaluated the e-Order according to the categories of medicine, namely generic (OGB)
and branded (OMD), using e-Catalogue data 2014-2015. The study also conducted interviews with
employ-ees LKPP authorities in the preparation of e-Catalogue. Evaluation has been done by analyzing
the profile of drug deals JKN (e-Catalogue or RKO) and gaps with demand by the public health care
facilities, either in groups or OMD OGB.
Conclusion
To address this fundamental problem, it is necessary to improve the determination of RKO and
HPS and related agreements about e-Catalogue preparation flow and schedule should be made.

Pendahuluan
Sejak dimulainya Jaminan Kesehatan Nasional (JKN), awal 2014, pengadaan obat JKN di fasilitas pelayanan kesehatan (faskes) publik dilakukan dengan e-Purchasing melalui e-Catalogue. Berdasarkan
pada Rencana Kebutuhan Obat (RKO) Tingkat Nasional dan Harga Perkiraan Sendiri (HPS) yang ditetapkan Kementerian Kesehatan (Kemenkes), penyusunan e-Catalogue dilakukan melalui proses lelang dan
negosiasi yang dilaksanakan oleh Lembaga Kebijakan Pengadaan Barang dan Jasa (LKPP). Pengadaan
obat JKN melalui e-Catalogue diatur dalam Peraturan Presiden (Perpres) Nomor 4 Tahun 2015 tentang
Perubahan Keempat atas Perpres No.54 Tahun 2010 tentang Pengadaan Barang/Jasa dan Surat Edaran
Kemenkes No.167 Tahun 2014. Pengaturan tersebut dimaksudkan untuk menjamin ketersediaan dan pemerataan obat yang aman, bermutu, dan berhasiat di faskes secara transparan, efektif, efisien, dan dapat
dipertanggungjawabkan (Kemenkes, 2014).
Proses pengadaan obat dimulai dari pemilihan, pemesanan, dan pengiriman (distribusi) yang, pada
ujungnya, adalah pemberian obat oleh rumah sakit [atau puskesmas] kepada pasien. Agar berjalan dengan baik,
proses ini perlu didukung manajemen informasi, sumberdaya manusia, perencanaan, dan organisa-si yang baik
(Health, 2012b). Pada dasarnya, e-Catalogue merupakan sistem manajemen informasi yang menghubungkan
antara pemerintah (LKPP, Kemenkes, Badan POM), produsen (pabrik obat, distributor),

dan pengguna (rumah sakit, puskesmas) dalam proses pengadaan obat JKN. Berbasis teknologi informasi,
sistem e-Catalogue bertujuan memberikan kenyamanan, efisiensi, harga yang wajar, dan informasi obat
baru; menempatkan pembeli pada efisiensi tertinggi (Ketikidis et al., 2010). Dengan sistem online yang
transparan tersebut diharapkan rumah sakit dan puskesmas dapat melakukan pengadaan obat JKN secara
mudah.
Kenyataannya, masih sering dijumpai pasien JKN harus menebus obat sendiri karena kekosongan
obat, obat tidak tercantum dalam Formularium Nasional (Fornas), obat tidak termasuk dalam paket pengobatan, dan berbagai alasan lain yang disampaikan oleh pihak rumah sakit atau puskesmas. Akibatnya,
pasien harus mengeluarkan biaya tambahan untuk mendapatkan obat yang sesuai dengan dosis dan lama
terapi yang dianjurkan.
Dengan demikian, perlu dilakukan kajian tentang alur sistem e-Catalogue, termasuk pola pemesanan obat JKN yang meliputi jenis dan jumlah obat berdasarkan data LKPP.
Tujuan
Tujuan penelitian ini adalah mengevaluasi, secara kualitatif dan deskriptif kuantitatif, pengadaan
obat JKN melalui e-Catalogue pada 2014-2015, termasuk kategorisasi menjadi obat generik (OGB, yang
telah off-patent) dan obat dengan merek (OMD, yang umumnya masih in-patent) pada penyusunan eCata-logue yang dilakukan oleh LKPP.
Metodologi
Penelitian ini adalah studi evaluatif menggunakan data pemesanan obat melalui e-Catalogue dan
RKO yang tercatat dalam pangkalan data LKPP untuk tahun 2014 dan 2015. Analisis kuantitatif
dilakukan guna melihat berapa banyak RKO mendapat pemesanan dari fasilitas kesehatan publik (eOrder), jumlah industri farmasi dan satuan kerja kesehatan yang berpartisipasi dalam e-Catalogue, serta
mengkaji kesen-jangan yang ada terkait kelompok OGB dan OMD.
Selain itu, dilakukan pula analisis kualitatif untuk mendapatkan gambaran tentang proses pengadaan obat JKN melalui e-Catalogue dan masalah yang ada dalam rantai proses yang panjang tersebut.
Data kualitatif dalam penelitian ini diperoleh melalui wawancara mendalam pada November 2015, dengan
pegawai LKPP yang berwenang dalam penyusunan e-Catalogue obat.

Proses penetapan e-Catalogue melibatkan dua pihak sebagai pihak pertama, yaitu Kemenkes dan
LKPP. Kementerian Kesehatan menyusun RKO Nasional [dari RKO yang diajukan oleh Pemerintah Daer-ah
Tingkat I Provinsi seluruh Indonesia] selama satu tahun dan HPS. Sampai saat ini, e-Catalogue hanya dapat
diakses oleh faskes publik yang, menurut Permenkes No.68 Tahun 2010, wajib menggunakan obat generik
(Kemenkes, 2010), Sebab itu, dalam e-Catalogue, OGB didahulukan dibandingkan obat lainnya.
Sebagai pelaksana lelang harga dalam penyusunan e-Catalogue, LKPP membentuk Kelompok Kerja
(Pokja) yang bertugas melakukan pemilihan penyedia obat di Kementerian/Lembaga/Satuan Kerja Perangkat
Daerah/Institusi (K/L/D/I) (LKPP, 2015). Untuk itu, Pokja membuat dokumen lelang berdasar-kan RKO dan
HPS yang telah ditetapkan oleh Kementerian Kesehatan. Obat yang masuk dalam lelang diseleksi kembali
dengan melihat apakah obat tersebut memiliki versi generik (OGB)-nya. Bila OGB terse-but memiliki banyak
penyedia, dilakukan lelang. Tetapi, bila OGB tersebut hanya memiliki dua atau tiga pemasok, sehingga sulit
terbentuk harga yang wajar dalam pelelanganapalagi pemasok tunggaldilaku-kan proses negosiasi harga.
Demikian pula untuk obat yang tidak ada OGB-nya; bila memiliki banyak penyedia dilakukan proses lelang
dan, bila jumlah pemasoknya terbatas, dilakukan proses negosiasi.
Setelah dicapai kesepakatan harga untuk penyediaan obat dengan kuantitas tertentu, di provinsi
tertentu, baik melalui mekanisme lelang maupun negosiasi, dibuat kontrak payung. Kontrak payung inilah
yang menjadi dasar penayangan e-Catalogue di situs web LKPP. Penawaran dalam e-Catalogue berlaku
pada awal tahun atau, untuk item obat yang mengalami keterlambatan tayang, segera setelah tayang. alami keterlambatan tayang, segera setelah tayang. Proses pembentukan e-Catalogue yang secara umum digambarkan dalam Lampiran-1 tersebut, termasuk jadwal kegiatannya, dapat diakses oleh para pemangku
kepentingan kunci.
Karena harga obat dalam e-Catalogue umumnya lebih murah dibanding harga reguler, e-Catalogue
tersebut biasanya langsung dimanfaatkan untuk e-Purchasing. Proses e-Purchasing dimulai dengan pemesanan secara online obat yang dipilih oleh K/L/D/I. Pesanan tersebut dilanjutkan kepada perusahaan farma-si
pemenang lelang yang harus segera menyatakan apakah mampu memenuhinya. Pemenang lelang hanya satu,
yaitu perusahaan farmasi yang memberikan penawaran harga terendah. Jika tidak sanggup memenuhi pesanan
yang diajukan, perusahaan farmasi tersebut harus memberikan penjelasan yang dapat diterima dan, jika
diperlukan, dapat dilakukan revisi pesanan yang kemudian diberitahukan pada K/L/D/I. Jika sang-gup
memenuhi pesanan, pemenang lelang memberikan laporan kesediaan pada K/L/D/I. Berdasarkan pen-awaran
dan kesediaan tersebut, dibuat kontrak antara K/L/D/I dan perusahaan farmasi pemenang lelang un-tuk
pengiriman obat JKN yang dipesan. Secara umum proses e-Purchasing dapat dilihat pada Lampiran-2.

Pada 2014 dan 2015, dengan volume e-Order yang masing-masing mencapai 1.894,71 juta dan
3.072,42 juta satuan obat terkecil sementara nilainya hanya Rp861,84 miliar dan Rp1.549,07 miliar, rerata
harga OGB adalah Rp454,86 dan Rp504,19 per satuan obat terkecil. Di sisi lain, dengan volume e-Or-der
yang masing-masing hanya 33,79 juta dan 103,35 juta satuan obat terkecil tetapi nilai yang menca-pai
Rp337,16 miliar dan Rp1.652,37 miliar, rerata harga OMD pada periode yang sama, 2014 dan 2015,
Rp9.978,04 dan Rp15.957,70 per satuan obat terkecil atau sekitar 20 sampai 30 kali lipat rerata harga
satuan OGB.
Dari sisi penyedia obat, pada 2014 maupun 2015, perusahaan farmasi yang terpilih menjadi
penye-dia obat JKN sebagian besar adalah perusahaan nasional (PMDN). Tidak memiliki R&D yang
kuat, PMDN lebih banyak menawarkan OGB atau, kalaupun OMD, yang relatif rendah harganya.

Kesesuaian yang lebih baik ini tampaknya lebih disebabkan karena, pada tahun 2015
RKO beberapa item obat mengalami rasionalisasi setelah pada 2014 tidak mendapat e-Order
yang me-madai. Namun demikian, pada 2015 pun, proporsi e-Order dari 15 item obat dengan
RKO terban-yak masih jauh lebih rendah dari 50%, kecuali untuk tablet besi [II] sulfat + folat.
Pemesanan obat antianemia yang mencapai 111% RKO tersebut mungkin karena pada tahun
sebelumnya, 2014, e-Order oleh fasilitas kesehatan nyaris nihil.
Pada penyediaan obat penyakit kronis tidak menular, seperti anti hipertensi dan anti dia-betes,
terjadi pola kesenjangan yang serupa. Secara umum, kesenjangan RKO dan e-Order pada 2014 juga
lebih lebar dibanding pada 2015 (Tabel 3). Pada 2015, tidak ada lagi, misalnya, item obat yang
mendapat e-Order <25% RKO. Hal ini juga terjadi karena dilakukan rasionalisasi terhadap beberapa
item obat yang pada 2014 tidak mendapat e-Order memadai. Di sisi lain, karena RKO obat-obat
dengan e-Order tinggi pada 2014seperti tablet kaptopril dan tablet metforminjuga dinaikkan,
jumlah item obat yang mendapat e-Order >50% RKO tidak mengalami peningkatan berarti. Kalau
pada 2014, empat item obat mendapat e-Order >50% RKO, pada 2015 jumlah item obat yang
mendapat e-Order >50% RKO hanya meningkat jadi lima.
Data LKPP menunjukkan pula bahwa semua item obat dalam Daftar 15 Obat Penyakit
Kronis Tidak Menular dengan RKO Terbanyak adalah OGB. Laiknya OGB, rerata harga 15 obat
penyakit kronis tidak menular dengan RKO terbanyak tersebut jauh lebih rendah dibanding
rerata harga OMD, yaitu masing-masing Rp114,67 (pada 2014) dan Rp143,97 per satuan obat
terkecil (pada 2015). Di sisi lain, semua item obat dalam Daftar 15 Obat Antikanker dengan
RKO Terban-yak adalah OMD (Tabel 4). Belum memiliki versi off-patent, rerata harga obat
antikanker tersebut sangat tinggi, yaitu masing-masing Rp43.924,86 (pada 2014) dan
Rp31.080,91 per satuan obat ter-kecil (pada 2015).

Sistem e-Catalogue yang digunakan dalam pengadaan obat JKN pada dasarnya adalah
ba-gian dari rantai-pasok yang menghubungkan produsen dengan pengguna produk/jasa. Rantaipa-sok terbentuk dari partisipasi dua atau lebih perusahaan yang berkomitmen untuk
memberikan nilai-tambah pada produk/jasa terkait (Lu, 2011, Ripin et al., 2014). Akhir dari
rangkaian ran-tai-pasok produk/jasa tersebutyaitu obat, dalam hal JKNadalah pasien,
sebagai konsumen akhir. Kekosongan sebagian obat JKN, suatu hal yang mengindikasikan
kurang memadainya ran-tai-pasok, menurut Lu (2011), tidak akan terjadi kalau terbentuk aliansi
strategis, yaitu pengaturan formal maupun informal berbagai komponen dalam rantai-pasok, dan
sumber daya terkait terinte-grasi dalam aliansi strategis tersebut.
Penggunaan e-Procurement diyakini dapat menekan biaya dan keyakinan bahwa penggunaan e-Procurement, termasuk pengadaan obat JKN melalui e-Catalogue, dapat menekan biaya
akan mendorong percepatan penggunaan e-Procurement, termasuk di rumah sakit dan fasilitas
kesehatan lainnya (Pasipoulos et al., 2013). Teknologi, lingkungan, dan budaya organisasi
merupa-kan beberapa faktor yang dapat mempengaruhi penggunaan e-Procurement (Ronald &
Omwenga, 2015). Faktor berpengaruh lainnya adalah ketersediaan dana, kemampuan organisasi
mengatasi perubahan, dan keandalan pelatihan petugas (Matunga et al., 2013).
Penetapan Fornas di awal tahun akan memberikan kelonggaran waktu bagi LKPP dan Kementerian Kesehatan untuk menyiapkan e-Catalogue tahun berikutnya, sehingga dapat dipastikan
bahwa daftar obat yang masuk dalam e-Catalogue merupakan obat yang tercantum dalam Fornas.
Selain itu, LKPP dan Kemenkes dapat mengecek secara lebih saksama Nomor Izin Edar (NIE) dari
BPOM, sehingga obat yang masuk dalam e-Catalogue memiliki NIE yang masih berlaku.
Dalam dua tahun pertama pengadaan obat JKN ini belum terbentuk keselarasan yang baik; masih
ada obat yang masuk dalam e-Catalogue namun tidak ada dalam Fornas atau tidak memiliki NIE
yang valid.
Kekosongan sebagian obat JKN juga terkait dengan masuknya suatu obat dalam e-Catalogue. Kenyataan bahwa beberapa item obat Fornas yang tidak masuk dalam e-Catalogue, menunjukkan bahwa penetapan RKO Nasional dan HPS yang menjadi dasar bagi industri farmasi untuk
melakukan penawaran harga perlu ditinjau-ulang. Menurut Bogaert, Bochenek, Prokop & Pilc
(2015), di kawasan dengan perekonomian maju, seperti Eropa, kekosongan obat umumnya disebabkan oleh ketidaksesuaian harga dan hambatan dalam proses lelang. Penggunaan historical data
dapat diharapkan membuat penetapan RKO lebih mendekati permintaan riil, seiring waktu. Sementara itu, penggunaan referensi yang lebih komprehensif, termasuk harga internasional, dapat
diharapkan akan membuat HPS yang ditetapkan lebih realistis.
Hal lain yang mengurangi peluang industri farmasi untuk mengajukan penawaran harga
adalah pemisahan antara obat generik (OGB) dan non-generik (ODM), yang membuat tidak
semua perusahaan dapat mengikuti proses lelang harga. Pemilihan pemenang lelang yang hanya
didasar-kan pada penawaran harga terendah berpengaruh terhadap kualitas layanan, yaitu
pengiriman (de-livery) obat. Untuk mendapatkan pemasok terbaik perlu dipertimbangkan
beberapa kriteria tam-bahan yang lebih terkait dengan kualitas layanan (maupun kualitas obat
yang dikirim). Menurut beberapa peneiliti, harga bukanlah faktor terpenting dalam pemilihan
pemasok obat (Abdolshah, 2013; Enyinda, Dunu & Bell-Hanyes, 2010).
Kemampuan industri farmasi dalam memenuhi permintaan obat dari faskes tergantung pula
pada kesiapan pemenang lelang. Untuk memproduksi obat, mulai dari pemesanan bahan baku yang
harus diimpor, diperlukan waktu sekitar tiga bulan. Karena itu, penetapan pemenang lelang

sekitar tiga bulan sebelum e-Catalogue berlaku, akan menjamin pemenuhan komitmen oleh perusahaan farmasi pemenang lelang tersebut. Kemudahan para pemangku kepentingan kunci meman-tau
proses dapat diharapkan akan membantu penetapan e-Cataloguetermasuk waktu penetapan
Fornas dan pemenang lelang yang cukup dinisehingga meminimalkan kekosongan obat.
Terkait faskes swasta yang tidak meperoleh akses terhadap e-Catalogue sehingga harus
melakukan peme-sanan obat secara manual, kemudahan mendapatkan pasok obat JKN dengan
harga e-Catalogue akan meminimalkan kekosongan obat secara lebih luas.

Abstrak
Gagal Ginjal Kronis (GGK) merupakan kondisi yang semakin meningkat kejadiannya di Indonesia, menghabiskan banyak dana publik Jaminan Kesehatan Nasional (JKN). Dalam program JKN,
hemodi-alisis (HD) untuk penanganan GGK dijamin tetapi perleu keseimbangan antara biaya dan
outcome. Sejak 2014, BPJS menanggung hampir seluruh biaya HD di Indonesia dengan besaran tarif
Casemix Base Group (CBG) yang berbeda menurut kelas Rumah Sakit (RS).
Tujuan
Pertanyaan penelitian ini adalah apakah tarif yang dibayarkan lebih tinggi pada kelas RS lebih
tinggi menghasilkan hasil yang lebih baik? Selain itu, perlu diketahui pula apakah terdapat perbedaan
biaya yang dikeluarkan RS pada kelas yang berbeda.
Metode
Studi evaluasi ekonomi ini dilakukan di dua RS dengan kelas berbeda: kelas B (RS B) dan kelas
C (RS C) dengan perbedaan kepemilikan. Kepemilikan RS B adalah pemerintah daerah sedangkan RS C
dimiliki oleh yayasan swasta. Outcome HD diukur dengan suatu survey ke pasien HD. Analisis outcome
dilakukan dengan penilaian kualitas hidup (instrumen EQ-5D) dengan Indeks EQ, EQ VAS, intermediate
outcome berupa rerata Intra Dialytic Weight Loss (IDWL), dan rerata Hb. Perbedaan rerata nilai hasil
diuji dengan Students t-test. Responden dipilih dari pasien GGK yang menjalani HD di kedua RS selama
Feb-ruari-April 2016. Analisis biaya menurut perspektif pasien, meliputi biaya langsung medis, biaya
langsung non medis, dan biaya tidak langsung. Biaya sebenarnya yang dikeluarkan oleh RS dikumpulkan
dari doku-men RS. Studi kualitatif tambahan dilakukan dengan wawancara mendalam kepada informan
kunci di RS yang bertanggung jawab atas unit HD.

Kesimpulan
Besarnya tarif CBG untuk HD di RS kelas B dan C berbeda, tapi outcomenya tidak berbeda. CBG
yang lebih tinggi tidak memberikan outcome kesehatan yang lebih baik. Kementerian Kesehatan diharapkan merevisi tarif CBG tanpa diskriminasi antar kelas RS, tetapi perbedaan tarif CBG menurut
kepemilikan perlu dilakukan. Riset komprehensif untuk menemukan tarif keekonomian yang layak di
berbagai wilayah perlu dilakukan Kemenekes dan atau BPJS.

Abstract
Chronic renal failure (CRF) has been increasing in Indonesia and consuming a lot of the National
Health Insurance (JKN) fund. The JKN should cover all medically necessary, including hemodialysis
(HD), but should balance between spending and good outcome. Since 2014, the JKN covered all HD
using differ-ent Case mix Base Group (CBG) prices for different class of hospitals.
Objective
The research question is whether the price differences correlate with different outcome of HD. In
addition, differences of costs in providing HD in different class of hospital are also questioned.
Method
This is an economic evaluation as case study in two different hospital and survey of HD patients in the
two hospitals, class B and class C hospital. The class B hospital is owned by a local government while the class
C hospital is owned by a private not for profit organization. The Outcomes of HD are measured us-ing
intermediate outcomes of mean hemoglobin levels, Intra Dialysis Weight Loss (IDWL), EQ index and EQ VAS
index. The measurement of quality of life used EQ-5D instrument. The Students t-test is measured to test
differences in costs and outcomes between the two different classes of hospitals. To measure quality of life, a
survey to all eligible HD patients undertaking HD during February-April 2016 was taken. Costs (direct
medical, direct non medical, and indirect) are measured to hospitals and patients perspectives. To explore
qualitative information, in-depth interviews were taken to managers of HD units in both hospitals.

were no difference with the average EQ index of 0.7208 and EQVAS index of 64,79 in class C hospital, p
value of =0.94 and 0.986 respectively.
Conclusion
The difference in price of CBG paid to class B and class C hospital produced the same outcome
level. The operational costs in providing HD in both hospitals was no difference statistically. It is
concluded that the Government should not pay difference prices for different class of hospitals unless
there is different outcome. The Government (Ministry of Health) should revise the CBG prices and pay
equal amount of HD for different class of hospitals. However, difference prices are justified between
public and private hospital to compensate investment costs of private hospitals.

Pendahuluan
Gagal Ginjal Kronik (GGK) sejak Jaminan Kesehatan Nasional (JKN) dijalankan semakin meningkat. Insidens gagal ginjal kronik meningkat dua kali dalam 15 tahun terakhir (Pernefri, 2012). Salah satu terapi
simpomatik yang banyak digunakan adalah hemodialisis yang memakan biaya besar. Selain biaya besar,
hemodialisis menyerap biaya tidak langsung yang besar karena durasi dan frekuensi terapi (Kaite-lidou et al,
2005). Di dunia, sejak 1972, hemodialisis dijadikan menjadi benchmark terapi mempertahankan hidup yang
umumnya dibiayai dana publik (Mendelssohn & McFarlanet, 2011). Meskipun prevalensi GGK hanya 0,050,07%, tindakan ini menghabiskan 1 sampai 1,6% belanja kesehatan nasional (Kaitelidou et al, 2005). Pada
tahun 2014, BPJS Kesehatan membayar klaim kasus GGK sebesar Rp 2,2 Triliun dan pada tahun 2015
meningkat menjadi Rp 2,7 Triliun (BPJS Kesehatan, 2016).
Penanganan GGK merupakan tantangan ekonomi dan politik yang utama bagi pelayanan
kesehatan (Teerawattananon, et al, 2007). Sejak 2014, BPJS menanggung biaya hemodialisis (HD)
dengan besaran tarif InaCBG (Indonesian Casemix Based Group, Ina-CBG) yang berbeda menurut kelas
RS. Tarif hemo-dialisis di RS kelas B di Jakarta sebesar Rp 982.600 sedangkan tarif di RS kelas C
sebesar Rp 893.300. Sementara tarif di RSCM mencapai Rp 2.110.500 (Permenkes No. 59 Tahun 2014).
Pertanyaannya, apakah tarif CBG lebih tinggi pada RS kelas B mencerminkan biaya yang lebih
tinggi dan menghasilkan outcome yang lebih baik? Dari perspektif pasien, menurut Sculpher, perlu juga
di-kaji apakah perbedaan tarif CBG berdampak pada perbedaan biaya tidak langsung bagi pasien
(Drummond & Mc Guire, 2001). Dari perspektif RS, apakah biaya aktual yang keluarkan RS berbeda
menurut kelas RS? Untuk menjawab pertanyaan tersebut, telah dilakukan studi kasus di dua RS, yang
satu RS kelas B milik pemda dan RS kelas C milik swasta pada tahun 2016. Secara umum RS kelas B
memiliki kemampuan pe-layanan kesehatan yang lebih tinggi dari RS kelas C sesuai persyaratannya.
Namun pada unit hemodialisis, persyaratan di kedua RS tidaklah berbeda sesuai Peraturan Menteri
Kesehatan Republik Indonesia No. 812/ Menkes/PER/VII/2010.
Tujuan
Studi ini bertujuan menguji apakah ada perbedaan biaya dan keluaran (outcome) hemodialisis
yang dilakukan di dua RS dengan kelas dan pemilik berbeda, yaitu RS kelas B dan RS kelas C.

Metode
Studi ini merupakan studi evaluasi ekonomi dengan kelas mengambil dua RS dengan kelas berbeda: kelas B dan kelas C. Pilihan hanya dua RS adalah dengan pertimbangan bahwa pada studi-studi
terdahulu, biaya HD relatif standar dengan varians kecil (Novelia, 2014). Selain itu, pada umumnya RS di
Indonesia menggunakan skema sewa alat untuk HD dengan kontrak hanya membayar dengan membeli
cairan dan filter/dialiser. Di tiap RS diambil pasien HD untuk mengukur kualitas layanan dengan
mengukur kualitas hidup menggunakan perangkat alat ukur EQ-5D. Secara umum, kerangka konsep studi
ini adalah sebagaimana terlihat dalam Gambar 1. Biaya HD di RS dan biaya tidak langsung diukur
dengan wawancara pengelola RS dan wawancara kepada pasien. Analisis biaya, outcome, dan rasio biayaoutcome dilakukan oleh penulis dengan menggunakan perangat lunak spread sheet dan statistik.
Rumah sakit kelas B yang dipilih adalah RS milik pemerintah daerah yang telah beroperasi lebih dari
14 tahun dan telah melakukan hemodialisis sejak lebih dari 12 tahun. Sedangkan RS kelas C adalah RS milik
swasta nirlaba yang relatif dan menyediakan layanan hemodialisis dalam 2 tahun terakhir. Pada saat
pengambilan data, RS B menyediakan 32 mesin HD dengan jumlah pasien HD rawat jalan sebanyak 127
orang, sedangkan RS C menyediakan 37 mesin HD dengan jumlah pasien HD rawat jalan sebanyak 129 orang.
Untuk mengukur biaya tidak langsung dan outcome, dilakukan survei kepada pasien hemodialisis di kedua RS
tersebut. Populasi penelitian adalah pasien dengan gagal ginjal kronik yang menjalani HD yang terdaftar di RS
kelas B dan kelas C. Dilakukan sampel waktu, seluruh pasien yang memenuhi kriteria inklu-si yang menjalani
hemodialisis selama bulan Februari - April 2016, dan telah menjalani hemodialisis lebih dari 2 (dua) tahun
untuk dapat mengukur outcome kualitas hidup. Selain itu, kriteria inklusi lain adalah ter-dapat rekam medis
riwayat terapi lengkap dan persetujuan pasien untuk menjadi sampel studi. Pasien yang memiliki gangguan
kognitif, mental, pendengaran atau bicara yang tidak memungkinkan diwawancarai dikeluarkan dari sampel.
Untuk mengukur kualitas outcome, dikumpulkan data laboratorium perkemban-gan kadar Hb, IDWL ( dan
pengukuran kualitas hidup menggunakan format EQ-5D.
Untuk mendapatkan data biaya RS, dilakukan wawancara dengan 6 (enam) orang dari kedua RS
yang terdiri dari 2 (dua) orang yang mewakili manajemen RS, 2 (dua) orang yang mewakili penanggung
jawab layanan HD, dan 2 (dua) orang yang mewakili pelaksana harian HD. Untuk menjamin validitas
data biaya, dilakukan triangulasi metoda dan telaah dokumen.
Analisis biaya dilakukan dengan mengambil biaya menurut perspektif pasien dengan perhitungan
estimasi biaya HD selama satu bulan dengan rumus berikut:

Perhitungan biaya HD menurut perspektif pasien terdiri dari biaya langsung medis untuk satu tindakan HD yang diperoleh dari tagihan RS. Biaya tagihan RS sudah termasuk biaya tindakan HD, jasa/
gaji tenaga medis, bahan medis habis pakai, obat dan pemeriksaan lab. Biaya langsung HD selama satu
bulan diperoleh dengan mengalikan biaya satu tindakan HD dengan delapan tindakan HD selama sebulan.
Sedangkan biaya langsung non medis dihitung dari biaya yang dikeluarkan pasien maupun keluarga yang
menunggu pasien untuk transportasi dan biaya makan selama satu tindakan HD. Biaya selama satu bulan
diperoleh dengan mengalikan biaya satu tindakan HD dengan delapan tindakan HD selama sebulan. Sementara biaya tidak langsung dihitung berdasarkan biaya yang terjadi akibat hilangnya pendapatan ketika
pasien ataupun keluarga menjalani tindakan HD atau mengantar pasien sehingga mereka tidak bekerja
dan kehilangan penghasilan. Biaya ini dihitung per hari dengan membagi penghasilan pasien/penunggu
satu bulan (dari wawancara pasien) dengan jumlah hari yang digunakan untuk tindakan HD. Biaya total
HD di-hitung berdasarkan penjumlahan biaya langsung medis, biaya langsung non medis dan biaya tidak
langsung selama sebulan.
Outcome hemodialisis diukur dengan kualitas hidup pasein menggunakan instrument standar
yaitu EQ-5D dan rerata Hb. Anaylisis biaya-outcome dilakukan dengan membandingkan jumlah total
biaya HD di RS kelas B dan RS kelas C selama sebulan dibandingkan dengan perbedaan outcome HD
yang dinilai dengan rerata skor EQ-5D, rerata skor EQ VAS, rerata IDWL dan rerata Hb.

Di RS kelas C layanan HD tersedia sejak JKN berlaku tahun 2014 dengan tarif CBG untuk HD
yang dibayar oleh BPJS Kesehatan sebesar Rp 893.300 per tindakan sebagaimana tercantum dalam
Permenkes No. 59 Tahun 2014. Tagihan HD di RS C ini bagi pasien bukan JKN sebesar Rp 890.000
ditambah jasa dokter sebesar Rp 100.000. Pada pasien bukan JKN, tagihan dapat berbeda-beda tergantung jumlah obat, bahan medis habis pakai, lab dan biaya lain-lain. Biaya langsung medis RS C, sesuai
tagihan, adalah minimum Rp 721.318 dan maksimum Rp 1.145.156, dengan rerata Rp 872.718. Modus
tagihan adalah pada rentang Rp 750.000- Rp 800.000, diluar jasa dokter. Total biaya per tindakan HD
rata-rata adalah Rp 972.718. Sedangkan BPJS membayar RS kelas C sebesar Rp 893.300. Rata-rata total
biaya langsung medis sesuai tagihan RS C adalah Rp 7.781.744 (belum termasuk biaya investasi). Jika dimasukkan dalam tarif, akan terlihat selisih yang lebih besar. Informan di RS tersebut mengatakan bahwa
RS swasta baru mencapai BEP jika tarif CBG sebesar 1,5 kali tarif kelas C yang berlaku sekarang.
Rerata biaya transpor di RS kelas B sebesar Rp 302.130 perbulan dengan rentang Rp 0 Rp1.200.000. Sedangkan di RS kelas C biaya transpor rata-rata sebesar Rp 204.830 per bulan, dengan
rentang Rp 0-Rp 880.000. Rerata biaya makan di RS kelas B sebesar Rp 264.130 perbulan, dengan
variasi Rp 0 Rp 1.600.000 per bulan. Responden di RS kelas C menghabiskan biaya makan rata-rata
sebesar Rp 129.670 per bulan (rentang Rp 0- Rp 480.000). Rata-rata biaya langsung total non medis di
RS kelas B sebesar Rp 566.260 dengan variasi Rp 0 - Rp 1.920.000. Sedangkan di RS kelas C, rerata total
biaya langsung non medis sebesar Rp 334.500, dengan variasi Rp 0 - Rp 960.000. Biaya tidak langsung
HD selama sebulan di RS B Rp 165.530 dan di RS C Rp 45.830.
Rerata total biaya HD pada RS kelas B (Rp 6.149.285) lebih rendah dari total biaya HD di RS kelas C
(Rp 8.162.077) karena perbedaan faktor biaya investasi. Pergub membatasi tarif RSUD tidak boleh lebih tinggi
dari biaya satuan diluar investasi. Maka median total biaya HD di RS kelas B sebesar Rp 5.755.331, lebih
rendah dari median di RS kelas C sebesar Rp 8.069.744. Rerata biaya di RS B berhubungan dengan sesi HD
yang lebih lama (5-6jam) dibandingkan sesi HD di RS C (4-5 jam) sehingga responden lebih ban-yak
kehilangan waktu produktif. Ilustrasi visual perbadingan biaya disajikan dalam gambar 2.
Semua responden di unit HD di kedua RS merupakan peserta JKN. Responden mengaku harus
membayar sebagian biaya yang tidak ditanggung BPJS, seperti biaya vitamin, obat yang tidak terkait penyakit GGK/HD, ataupun obat yang atas keinginan pasien sendiri. Beberapa pasien mengaku mendapat
obat yang diresepkan oleh dokter RS namun mereka enggan menebusnya di apotek RS karena antrian
yang panjang. Rerata biaya yang dibayar pasien di RS B adalah Rp 157.470 (Rp 0 Rp 1.450.000) per
bulan. Sedangkan rerata biaya yang dibayar pasien di RS C adalah Rp 203.750 (Rp 0-Rp 3.000.000).
Menurut perspektif RS, dari wawancara mendalam, secara umum tidak terdapat perbedaan bi-aya
operasional yang dikeluarkan oleh kedua RS untuk HD. Persyaratan layanan HD menurut Permenkes
812/2010 tidak membedakan kelas RS. Tindakan yang dilakukan sama, prosedur sama, mesin dan bahan
habis pakai juga sama, semua komponen lainnya pun sama. Kondisi pasien yang dilayani (severitas, komplikasi, dll) pun tidak berbeda. Tetapi, ada perberbedaan biaya investasi. Bagi RS C yang milik swasta;
semua investasi harus ditanggung sendiri, sementara di RS B yang milik pemda, investasi maupun biaya
operasional didanai pemerintah.
Beberapa kendala yang dikeluhkan kedua RS antara lain tarif CBG untuk HD rawat inap tidak
mencukupi, ketersediaan sumber air yang mahal, banyak pasien anemia yang membutuhkan biaya besar,
pemeriksaan SI-TIBC atau transfusi, dan penggunaan dialiser

sebesar 10,26 g% berbeda bermakna dengan rata-rata Hb di RS C yang hanya 8,21 g% (p= 0,000). Penelitian (Frankenfield DL, 2002) menemukan asosiasi signifikan antara nilai rerata Hb dengan rerata Kt/V
dan nilai serum albumin yang menggambarkan pencapaian adekuasi dialisis. Pada pasien hemodialisis,
anemia terjadi karena dialisis yang tidak adekuat (Stefansson, 2011; Locatelli et al, 2003 dalam Aprilianti
R, 2013). Penelitian Aprilianti (2013) menunjukkan bahwa HD yang tidak adekuat berpeluang dua kali
menyebabkan anemia dibanding dengan HD memadai.
Perbedaan rerata Hb tersebut berkaitan erat dengan penanganan anemia dan tarif CBG. Dalam
wawancara, responden mengakui bahwa biaya transfusi darah di RS C dibebankan kepada pasien. Karena
banyak pasien tidak sanggup, sebagian pasien tidak melakukan transfusi meski kadar Hb nya rendah. Selain itu
di RS C pasien hanya diberikan hemapo 2x dalam sebulan, pasien harus bayar sendiri hemapo ke-3 dst.
Sedangkan di RS B transfusi darah dan hemapo diberikan sesuai kebutuhan pasien tanpa biaya pada pasien.
Prilaku RS menyiasati beban biaya ini sejalan dengan studi Dor (2007) yang mengindikasikan bukti kuat
bahwa tingkat pembayaran mempengaruhi prilaku RS.
Indikator IDWL menentukan tingkat mortalitas pasien hemodialisis (Szczech et al, 2002; Tentori et al,
2012; Foley et al, 2002; Saran et al, 2003; Kalantar et al, 2009; Szczech et al. 2002). Persentasi IDWL
berkorelasi dengan berat badan (BB) pasca dialisis, indeks massa tubuh yang lebih besar, dan kadar natri-um.
Pada penelitian Foley et al. (2002) IDWL/BB >4,8% menaikan hazard ratio (HR) menjadi 1,12. Saran et al.
(2003) melaporkan risiko mortalitas pada IDWL/BB >5,7% menhasilan yang sama HR= 1,12. Studi Kalantar
et al (2009) di Amerika mendapatkan risiko mortalitas meningkat (HR 1,25) pada IDWL/BB 4
%. Hasil pengukuran rerata IDWL pada penelitian ini didapatkan hasil yang tidak berbeda di antara kedua
RS. Rerata IDWL pada RS kelas B sebesar 0,0403 (4%) tidak berbeda bermakna dengan rerata IDWL di RS
kelas C dengan IDWL=0,0438 (p= 0,188). Artinya, keluaran HD, yang diukur dengan IDWL, di kedua
RS tidak berbeda.
Pengukuran kualitas dengan instrumen EQ-5D juga menghasilkan rerata skor EQ-5D Indeks dan
rerata skor EQ Visual Analog Scale (EQ VAS) yang tidak berbeda. Rerata indeks EQ di RS B sebesar
0,7178 dan di di RS C sebesar 0,7208 (p= 0,94). Rerata EQVAS di RS B sebesar 64,74 juga tidak berbeda

bermakna dengan rerate EQVAS di RS C sebesar 64,79 (p= 0,986). Terdapat konsistensi pengukuran utilitas dengan indeks EQ-5D dan indeks EQ VAS, dimana responden dengan indeks EQ-5D yang tinggi juga
memiliki indeks EQ VAS yang tinggi. Hasil pengukuran keluaran kualitas hidup pasien HD yang telah
dua tahun menjalani HD di kedua RS tersebut menunjukkan tidak ada perbedaan bermakna.
Namun perlu difahami bahwa secara umum pasein GGK umumnya memiliki HRQOL yang lebih
buruk dibanding populasi umum (Morales et al, 2007; Reina-Neyra et al, 2008; Varela et al, dalam
Reyna-ga-Ornelas, 2011). Skor indeks EQ-5D di kedua RS berkisar pada angka 70 dan EQVAS pada
angka 0,71. Skor maksimum (sehat sekali) indeks EQ adalah 100 dan EQVAS adalah 1.Dapat
disimpulkan bahwa skor sekitar 70% dari nilai sehat sekali pasien HD merupakan skor status sehat yang
sama baiknya di kedua RS tersebut. Faktor kemandirian pasien memberi nilai tinggai pada EQVAS. Hal
ini sesuai dengan temuan Abraham & Ramachandran (2012) yang menyatakan turunnya kualitas hidup
pasien HD disebabkan oleh ketergantungan pada dialisis. Tindakan HD memicu rasa ketidak puasan diri,
ansietas, dan depresi. Masalah finansial memperburuk kualitas hidup pasien HD.
Karena kini tersedia pilihan Peritoneal Dialisis (PD) atau transplantasi, riset perbedaan kualitas hid-up
pasein GGK dengan HD dan dengan PD di Indonesia perlu dikaji dan JKN dapat memberikan layanan dengan
hasil kualitas hidup yang lebih baik. Tajima et al. (2010) menemukan penurunan kualitas hidup pa-sien HD
berhubungan dengan progresivitas GGK, anemia, kurang gizi, hipertensi, diabetes, atau penyakit
kardiovaskular. Penelitian Lee et al. (2005) melaporkan pasien transplantasi ginjal memiliki kualitas hidup
yang lebih baik (skor 0,712) dibanding dengan pasien HD (skor 0,44) dan pasien PD (skor 0,569).

RSCM yang lebih dari dua kali biaya HD di RS kelas C sama sekali bukan merupakan kebijakan publik
yang berkeadilan. Hal itu merupakan pemborosan besar dana JKN yang menguntungkan tenaga kesehatan
di kota besar.
Besaran tarif CBG, secara umum, yang jauh lebih besar di RS kelas A dan tarif di RS khusus seperti di
RSCM merupakan kebijakan publik yang tidak berkeadilan dan tidak mendorong pemerataan layanan. Dengan
tarif yang lebih besar di RS kelas A dan khusus, yang ada di kota-kota besar, para spesialis justeru didorong
atau diberi insentif untuk bermigrasi ke kota. Padahal, kebutuhan spesialis di pedesaan atau kota/ kabupaten
kecil, justeru tidak terpenuhi. Tarif CBG yang layak dan prospektif/futuristik justeru harus diba-lik, yaitu lebih
besar di RS kelas C dan kelas D serta lebih besar di RS milik swasta agar terjadi insentif untuk dokter spesialis
dan investor swasta memberikan layanan kepada penduduk Indonesia di pedesaan atau di kota/kabupaten kecil
yang selama ini tidak mendapat layanan yang sama. Hal ini sesuai dengan azas Keadilan Sosial yang secara
Nasional kita sepakati dalam Pancasila.
Sementara bearan tarif CBG yang ditetapkan Kementrian Kesehatan berbeda cukup besar. Perbedaan biaya HD di kedua RS dalam penelitian ini hanya kerena perbedaan biaya investasi yang tidak dikompenasi
oleh besaran tarif CBG. Seharusnya besaran tarif CBG untuk HD dibedakan berdasarkan kepemilikan RS,
bukan berdasarkan kelas RS. Penetapan tarif khusus HD di RSCM yang lebih dari dua kali tarif HD di RS
kelas C merupakan pemborosan dana publik dalam JKN.
Peneliti menyarankan agar secara bertahap, namun segera, besaran bayaran CBG, khususnya untuk
HD direvisi untuk keadilan pembayaran sesuai dengan rata-rata biaya pasar (average market cost) atau harga
keekonomian yang layak. Besaran tarif HD harus sama antara RS kelas A sampai kelas D, tanpa ada besaran
HD di RS khusus. Agar besaran tarif CBG memenuhi harga keekonomian dan perdebatan perbe-daan tarif
dikurangi, disarankan agar Kemenkes atau BPJS Kesehatan melakukan atau menyediakan dana yang memadai
untuk riset komprehensif penetapan tarif harga keekonomian yang berkeadilan.

Abstrak
Defisit layak disandang sebagai penyakit kronis JKN. Indikasi defisit terungkap dari angka rasio
klaim. Pada tahun 2014 dan 2015 angka rasio klaim selalu berada diatas 100%. Angka ini merupakan
hasil pembagian biaya klaim (atau biaya kesehatan peserta) dengan pendapatan iuran. Dengan demikian
rasio klaim menggambarkan penyerapan dana iuran untuk biaya kesehatan saja. Padahal pendapatan iuran
juga harus dialokasikan untuk biaya operasional dan cadangan.
Defisit JKN akan terus bergulir jika terapi sistemik nihil. Untuk tahun 2016, hasil estimasi penulis
dengan merujuk pada asumsi besaran iuran sesuai Peraturan Presiden No 28/2016 (Sekretariat Kabinet, 2016)
dan tarif pelayanan Permenkes 59/2014 (Menteri Hukum dan Hak Asasi Manusia, 2014) menemukan angka
rasio klaim 101%. Artinya, pendapatan iuran masih kurang meski hanya untuk mendanai pelayanan kesehatan.
Dari mana sumber dana untuk mendanai biaya operasional? Apakah JKN hanya mengandalkan suntikan dana
pemerintah? Label penyakit kronis defisit layak disandang oleh JKN. Apa obatnya?

atau bloody discharge (Qian et al., 2011) dengan harapan pasien berobat kembali. Bloody discharge juga terjadi akibat alokasi dana DRGs untuk perawatan pasien sudah terserap semua. Pada
pelayanan rawat jalan, kaveat DRGs dapat berupa pemecahan kasus menjadi beberapa kunjungan
yang mendorong lahirnya readmisi (Bjorvatn, 2013). Tidak heran jika DRGs, kecuali Indonesia,
tidak lazim digunakan untuk membayar provider yang memberikan layanan rawat jalan. Altrnatifnya discount on charge yang digunakan di sejumlah negara, atau ambulatory payment
classifica-tions yang dipakai oleh program medicare di Amerika.
Pemborosan
Nihilnya pengendalian membuka peluang subur utilisasi abnormal yang berujung pemborosan. Adalah sistem jaminan kesehatan di Jerman yang menerapkan audit medis untuk menelisik
rekam medis, minimal 10% dari kasus klaim DRGs, yang diambil acak per tahun dari sejumlah rumah sakit sampel. Tujuannya untuk menilai apakah pasien jaminan memperoleh layanan keseha-tan
sesuai standar pelayanan, dan untuk mengklarifikasi apakah tagihan yang diajukan rumah sakit
sejalan dengan diagnosis, serta jenis pelayanan yang diberikan oleh rumah sakit kepada pasien.
Di Indonesia, celah untuk meraih efisiensi terbuka lebar. Berapa triliun rupiah dana JKN yang
bisa dihemat? Analisis data klaim INA-CBGs sampai bulan pembayaran Agustus 2015 me-nemukan
76% klaim rawat jalan (atau 36 juta kasus) merupakan kasus-kasus readmisi. Memang tidak semua
kasus readmisi tergolong bermasalah, namun ada sekitar 34% (12.6 juta kasus) ter-golong sebagai
readmisi bermasalah dan diduga akibat motivasi SID. Dugaan SID diidentifikasi dengan kriteria jeda
kunjungan pasien ke faskes sama untuk jenis penyakit yang sama antara kun-jungan berikut dengan
sebelumnya berada dalam rentang maksimal 7 hari. Nilai klaim atas kejadi-an readmisi dengan jeda
antara kunjungan sebelum dan terakhir selama maksimal 7 hari mencapai
Rp 5.1 triliun.
Selain pelayanan rawat jalan, utilisasi abnormal terdeteksi pula pada pemanfaatan rawat inap.
Angka bloody discharge yang mendorong kejadian readmisi juga menyedot dana signifikan
(mencapai 4% dari total klaim rawat inap). Ada indikasi pula kasus klaim yang mengarah pada
dugaan upcoding (Dafny, 2005, Berta et al., 2010) yang jika dihitung dampaknya terhadap biaya
nilainya fantastis. Pembuktian empiris kasus-kasus upcoding harus dilakukan melalui audit medis.
Estimasi total biaya klaim INA-CBGs yang sudah dibayarkan oleh BPJS Kesehatan sampai dengan bulan pembayaran Agustus 2015 mencapai Rp 55,9 triliun. Seandainya saja readmisi
dan SID pada pelayanan rawat jalan, serta dugaan praktik upcoding dan bloody discharge pada
jenis pelayanan rawat inap terdeteksi oleh radar audit medis seperti halnya di Jerman, perhitungan penulis menemukan efisiensi dana mencapai sekitar 12.6% dari total klaim. Untuk itu, BPJS
Kesehatan harus mengembangkan sebuah sistem yang sanggup untuk mendeteksi kasus-kasus
tersebut. Algoritma sistem sangat mudah dikembangkan, dan cukup sederhana. Luaran dari
sistem pendeteksian ini adalah basis data kasus-kasus klaim yang teridentifikasi bermasalah.
Data ini selanjutnya digunakan sebagai dasar untuk menelusuri kasus klaim di lapangan dalam
program audit medis. Dengan demikian, ketika melakukan aktivitas audit medis, para auditor
tidak harus membokar jutaan klaim, namun cukup melacak kasus klaim yang sudah masuk dalam
daftar ber-masalah. Cara cerdas inilah yang harus dilakukan.

Strategi kedua untuk meredam defisit JKN dapat dilakukan dengan menggencot pendapa-tan
JKN. Pendapatan dipengaruhi oleh besaran iuran dan jumlah peserta. Untuk itu, langkah siste-

mik yang harus dilakukan adalah menaikkan nilai iuran. Sayangnya revisi iuran yang dituangkan
dalam Peraturan Presiden 28/2016 (Sekretariat Kabinet, 2016) masih jauh berada dibawah nilai
ideal. Tampak disini pemerintah ragu menjadikan JKN sebagai produk superior.
Cara kedua membenahi pendapatan adalah membangun tata kelola kepesertaan. BPJS Kesehatan tidak hanya focus mendorong jumlah peserta, tetapi harus memperhatikan pada kelom-pok
mana prioritas harus dibidik sedini mungkin, serta bagaimana memastikan mereka konsisten
membayar iuran. Sistem inilah yang harus dibangun. Rubah pola pikir atau mindset masyarakat
untuk menyikapi klausul wajib dalam UU SJSN menjadi sebuah kebutuhan. Mereka yang bu-tuh
tentunya akan berupaya memenuhi kebutuhannya. Sholat lima waktu adalah rukun Islam, dan
kewajiban mutlak bagi kaum muslim. Seorang muslim yang taat akan selalu mengerjakan Sholat
sebagai wahana dalam memenuhi kebutuhan melepas rindu untuk berjumpa dengan Allah. Ini contoh
perubahan mindset kewajiban menjadi kebutuhan yang tampaknya perlu ditularkan bagi masyarakat
Indonesia dalam menyikapi JKN. Sosialisasi dan edukasi publik terkait keberadaan dan seluk beluk
JKN harus dilakukan terus menerus untuk membangun mindset tersebut.

perlu digunakan untuk kemudian dilebur dalam skim Kapitasi dan INA-CBGs? Ini juga
pekerjaan lanjutan yang harus difikirkan sedini mungkin.
Kesimpulan
Dari uraian dimuka, artikel ini menyimpulkan bajwa ada empat intervensi sistemik yang
bisa dijalankan untuk meredam defisit: rasionalisasi harga, pelembagaan pengendalian, revisi
nilai iuran, serta membangun manajemen kepesertaan. Keempat intervensi sistemik tersebut
harus dilakukan simultan, serta melibatkan semua pelaku inti dalam program JKN.
Implementasi berbagai kebijakan yang terkandung dalam program JKN (missal reformasi
pembayaran provider: Kapitasi dan INA-CBGs) tengah mendatangkan pekerjaan baru. Mempertahankan apa yang baik, dan berupaya terus untuk memperbaiki kelemahan adalah hal logis. Ini
terjadi diseluruh dunia. Reformasi jaminan kesehatan tidak pernah berhenti. Maju dan benahi
terus JKN. Rakyat BUTUH, jangan sampai mereka membencimu.
