Introduction
Intended Use
3Shapes Ortho System is a 3D scanning and software system used for:

Recording the topographical characteristics of teeth by scanning either physical

gypsum-models or impression-models of full arch mandibular and maxillary dental
arches
Accessing and managing scanned 3D models
Measurement, analysis, inspection and visualization of scanned orthodontic models
Virtual Planning of orthodontic treatments
3D appliance design based on scanned orthodontic models
Warning! The software does not contain any functionality attempting to check
whether any such real-life implementation attempt might result in damage, e.g. to
the patients teeth or jaw. Any treatment attempt to implement on a patient a tooth
movement or other manipulation visualized by way of the software should be planned
and made only by a dental professional having with due diligence assessed whether
the visualized manipulation can actually be implemented without causing damage to
the patient.

Indications for Use

Ortho System is intended for use as a medical front-end device providing systematic
inspection, detailed analysis and virtual appliance design options before the start of an
orthodontic treatment. It can also be applied during the treatment to inspect and analyze the
progress of the treatment. And finally at the end of the treatment to evaluate if the outcome is
consistent with the planned/desired results.
Contraindications: no specific contra-indications. It is the clinicians responsibility to assess
whether some specific treatment planning decisions (for example tooth movements, tooth
extractions or inter-proximal reductions) can be carried out in practice during the treatment.

Software Device Performance

Ortho System has been developed with the intention of providing maximum safety and
accuracy with regards to analysis results, measurements, tooth movement simulations and
virtual orthodontic appliance designs. The actual performance of the software is limited by the
quality, resolution and accuracy of its input data (scan surface) and the users decision on how
to perform the measurements and designs.

2.1.3.2 Open Model Set

In order to complete this step, it is required that there is a patient case created (see chapter
New Patient). Having opened a patient model set in the session window (see the chapter
Patient Model Set) you may employ the
Open model set option to see the record of just
the patient case specific to the model opened. The function is provided in order to open a
different model set for the same patient and to compare models while making a complete
analysis of a single model set.

2.1.4 Starting a New Session

2.1.4.1 Import the Third Party Model Sets
This section explains how to import a scan generated from a non-3Shape Scanner.
Watch also video:
How to import and export data
Warning: Please assure that any scan models imported into the system are
representing the specific patient dentition in adequate quality.

30

To start an analysis session it is first necessary to open existing models for inspection.

1. Click
Open models in the Main toolbar. The empty Open models window pops up
suggesting browsing for a model in .dcm, wrl or .stl formats.

General:
Select either of the 2 models to automatically locate the complimentary model and open the
model set.

31

Maxillary model:
Choose a maxillary model only, to load into the system.

Mandibular model:
Choose a mandibular model only, to load into the system.

2. To browse for a model set in the Orthodontics Control Panel, use the Browse buttons under
each of the blank fields. The default path to the folder with files of the patient database is
C:\Program Files\3Shape\OrthoData.

You may browse exclusively for the maxillary or mandibular models using the Maxillary
model or Mandibular model fields of the window.
If both parts of a model set are required, then it is possible either to select them using
the option described above or to browse for any of the two models in the General field.
The system automatically locates the pair model in the same folder to form a properly
occluded double-cast for further inspection.

32

It is possible to load CT Scan file at this stage. Click Browse and follow the same procedure as
described in the DICOM section.
3. Check Show preview to see the selections made in the preview boxes to the right of the
window.
4. Check Decimate db models to...triangles to decrease the number of triangles on the
model to a predefined value.

33

Note: Decimation might be required if the imported model contains too many
triangles for proper processing in the software. The decimation process will not affect
the geometry of the original model but rather optimize the number of triangles to
preserve it.
5. Click Rotate to set the appropriate rotation angle for the relational positioning of the
maxillary and the mandibular models. In the new window which pops up immediately after you
push the Rotate button, move the Rotation degrees sliding bar (minimal value being -180,
maximal 180) to bring the rotation angle to the necessary value. View the changes made in
the preview boxes provided, to help you to control the maxillary, mandibular and the occluded
modes rotation.

6. Click Load to display the selected model(s) in the session window and start the analysis
session.
Warning: Please assure that any scan models imported into the system are
representing the specific patient dentition in adequate quality.

34

2.2 Exporting
The Export section allows you to make changes the settings for the Export option in the Ortho
System Appliance Designer and Ortho Analyzer files destination, unique file names etc. It is
recommended to use this feature for all import and export of model sets, rather than copying
the files from the Windows Explorer.
Watch also video:

SCANNERS
3Shape offers a flexible range of advanced dental scanners for 3D scanning of both impression
and gypsum models.
3Shapes Ortho Scanners - series R500, R700, R900, R1000 and R2000 give increased
productivity plus customer satisfaction through faster delivery and superior results.
The R-series scanners allow you also to scan models directly as poured from the impression
tray without the need for a base.
Easy and quick object fixation followed by a few clicks in the scanning software makes the
scanners easy to use and require a minimal amount of training.
The patients original occlusion is registered using the provided 2-cast fixture and can easily be
altered on-screen with the help of the virtual articulators.

61

SCANNING SOFTWARE
ScanServer/ScanSuite
The 3Shape ScanServer/ScanSuite applications work as a driver to pair a 3D scanner with the
scanning software/firmware (refer to chapter ScanSuite for more details).
Your scanner comes with either ScanServer or ScanSuite application depending on the scanner
model you have purchased:

Content of the calibration box

The R-series scanner is tested, sold and delivered together with an approved PC.
3Shape does not guarantee the functioning of the R700 if:

third party software is installed on the ScanServer PC;

third party computer peripherals are installed (e.g. USB flash key, WLAN adapters);
the software is installed on a different PC.

64

Note: The default Windows user name and password for the 3Shape PC is "scan".

5.1.2 ScanSuite
The 3Shape ScanSuite application program works as a software driver/firmware to connect the
scanner hardware and the scanning applications e.g., ScanItOrthodontics.
Therefore, its function is to provide a low-level hardware communication with the scanner and
to output the 3D point cloud based on the scanner cameras input. Whenever a scan is to be
performed, the ScanSuite needs to be "ON" and ready for operation.
Connect your scanner to the PC using the USB cable. USB (Universal Serial Bus) cables are
standard data transmission cables. The flat end of the cable is plugged into the computer, the
square end into the scanner. The USB cable should be plugged into one of the rear ports of the
computer.

Connect the power supply to the scanner power supply port and plug the power supply to the
power outlet. Use standard precaution when operating the scanner power supply.
65

Caution! We strongly recommend that both the scanner power supply and scanserver PC power cable are plugged into a UPS unit (rather than directly into the
mains outlet) to prevent work losses and damages to the equipment.
With the launch of ScanSuite, an icon with a flashing green light appears in the
system tray. When this green light becomes steady, the ScanSuite is ready for
operation.
Launch the ScanSuite application only after having plugged the scanner to the scan-server PC
and switching the scanner "ON"; otherwise, the following error message will pop up: "No
scanner is connected to the PC by USB. Please check the power and USB cable and try again".

5.1.3 ScanIt Manager

ScanIt Manager is used for scanning with R2000/R1000 scanners. Follow the next workflow to
perform scanning.
1. Create a new patient in the ScanIt Orthodontics application:

2. Choose the model to work with. Right-click it and select Scan:

66

ScanIt Manager will open:

(1) Scanning settings - this tab displays instruction on how position plates in the scanner. In
the Interface plate drop-down menu it is possible to select the Occlusion setup tool. This allows
you to avoid additional scanning of occlusion.
67

(2) Scanning steps:

Scan - takes you back to the ScanIt manager menu
Maxillary - starts scanning a maxillary
Mandibular - starts scanning a mandibular
Occlusion - indicates scanning the occlusion
Save - allows you to inspect and save the scan.

3. If you use the occlusion setup tool, attach the maxillary and mandibular models to the
centers of the occlusion setup tool plates with the scanning clay and align them in the occlusion
setup tool. Unlock and remove both models from the occlusion setup tool while being careful
not to move them from the plates.
Note! Before using the occlusion setup tool, you must calibrate the scanner
accordingly (refer to chapter Occlusion Setup Tool Calibration for details).
Otherwise, continue from step 4.
4. Insert the maxillary model into the scanner and click Next to start scanning. Repeat the
same for the mandibular model.
Note! With R2000 scanner, you can scan both maxillary and mandibular models at
once.

68

5. It's possible to trim the scanned models after the scanning if needed. Click Next to
continue.

69

6. If you have chosen scanning with the occlusion setup tool, the Occlusion step is performed
automatically, otherwise you can use the 2-cast fixture to scan occlusion (refer to chapter Scan
Occlusion for details). Inspect the scanned model and click Exit:

5.1.4 Scanner Calibration

By calibrating the scanner the internal scanner geometry and camera parameters are
calculated. It is recommended to calibrate the scanner each time it is moved or at least twice a
week. However, if the scanner is placed on a stable table and at a constant temperature, the
calibration intervals can be increased. A calibration is always required when a scanner is first
unpacked.
An indication of a poorly calibrated scanner is a scanned model with a rough surface. When this
happens, check if it is a recurrent problem, as it might be attributed to the object not properly
placed on the fixture, or the fixture is not firmly fixed to the black interface plate. Try scanning
different objects, and in particular objects of different materials. If the problem persists, please
contact your 3Shape re-sellers customer support. No other maintenance besides calibration is
required for the scanner.
Caution: Beware that the scanner is a precision mechanical device sensitive to
temperature and power variations as well as vibrations.
The calibration is performed using the dot plate calibration object (see image below). The
procedure is simple and nearly completely automatic:
1. Click on the ScanSuite icon in the system tray, and the ScanSuite window will pop-up.
Press Calibrate scanner button on the ScanSuite window in the lower right part of the
70

screen. You will be prompted for a password, which is docal. The reason for the
password is to keep unauthorized personnel from trying to calibrate the scanner.
2. A new window opens. Follow the detailed instructions in this window, which will guide
you through the full calibration process.

Calibration "dot plate"

Note! Once the scanner calibration with the "dot plate" calibration object is done,
additional scanner calibration with the occlusion setup tool and its plates (both
purchased separately) is required if you want to use the automatic occlusion option
(for more details, refer to chapter Occlusion Setup Tool Calibration).

5.1.5 Occlusion Setup Tool Calibration

The occlusion setup tool and its plates (both occlusion setup tool and its plates are not included
into the box, therefore purchased separately) are used to align both upper and lower models
and then automatically transfer the occlusion to the software, which allows you to perform the
Occlusion step automatically.

71

Occlusion setup tool and its plates

The use of the occlusion setup tool and its plates requires additional calibration after the
scanner has been calibrated with the "dot plate" calibration object. Please follow the steps
below to perform the occlusion setup tool calibration.
Note! The following calibration procedure applies only to the scanners compatible
with the occlusion setup tool and its plates (all 3Shape R-series scanners except
R250 and R640).
1. Open ScanSuite
Click the ScanSuite
icon in the system tray to launch ScanSuite (refer to the chapter

Data folder is specified in the Control Panel). All scan data for the current scanning job will be
automatically saved to this folder. It is important to note that patient and model set
information forms should only be filled out from a scanning PC (i.e. not from a PC in the Postprocessing mode please see the Processing on a Second PC chapter for details).
You can create a new patient at any time since the Create new
order button (image to the left) is available throughout the whole
scanning process.

75

5.2.3 Scanning Dental Models with ScanItOrthodontics (SIO)

Now that all relevant information has been entered, you will be guided through the steps
needed to scan all models. Essentially, the only thing you need to ensure is that the models are
inserted in the correct way into the scanner:
Warning: The front teeth should always face the inside of the scanner, and there
should be approximately 1cm of blue clay underneath the model, holding it firmly in
place.
A typical scanning job will consist of three scans: the maxillary model, the mandibular model,
and the occlusion model (which consists of the maxillary on top of the mandibular, in correct
occlusion, e.g. using 3Shapes specially designed fixture). After all three scans are complete,
you will be asked to help the software in performing the alignment.

After the mandibular model has been scanned, the application offers you to insert the occlusion
model, i.e. both the maxillary and mandibular models together, using the provided 2-cast
fixture.
Put the models in occlusion (potentially using a wax bite if one is necessary) and close the
fixture on the two models as illustrated in the image to the left. Insert the fixture with the
models front teeth facing the inside of the scanner. After the occlusion scan, press the fixtures
top button to release the models:

79

Note that the Rescan mandible button has appeared in the Function panel so you can rescan
the mandibular model if necessary.
Use the Sculpt mandible option if you want to trim your scan (for more information see the
Scan Mandible chapter).
Insert the occlusion correctly and click Next.

80

5.2.3.4 Model Alignment

After scanning has been finished, the program displays all three scans and asks you to align
the models. There are three ways to do it: Normal, Advanced and Auto. You can choose
either of them by selecting the corresponding radio button (see image below).
Warning: Please assure that the occlusion is correct after scanning by using the
miscellaneous available occlusion-adjustment tools

Normal method

The alignment is performed by placing the corresponding points on the models. The hints on
how to place the points correctly are illustrated at the top of the Register scan alignment form
(see the image below).
Click on the maxillary model, somewhere near the front teeth (try to avoid flat areas). Then
click on the corresponding area on the occlusion model. Do the same thing for the mandibular
model, thus resulting in 4 points. You can change the position of a point by dragging it (click
and hold down the mouse).

81

If you are satisfied with the result, click Apply to perform the alignment. Once the alignment is
complete, you can click OK to accept it, or click one of the Reset buttons to try again using
different points. You can use the Models visibility form in the upper right corner to show the
occlusion model at any time this can help you evaluate the quality of the alignment, as the
occlusion model is of a different color.

Note that there is a 2D cross-section tool, as well as the Difference map, in the
Visualization toolbar. These tools (particularly the 2D cross-section) can help you evaluate
the quality of the alignment.

Advanced method

If for some reason the Normal alignment method does not produce a satisfactory result, you
can click Reset all and switch to the Advanced mode. With this method, you must set 3
points on each model, as illustrated in the image below. The order of operations is:
1.
2.
3.

5.
6.

Set 3 points on maxillary model.

Set 3 corresponding points on occlusion model.
Click Apply.
Set 3 points on lower part of occlusion model.
Set 3 corresponding points on mandibular model.
Click Apply.

The hints on how to place the points correctly are illustrated in the Register scan alignment
form (see image below).

82

Auto

To avoid the manual alignment and save time, you can use the Auto alignment option. Click
Apply and the scans will be adjusted by the program.
It is possible to adjust the transparency of the models by dragging
the appropriate sliders. To activate the slider mark the checkbox in
front of it first.

It is possible to realign models or skip any steps you like.

5.2.4 Scanning Dental Impressions with ScanIt Ortho Impression

(SIOI)
When everything is setup and ready for scanning, you start the application.
ScanIt Ortho Impression is started by double-clicking the ScanIt Ortho Impression desktop icon

or via the Windows Start menu: Start->All Programs->3Shape->3Shape ScanIt Ortho

Impression.
Warning: Since 3Shapes R-series lab scanners use optical technology (i.e. laser
light or LED source), the impressions surface must not be too wet/reflective. It is
therefore recommended to wipe excess liquid from the alginate impressions surface
using a tissue, or to use a spray on a dry surface in the case of too shiny silicone
materials. A spray can also be applied to the material used for the bite registration
(wax or silicone).
Important remarks on dental impression scanning with the R700:

84

Using the ScanIt Ortho Impression application allows you to scan full dental impressions
for orthodontic applications.
Due to the complexity of the impressions geometry, and the use of 3Shapes unique
adaptive scanning method to automatically cover this geometry, the scan of dental
impressions is typically slower than for dental casts.
Alginate and Silicone impressions can be scanned in the R700 scanner. Most impression
materials can be scanned but 3Shape will publish a list of materials, which are known to
cause scanning issues and should be avoided.
Since the R700 scanner uses optical technology (i.e. laser light source), the impressions
surface must not be too wet/reflective. It is therefore recommended to wipe excess
liquid from the alginate impressions surface using a tissue, or to use a spray on a dry
surface in the case of too shiny silicone materials. A spray can also be applied to the
material used for the bite registration (wax or silicone).
The impression must be scanned in its original tray.
Note: To fit into the scanner, the impression trays handle must not be more than 4
cm long and 0,7cm thick. The impression handle must preferably be straight so that
the impression is not elevated in the scanner.

5.2.4.1 Model Set ID Form

Once the New patient info form has been filled out, there appears a New patient model set info
form. In the appeared form enter the Model set ID information, which is used to identify the
scanning job you are about to perform for the current patient:

Click on the drop-down menus in the Model set date and Scan date fields to use the calendar
and choose a date (see the image above).
Notice the two model pictures on the left of the form. Clicking on one of these unselects that
model, meaning it will not be included in the scanning job.

85

A selected model is yellow (by default)

An unselected model is gray

Scan bite allows scanning the patient's bite for further model alignment. This
option is selected by default.
When you have entered the model set information, click Create to proceed.
Now that both the patient information and model set information have been filled out, a folder
called Data folder/Patient_ID/Model_Set_ID/ has been automatically created (recall that
Data folder is specified in the Control Panel). All scan data for the current scanning job will be
automatically saved to this folder. It is important to note that patient and model set
information forms should only be filled out from a scanning PC (i.e. not from a PC in the Postprocessing mode please see the Processing on a Second PC chapter for details).
You can create a new patient at any time since the Create new order button
(image to the left) is available throughout the whole scanning process.

5.2.5 Scanning Job

Once all relevant information has been entered, you will be guided through the steps needed to
scan all models. Place the impression holders handle in the supplied Impression fixture.
Note: For impressions, the front teeth should always face the outside of the scanner.
If the impression material and/or tray cause the fixture to tip, place approximately
1cm of blue clay underneath the impression to hold it in place.
A typical scanning job will consist of three scans: the maxillary model, the mandibular model,
and the occlusion model (which consists of the maxillary on top of the mandibular, in correct
occlusion, e.g. using 3Shapes specially designed Waxup fixture). After all three scans are
complete, you will be asked to help the software in performing the alignment.

5.2.5.1 Scan Maxilla

After you have filled out the New Patient info form and the New patient model set info form,

model set info form appears (for more information see the sections Model Set
Form and Model Set ID Form). The new model set is saved under the active
patient and is available in the Open patient case form.

5.3 Processing on a Second PC

Scanning a model consists of two independent parts:
1. The actual scanning, during which raw scan data is captured.
2. Post-processing, during which the raw scan data is transformed into a surface.
ScanItOrthodontics allows these parts to be split onto two separate PCs, in order to save time.
Such a configuration can be set up in the Scan settings form.

104

5.3.1 Scan Settings Form

The Scan Settings form is accessed through the File menu in the top left corner:
The form contains two tabs: Scanners and Options.
The Scanners tab should essentially never be changed, except in rare cases. Fill in your
scanner's name/location in the Add scanner to list field (click Add to save) or select the
required scanner from the Current scanner drop-down menu. Click Remove to remove the
current scanner from the list.
The content of the Options tab is described below:
Mode This is where you can set up processing on a second PC. The following modes are
available.

Scanning and post-processing: The current PC will perform both the scanning and
processing (this is the default configuration).

Scanning: The current PC will perform only the scanning part. In this mode, you must
also specify the following:

105

Name of the second PC This is the computer name of the second PC. It can be found on
the second PC under the System Properties (Start -> Control Panel -> System -> Computer
Name -> Full computer name).
Note: All references to network or company name should be omitted (for example, if
the computer name in the System Properties is PC101.3Shape.local, then you
should only enter PC101 here.
Shared folder Here you specify a shared folder on your local network, or on the second PC,
which will be used to exchange files between the two PCs.
Note: If this folder is on the second PC, as opposed to the network, then it must be
specified in the form \\Second PCs name\, and not C:\..... The shared folder
must have write privileges (i.e. it must not be "Read only").

Post-processing: The current PC will perform only the post-processing part.

Scanning optimized for - presents 2 options: Speed and Universal. Speed allows for a
quicker scan scanning process. Universal is he scanning process including additional sub-scans
and post-processing steps. Note that ScanItOrthodontics is set to Universal by default.
Caution: Your configuration of the setting Scanning Optimized for (Speed and
Universal) has a direct impact on the performance of the scanning process, as well
as the hardware requirements. Thus, the Universal setting requires significantly
more virtual memory and RAM during operation when compared to the Speed
settings. Please contact your local 3Shape service provider in case of problems with
this feature.
Noise reduction mode - allows to reduce visual noise, i.e. points that are not aligned with the
captured surface.

5.3.2 Workflow
When splitting the scanning and post-processing steps on separate PCs, it is very important to
do things in the correct order; otherwise your work will not be saved! The correct order of
operations is as follows:
1. Start ScanItOrthodontics on the processing PC (see the Post-Processing Mode chapter).
2. Start ScanItOrthodontics on the scanning PC and fill out the patient and model set
information forms. Start scanning, as usual (see the Performing a Scanning Job
chapter).
3. The alignment is performed on the processing PC.
Performing and evaluating the alignment are the only things you should do on the processing
PC. Everything else must be done from the scanning PC: creating new orders, re-scanning, etc.
Note: All files are saved on the processing PC, in the location specified in the Control
Panel. Nothing is saved on the scanning PC.

106

5.3.3 Post-Processing Mode

When ScanItOrthodontics is started in the Post-processing mode, there will be no patient
information to fill out. Instead, you will immediately see the following:

At this point, the program is waiting for the scanning PC to send information. You should now
start ScanItOrthodontics on the scanning PC and enter all patient and model set information.
As soon as this is done, the patient ID will be displayed in the programs title bar on both PCs.
Therefore on the processing PC, you will see the following:

5.3.4 Switching to Post-Processing Mode from Another Mode

If your version of ScanItOrthodontics is currently in a different mode, and you would like to
switch to the Post-processing mode, please do the following:
1. Run ScanItOrthodontics. You will immediately see the patient info form (since you are
not yet in the Post-processing mode). Click Cancel on this form, thus closing it.
2. Go to the File menu and choose Scan settings. In the Options tab, change the mode
to Post-processing.
3. Click the Create new order button in the top left corner. Your PC is now ready to
receive data from the scanning PC.

5.3.5 Scanning Mode

In the Scanning mode, ScanItOrthodontics works almost exactly as in the Scan and postprocessing mode described in the Performing a Scanning Job chapter. Essentially, the only
difference is that it will not perform the alignment this is done on the processing PC. Note
also that all surface models (i.e. STL or DCM) will appear only on the processing PC.
107

Note: The scanning PC does not know anything about what the processing PC is
doing or has done. Therefore, do not create a new order from the scanning PC if
there is a pending order on the processing PC, on which you have not yet performed
alignment. Only once the alignment is complete should you define a new order on the
scanning PC (this does not apply if the pending order does not require alignment).

5.4 Updating Existing Orders in ScanItOrthodontics and

ScanIt Ortho Impression
ScanItOrthodontics allows you to load orders you have previously scanned. This can be useful
in the following situations:
1. If you want to (re-)scan the occlusion of the patient once the impressions have been
scanned. In this case, you will scan a new occlusion bite, i.e. add data to the existing
scanned data -and then perform a new alignment according to this new scanned bite, so
here you can modify an existing scanned data set without creating any new data.
2. After you have completed a scanning job, you find some problem that you had not
noticed while performing the scanning job. For example, you might think the alignment
is a bit off and would like to redo it.
3. You might feel that the file sizes of the models are too large, and would like to produce
lighter (more decimated) models.

Note: In cases 2 and 3 you will only use the existing scanned data.

To load an existing order, click the Load order button (image to the left).
Alternatively, you can use the Browse button on the New patient info form
(see the Patient Information Form chapter for details).
1. Click the Search button in the top right corner to see all cases which are currently in
your Data folder (recall that this folder is defined in the Control Panel).
2. Click the little black triangle
next to a patient: this will display all model set IDs (i.e.
scanning jobs) for this patient.
3. Double-click the scanning job you want to load.

108

Adding the occlusion scan

To add an occlusion scan to the existing scanned Model Set (i.e. new scan data for occlusion
only), please perform the following steps:
Step 1: Select the patient in the list (i.e. not the model Set). Carefully note the ID of that
model set.
Step 2: Double-click on the patient name, or right-click and choose Open Patient.
Step 3: The Model Set ID Form is displayed. Enter the ID of the model set you want to modify
(VERY IMPORTANT to modify the existing set), select the Occlusion scan option and click
Create.
Step 4: The conformation dialogue appears. Click Yes to confirm you want to overwrite the
existing Model Set:

Step 5: Click on Next to import the existing Maxillary scan and click Yes when asked whether
to load the existing data:

109

Step 6: Wait for the post-processing of the maxillary scanned data to be completed until the
model appears on screen.
Step 7: Click on Next to proceed to the Mandibular side and load existing scan (click on Yes)
and wait until the mandibular post-processing has been completed and the surfaced model is
displayed on screen.
Step 8: Insert the occlusion bite and complete with the scanning and alignment processes
described in section Scan Bite.
Redoing the alignment

Load all surface models (i.e. STL or DCM files) to redo the alignment. Go to the File menu and
select Load all surface models. This will make the models appear on the screen. You can now
click the Realign models button and redo the alignment.
Note: It is only possible to load surface models if all three models exist: maxillary,
mandibular, and occlusion. This is because this functionality is only intended for
performing re-alignment.

Re-processing the existing raw scan data

As mentioned previously, you can obtain new surface models

without re-scanning, applying alternative file reduction presets in
the Ortho Control Panel. To do this, click Next as if you were
starting a scan. You will then see the message illustrated on the
image to the right.
If you choose Yes, the scan will be skipped and post-processing
will begin immediately. If you choose No, a new scan will be
started. When post-processing of a model is complete, click Next
as you would normally do when scanning.
Note: This functionality is only possible if Save raw scan data checkbox was
selected in the Control Panel at the time of the original scan.

Hint: The existing surface models will be automatically replaced by the newly
processed models. So if you want to retain the originals, you will need to manually
back up these files before proceeding.

6.3 Function Panel

Each stage in the analysis and planning process is represented by a tab in the Function panel.
As different jobs require different functions, the Function panel changes its look depending on a
job. You are free to choose the first step in the Function panel to complete. Therefore, there is
typically no a predetermined workflow of tasks however, the tasks can be organized in
predefined workflows by using the workflow wizard (see section Customized Workflow Wizard).
An example of a workflow is the Analysis tab which deduces the analytical meaning out of the
measurements set up in the Configuration section.
A brief overview of functions available in the Function panel is presented in the table below.
Hint: Please refer to the chapter 4 for more information about each of the tools
included in the Function panel.

INFORMATION AND LEGAL DISCLAIMER window also shown below. Please read the
information carefully before proceeding.
Ortho Analyzer 2015 IMPORTANT INFORMATION AND LEGAL DISCLAIMER
The functionality of the Ortho Analyzer software allows you to manipulate a 3D visual model
of a patients dentition by dragging and moving individual teeth in the model, whereby you may
explore how the visual appearance of the dentition may change if such tooth movements are
implemented.
Please be informed that the Ortho Analyzer software does not in any way attempt to check if
any specific tooth movement or combination of tooth movements visualized in the software is
actually possible to implement on the patient in real life.
Manipulations visualized in the software may not be possible to implement in real life, e.g.
because the teeth may not actually be moved as visualized. A visualization created in the
software of any tooth movements may also be inaccurate, e.g. because such movements, if
implemented in real life, might cause more or different effects than as shown in the
visualization created within the software.
Furthermore, the software does not contain any functionality attempting to check whether any
such real-life implementation attempt might result in damage, e.g. to the patients teeth or
jaw. Any treatment attempt to implement on a patient a tooth movement or other manipulation
visualized by way of the software should be planned and made only by a dental professional
having with due diligence assessed whether the visualized manipulation can actually be
implemented without causing damage to the patient.
The risks pertaining to any such treatment attempt shall be assumed entirely by the person
carrying out the treatment.
3Shape A/S disclaims any and all liability for the results of any treatment attempts made on
the basis of the visualizations generated within the Ortho Analyzer software.
6.3.1.4.1 Setups
Setups section offers you to create new setups (scenarios) and sub-setups or choose the
existing ones.

date from the drop-down menu:

Now the selected case will be used as a base for the virtual setup.
In the Export setup window, you can fold/unfold the groups and select which models or model
sets you wish to export:

165

Note: The export of planning models (virtual setups) as STL requires an active
license of Planning Model Builder.
Note: Subsetups are divisions of a given setup (i.e. new tooth positions) into a userdefined number of steps. Treatment decisions, such as an extraction, stripping or
attachments are disabled for subsetups as they cannot be applied to the subsequent
steps in a series of subsetups. It is therefore recommended to start a new setup with
a treatment decision and then subdivide it into subsetups. Creating a copy of a given
setup can be useful to create the desired sequence.

166

6.3.1.4.2 Tooth Movements and References

The method of calculations of tooth movements in Ortho Planner 2015 has been improved to
reflect the practice of orthodontists.

6.3.2 Configuration
The Configuration step is used to prepare the upper
and/or lower model(s) for future manipulations and
analysis.
The Configuration tab contains four steps:
Setup maxillary model local origo
Setup occlusion plane
Preparation for maxillary model
Preparation for mandibular model.
These steps are described in details below.

6.3.2.1 Standard Planes

The aim of this step is to specify the local origin for the maxillary model of the virtual
cast. These predefined reference planes namely the horizontal, sagittal and anteriorposterior, can be used as basis for the 2D cross section analysis.

184

Note: these planes need to be predefined to be available as default positions for the
2D cross section analysis.

Note: if the models have been prepared with a virtual base, (see Preparation) the
occlusion and sagittal planes are already defined on the model set and the present
step can omitted

1. Click the Standard Planes

button in the Configuration tab.
2. Pick a point on the maxillary model to set up local origo and coordinate system.
3. Rotate the model using the mouse:

Drag the control points (they turn yellow when activated) to rotate coordinate axis
freely and set up the appropriate relation of the local origin planes.
Drag the arrowhead to move the horizontal plane up or down.
Drag the arrowhead to move the sagittal plane left or right.
Drag the arrowhead to move the vertical plane to or from.

4. When needed, you can click the Clear button to reset the positioning of the planes and
start the process again. Click OK to save the changes made to the local origin and leave the
step.
Note: All planes can be moved and saved as additional planes in the 2D cross-section
toolbox.

185

6.3.2.2 Setup Occlusion Plane

The Setup occlusion plane step allows you to place the occlusion plane that is used for
the 2D measurements at the Analysis objects step (Custom spline 2D, Measure 2D
and Distance 2D).
Note: The occlusion plane need to be predefined to be available as default position
for the 2D cross section analysis.
It is also required for the calculation of the tooth long axis at the preparation stage.
Note: if the models have been prepared with a virtual base, (see Preparation) the
occlusion and sagittal planes are already defined on the model set and the present
step can omitted
To setup the plane, please follow the hints on the form (see the image below). You will be
asked to place 3 landmarks on the teeth number 16, 11-21 and 26 (or equivalent depending on
the chosen tooth numbering system).
When done, the occlusion plane appears on the cast:

186

Note: All planes can be moved and saved as additional planes in the 2D cross-section
toolbox.
Click Clear to reset the plane or click OK to save and finish the step.

6.3.2.3 Segmentation for Maxillary Model

This step has 4 main objectives:

1) It lets you define the composition of the maxillary tooth row, including missing teeth to
calculate the required space. You can also set up the width measurements for missing or non
erupted teeth.
2) It also segments the teeth according to their definition so you can perform a virtual setup.
3) It defines the axis of the crown, which will be used as basis for all tooth movements at the
virtual setup stage
4) It allows to edit position of the rotation center of the tooth.
Completing this step is crucial for the further analysis and for virtual setup operations to be
successfully performed in the Function panel.
Warning: Please make sure to validate separation boundaries and tooth axes
sufficiently when segmenting teeth by using the visual inspection tools available,
notably the overlay of DICOM or X-ray 2D images.

The

Segmentation for maxillary model option contains four steps:

1:
2:
3:
4:

These steps are described in details below.

Watch also video:
How to segment teeth
6.3.2.3.1 Set Points
Set points - the first step of the preparation process.

Click on the
Preparation for maxillary model icon, the program automatically brings you
to the first step.
The system automatically presents the model of an open mouth view, making the inactive part
of the virtual cast transparent (as shown on the image).

187

At this step, you have to place the distal and mesial control points on the teeth with
corresponding tool (1) to set up the maxillary model. To accomplish this task, mentally divide
the model into two parts along the median line, and indicate the distal points (aqua) on the
surface of the tooth that is away from the median line, and the mesial points (purple) on the
surface that is towards the median line, as shown on the image below. The width value for
each tooth is displayed under Width (2D) under the Overview window.
To remove the tooth width selection, right-click on the control point and select Delete from the
appeared menu.
To move the control points, click on the control point to activate the tooth and drag the point
with the cursor.
Click on the tooth and press Clear to remove the selected tooth width.
Click Clear all to remove all the width measurements.
Show teeths axes when checked, displays the teeth axes in the form of the red
arrows. The tooth axis can be changed with the help of the Set tooth normal
button in the upper left corner of the session window. Its position is by default set
by the user-defined mesial-distal segment.
Move close points synchronously when checked, simultaneously moves the control points
that are set close together on the spline (used at the Define cut step).
(2) Place the control points on the teeth.
(3) It is possible to customize the view of the model by moving the Visualization sliders:
Option

Function
Maxilla - allows you to show/hide as well as adjust the transparency level for the
maxillary model.
Mandibular - allows you to show/hide as well as adjust adjust the transparency level
for the mandibulary model
Show all teeth axes - displays all teeth axes or hides them.
Show tooth axis angles - allows you to display the axis angles of the selected tooth.

188

It is possible to place additional landmarks on the selected tooth from landmark lists
that are created in the Control panel. These landmarks can be used optionally to
position the reference plane used to define FA point of molars.
Select a tooth on the Overview scheme, choose the preset from the corresponding drop-down
menu and place the landmarks according to their description. If to hover a cursor over the
landmark it turns white:

189

The user point-of-view should be the growing direction of the tooth. The default axis of the
tooth crown is defined by the distal-mesial segment you define here. Normally it is the default
top-view plane for the 2D measurements and for most teeth for 3D measurements. If some
teeth grow in a non-standard direction, the plane for 3D measurements should be selected
according to those individual directions.
Note: Taking a perpendicular view to each crown (using the right button of the
mouse) helps to position the mesial-distal segment correctly through the center of
the crown, therefore creating a more realistic tooth axis from the start.
At this step the DICOM files and X-Ray 2D Images will be very useful to define the teeth axes
and rotation centers:

(1) Use the Overview window to navigate around the teeth or Ctrl + arrow buttons to change
to next tooth:
The small green arrow
indicates the tooth selected.
Its number appears under
Active tooth below. The
tooth marked with cursor
becomes circled black.

To mark the missing tooth

(shown as transparent),
select the tooth and check
Missing.

If the space left from

missing tooth is intended for
specific purpose (e.g.
implant or non-erupted
tooth), click the Set button
and enter the corresponding
tooth size value in the
Width (2D) field. The
virtual tooth becomes white.

190

Additionally, you can manipulate the teeth with the right-click

menu appearing in the Overview window when the cursor is
positioned over a tooth.
The default measurements are done in 2D mode as the
software emulates the traditional manual measurement
method. The dental technician can still use the manually
calculated data with the 2D tooth width. The measurement
method can be toggled between 2D and 3D with the Use 2D
tooth length checkbox located on System Settings page of
the Ortho Control Panel.

To move to the next step, you can click either the

button or

6.3.2.3.2 Define Cut

At the Define cut step you can adjust the cut spline or recreate it
manually. The program automatically creates the spline for each tooth
based on the mesial/distal points placed at the previous step.
To adjust the spline, click desired tooth in the Overview window to activate it. The control
points of the active tooth turn green from blue. Click on the control points (turn yellow when
active) and drag them with the mouse to form the desired spline. Select the Move close
points synchronously checkbox to move simultaneously the control points that are set close
to each other. When you need to delete a control point from the spline of the selected tooth
right-click on it and select Remove from the appeared menu.
To create the spline manually or to edit it using the fast edit mode, you can use the buttons in
the upper left corner of the session window Segment tooth manually and Spline fast edit
mode. Use the Smooth teeth sides (None, Low, High) to adjust the sides of the selected
tooth.

Set Tooth Normal

When the Show teeths axes checkbox is selected, the teeth axes become
visualized in the form of the red arrows. The tooth axis can be changed with the
help of the button in the upper left corner of the session window. To perform the
operation, select the tooth to make it active, position the cast as desired and click
192

the Set tooth normal button. Note that the default position of the tooth axis is
defined by the mesial-distal segment.

Segment Tooth Manually

To place a new spline, select the desired tooth to make it active and click Segment
tooth manually button. The spline that has been placed automatically by the
program will disappear and you will be able to place your own spline. Left-click on
the tooth to place the control points. The last control point should be placed on top
of the first one to close the spline.

Edit Cut Spline

To fast edit the spline, select the desired tooth to make it active and click the
Spline fast edit mode checkbox. Click and draw the new part of the spline that
needs correction and the program will adjust the spline.

193

When you want to remove the preparation information from a tooth, click on the tooth in the
Overview window and click the Clear button. The Clear all button removes the preparation
information for all teeth at once.
Click the Find hole in the model button to identify the holes between the teeth for
better separation. Select the tooth with the holes, click the section you want to place the
spline on and click the Find hole in the model button. The separating splines are
placed:

194

After you have performed actions with a single tooth you can use the green arrows
in the left upper corner to Undo/Redo the changes (or the combination of Ctrl +
Z/Ctrl + Y).
Hint: Due to difficulty to access interproximal areas for most orthodontic cases it can
be necessary to adjust the suggested segmentation manually. In case segmentation
fails for one given tooth, it is recommended to start by repositioning the distal and
mesial points at the first step. If this is not sufficient to solve the segmentation issue,
it can also be very useful to inspect the cutting splines alone on screen (disable the
Show maxilla or
Show mandible slider respectively from the Visualization
Toolbar and adapt the spline using the Spline Fast Edit mode.

6.3.2.3.3 Sculpt

The Sculpt tool allows you to trim the model after Define cut step For more
information on this tool please refer to the Sculpt in Preparation section.
It is possible to customize the view of the model by using the Visualization sliders in the upper
right corner:
Option

Function
Teeth - allows you to adjust the transparency level of the teeth, or even turn their
visualization off.
Tissue - turns on/of the visualization of the tissue and allows to adjust its
transparency.
Show selected tooth - allows to display the selected tooth only.

6.3.2.3.4 Rotation Center

At this last step of the Segmentation of maxillary model option
you can evaluate your model and choose to go back to the previous
steps if more adjustments are needed.
Click OK to finish the Segmentation of maxillary model.
You can also edit the position of the rotation center of the selected tooth. Adjust the visual
presentation of the model and choose the tooth from the overview or directly on the 3D model.
Select the rotation center with your mouse and drag it to adjust its position. Use the control
points to set the height and angle of the tooth rotation center.

195

6.3.2.4 Segmentation for Mandibular Model

This step defines the composition and segmentation of the tooth row of the
mandibular model. The workflow is similar to the described in the Segmentation for
Maxillary Model chapter.

6.3.3 Analysis

This functional tab serves the purpose of comparing

particular patient models to statistically obtained ideal
deviations in order to plan the treatment accordingly.

196

The Analysis tab contains six steps: Dental arch analysis, Analysis reports,
Overbite/Overjet, Questionnaires, Generate report and Occlusion map.

6.3.3.1 Dental Arch Analysis

The Dental arch analysis function provides a compilation of the tooth width calculations
based on the selections made in the Configuration tab and compares it with the customerdefined ideal curve shape.
Watch also video:
How to make a Dental Arch Analysis

When you click the

Dental arch analysis button, a dialog box with the Overview
window and the Ideal Curve and Wire Length tab appears.

Overview window

The teeth of the maxillary and mandibular right quadrants are marked red. The teeth of the
maxillary and mandibular left quadrants are marked green. Missing teeth are marked grey.
The green arrows along the dental archs indicate the full arch width calculated out of the
individual tooth widths of the teeth marked in the previous steps. Both types of measurements
appear next to the teeth and the arrow in mm.

197

Ideal Curve and Wire Length tab

Before defining an arch, you have to click the Add button and enter a name for your new ideal
arch (arches). The new arch will appear in the Ideal Arches drop-down list. You can delete the
selected arch with the Remove button.
The Maxillary arch and Mandibular arch tabs offer the following options (the same for the
upper and the lower arches). Define an ideal arch based on different default shapes. There are
four types of ideal curve shapes available in the system:
Ellipse

Custom

198

Compare the existing shape of the teeth row with an ideal curve and consider the necessary
modifications to the existing shape.
Select the arch shape and place several control points on the model to set up the ideal curve:
two points for Ellipse and Parabola (the third point will appear automatically creating an arch)
and four points for Custom as shown on the image.
To change the shape of the spline, change the position of the selected control points with the
help of the cursor. For the custom made ideal curve, the original spline is marked green,
whereas the current spline turns blue, for comparison as shown on the image.

Placing control points for the Custom arch

Editing the Custom arch

Click Clear to remove the arch. Click Copy opposite arch to get a copy of the arch from the
opposite cast. Select the Show opposite arch checkbox to show/hide the arch on the opposite
cast.

Hint: Using the 1:1 real scaling

of the Visualization Toolbar, you can easily
replicate a physical arch wire by drawing it on screen with the Custom Arch option.

199

If necessary, define the distance

between the ideal curve and the
bracket in the Bracket offset box.
The Bracket offset value
corresponds to the thickness of the
orthodontic appliance (e.g. the
brackets), whereas the Offset wire
arc length corresponds to the
length of its metal wire. Entering
both positive and negative values is
possible in Bracket Offset.
Compare the Ideal arc length
(blue) and the Offset wire arc
length (green) values, to define the
treatment measurements, if any. If
the offset value equals zero, then
the lengths coincide. In the example
above, the original length of the
upper tooth row spline is
103.89mm, whereas the offset value
of 2mm changes the offset length to
109.63mm.
Click OK to finish Dental arch
analysis.
At this stage the 2D overlay feature from the 2D Cross-section option can be very useful. You
can upload a file with the desirable arch there and use it as a guidance while drawing a digital
arch:

200

Using the 2D overlay to define the ideal arc shape. Position the arc shape template on the sagittal axis and use the
symmetric drawing option

6.3.3.2 Analysis Reports

The option Analysis reports is provided to enable effective and
versatile analysis of the models. OrthoAnalyzer provides three
officially recognized analyses: Tooth width analysis, Bolton
analysis and Space analysis. The option of comparing both,
the maxillary and mandibular teeth rows with the pre-set,
standard values is enabled with the data imported into the Ortho
Control Panel.
Click the
Analysis reports button in the Analysis tab to open a window containing the
Overview section illustrating the information presented in the three dental analysis tabs. In
the Overview window, the teeth of the maxillary and mandibular right quadrants are marked
red, the teeth of the maxillary and mandibular left quadrants are marked green and the
missing teeth are marked grey.
1. Customize the Tooth width analysis settings
The Tooth width analysis tab appears in focus automatically when you open the Analysis
reports, it contains the data for this type of analysis as shown on the image below. To view a
different type of the standard tables, select it from the Standard tables drop-down menu.
The Tooth width analysis tab presents a list of the actual individual tooth widths compared to
the standard average values in order to determine the locations and amounts of the toothwidth discrepancies. New standard tables may be imported into the Material settings of the
Ortho Control Panel.

values, the user can calculate the standard deviation
between them (marked SD on the graphs).

Tooth width analysis

2. Fill in the Bolton analysis tab

Click the Bolton analysis tab to view the table data available for this type of analysis.
The red spline in the Overview window is divided in three sections. It highlights the six front
teeth and the twelve teeth closest to the labial surface on each of the models for the anterior
and the overall Bolton analyses correspondingly (see the image below).
The tables automatically shown in this tab are reproduced by default. Specific to the patient
model of an individual, the Anterior and Overall ideal ratios are customized and easily
imported into the Ortho Control Panel.
According to the example in the tables, the ideal AR for the ethnic group to which the patient
belongs to equals 0.77 whereas, its ideal OR equals 0.91.
Based on the ratios of the maxillary and mandibular six and twelve widths of the current
model, the system calculates the AR and OR deviations from the standard ratios for the given
ethnic group. It is possible to predict the necessary measures to adjust the structure of the
whole teeth row in order to provide the orthodontic treatment.

202

Bolton analysis
1. Overall analysis (twelve width)
2. Anterior analysis (six width)

3. Apply the settings for the Space Analysis tab

Click the Space analysis tab to view the table data available for this type of analysis.
This section provides two types of space analysis the Moyers and the Tanaka & Johnston
table analyses for both the maxillary and the mandibular arches. This is done to predict the
orthodontic manipulations with the spaces indicated in the setup section of the analysis
process.
The Standard Moyers table is selected by default. To switch to a different type of the
standard Moyers table, select it from the Standard Moyers Table drop-down list. This is
customizable in Ortho Control Panel.
Having calculated the sums of the incisors, canines and premolars separately, the system yields
a set of Moyers and Tanaka & Johnston values for the Total space required.

6.3.4.2 Maxillary Model Registration

The aim of the Maxillary model registration step is to properly align the upper models of the
model sets chosen for comparison. This is a necessary pre-requisite for effective and
informative analysis of differences between the model sets.
Step 1: Start the maxillary model registration
process
Click the Maxillary model registration
the Model Set Compare tab.

button in

Choose the best option for the model alignment in the

appeared Registration form:

Surface 1-point: Indicate single corresponding

points on each model for alignment and optionally
select the surface to align with the help of the
Selection Tools.
Surface 3-point: Indicate three corresponding
points on each model for alignment and optionally
select the surface to align using the Selection
Tools.
Direct 3-point: Indicate three corresponding
points on each model for alignment. This method
does not use the surface information.

215

Note: It is advisable to use the Dual view

visualization in order to access both models at
the same time. The selection of points can be
done on the reference model first and then on
the comparative model, or vice versa. Use the
Reset button to delete the reference points if
needed.

Step 2: Gain better alignment with the Selection tools

The additional surface information provided with the Selection tools improves the alignment
process. When no surface correspondences are set, the system automatically uses the whole
surfaces of the models as the reference surfaces.
These tools are only available in Surface 1-point and the Surface 3-point registration
methods as the Direct 3-point method does not refer to surfaces.
The selection tools can be used in any order and
combination.
The tools include the following:

Unselect all Unselects all selections.

216

Reduce selection Reduces the selected area by excluding

the neighboring areas from the selection.

Extend selection Extends the selected area by adding the

neighboring areas to the current selection.

Invert selection Unselects the selected and selects

unselected areas of the surface.

Undo last selection Makes one step back in the selection

operation.

for comparison. This is a necessary pre-requisite for effective and informative analysis of the
differences between the model sets.
Click the
Mandibular model registration button in the Model Set Compare tab to
open the Registration form. The rest of the registration process is similar to the one described
in Maxillary Model Registration chapter.

6.3.4.4 Difference Map for Two Model Sets

The Difference map for two
model sets compares shape
differences between the reference
and the examined model sets
visualized with the help of the
color scale. The distance between
the surfaces of the two model sets
is measured yielding positive and
negative deviation values.
Click the
Difference map
for two model sets button in the
Model Set Compare tab. The
system will automatically mark the
virtual casts according to the Color
scale displayed on the right of the
session window
The areas of the model set rising
above the reference model are
marked red gradually transiting to
green, as the distance between
the models gets smaller, and then
continues to the colder color
palette, when the surfaces of the
compared model sets start to
intersect.

218

6.4 Saving

7 Bracket Placement Module

The dedicated Bracket Placement module of the Ortho Planner allows you to accurately position brackets
used for straight wire technique on the patients malocclusion in order to optimize the treatment efficiency
and reduce bonding chair time. The brackets positions can then be transferred to the Appliance designer
where the transfer media can easily be designed with the dedicated Bracket transfer module. Please refer to
the Bracket Transfer Master Model chapter for more details about the bracket transfer design process.
Watch also video:
How to use the Bracket Placement workflow
Note: The Bracket Placement module is an add-on to Ortho Planner and requires a
specific license activation.
Go through the workflow to place the brackets:
Note: Make sure that the bracket libraries have been downloaded via the Download
Center in Ortho Control Panel -> Tools before you can use this feature.

1. The Bracket Placement module gives opportunity to work on one side of detention
only. Select the jaws you will be working with (Maxilla, Mandible or Both)
2. Perform Segmentation for Maxilla and Mandible.
Hint: The position of the mesial and distal points step allows you to define the teeth
long axes which will in turn be used to define the optimal position of the bracket
(knows as FA points - see section below). Using DICOM Cone Beam scans overlay and
adjusting their transparency helps to position the tooth long axes in the most
accurate way according to the root situation.
Go along the arch and indicate the correct tooth axes.

223

3. Setup planes
3.1 Occlusion plane. At this stage you indicate the planes you will be using as a reference for
the brackets placement process.
Important: Using a fixed reference plane (e.g. Occlusal plane) allows to create an
absolute reference for teeth positions and therefore an expression of the patients
malocclusion, which can also be used in the treatment planning process.
3.2 Standard planes. This step allows you to define additional standard planes, which could be
used as alternative reference for the bracket placement process.
4. Perform bracket placement for Maxilla and
Mandible
a. Adjusting the FA points.
The default position of FA points is defined at the intersection of the projected long axis on the
vestibular side of the tooth and the mid-transverse plane of the crown. The midtransverse
plane of the crown is calculated from the top of the tooth at the gingival contour to the tip of
the tooth.
The FA points turn white when you hover the mouse cursor over them.

224

In order to position the FA points use the tools in the left

side of the screen.
Choose in what fashion to move the FA points along the
tooth by marking the corresponding radio button.
You can undo/redo the action by clicking on

In order to reset the positioning of the FA point to its

default position use the
button (1). To lock (2) and
unlock (3) the positioning of the FA point, use the
corresponding buttons.
Show plane and measurements checkbox enables the
display of the chosen reference plane in the views
section at the bottom of the screen (in green) and shows
the measurements of the FA points.
It is possible to choose the plane for reference for all
calculations from the drop-down menu. The occlusion
plane defined at step 3.1 is used as default reference
plane.
In case the occlusal plane doesn't intersect a cusp tip you can use the landmarks instead of the
reference plane to define the FA points.

225

The chosen reference plane will define the tooth previews offered at the bottom of the screen.
Choosing the occlusion plane will typically return the angle position of the tooth (tip and
torque), and therefore an expression of the patients malocclusion in absolute terms.
The tooth edge plane option will offset the reference plane at the tooth tip and show a
perpendicular view from the tooth long axis to return the height reference for the bracket
placement.
Finally, the use of the additional landmarks allows to apply the inter-cusp buccal segment as
reference for height calculations for molars instead of the occlusal edge.

Occlusal plane as a reference

Tooth edge plane view

When you finish with positioning the FA points, lock all the points by clicking at
. All the
FA points turn green, which means that brackets will be able to be positioned on them. Then
you can move to the next step.
b. Placing the brackets
Select the brackets library name from the corresponding drop-down menu.

Note: You can select different libraries for the upper and lower jaws.

226

It is possible to choose the Reference plane as well as the

movement option for the bracket placement, similarly to the FA
points positioning.
Movement distance - you can move the brackets with the
mouse using the red control points.

Collision detection tools ensure that the brackets geometry is

in maximum contact with the tooth surface, but never
intersects with it.
Disable collision detection to antagonist - allows you to
position brackets by disregarding the constraints from the
antagonist. That is it disables the function of collision detection
of the brackets to the antagonist jaw, but applies to the
considered tooth surface.
Hint: This option allows you to optimize the
bracket position on the lower jaw in spite of
occlusal interferences with the upper jaw. Using
the virtual articulator then allows you to open the
bite of the patient to avoid these occlusal
interferences.
Show collisions - will highlight collisions between the
brackets and the underlying, neighboring tooth or gingiva in
red.
Disable auto-snap - allows you to correct the position of the
bracket manually in all dimensions using control points.
Hint: Auto-snap is the function that automatically
ensures maximum contact of the bracket on the
tooth surface. If it doesnt perform properly
initially, disabling this function while manually
positioning the bracket and then enabling it again
can usually solve the issue.

227

The Positioning table lists the tooth specific

positioning values:
The Prescribed column indicate the
manufacturers prescription for the chosen
bracket.
The FA position column shows the values
calculated from the FA point position. This
expresses the malocclusion of the patient using
the absolute reference plane
The Current column shows the actual values
returned from the bracket placement. The
different values for each column are defined
below:
Tip (also referred to as Angulation) - the
angulation of the long axis of tooth (or bracket)
to the reference plane in the mesial distal
direction.
Torque - with respect to the straight wire or
edgewise appliance, torque is the force system
created when a rectangular arch wire is
engaged in a bracket slot in torsion, and which
has the effect of changing the inclination of the
tooth in the in the buccolingual or faciolingual
direction.
In - Distance from the inner side of the bracket
slot to the tooth surface at the entrance of the
wire (mesial side).
Out - Distance from the outer side of bracket to
the tooth surface at the exit of the wire (distal
side).
Dis. incisal edge -Distance to the incisal edge
from the FA point or bracket base point.
Dis. ging. contour - Distance from the gingival
contour to the FA point or bracket base point.
Bracket point deviation - Distance from the
defined and locked FA point to the base point at
the center of the bracket in its actual position.
This is the reference of the positioning of the
bracket.
Locking brackets: once each bracket position has been reviewed, you can lock it individually,
or using the lock all option. The control points of the locked bracket(s) will turn green; this
indicates that they will be usable for bracket transfer once the project is validated.

228

Hint: Use the model preview buttons and visibility sliders to control your bracket
placement from the occlusal side and by removing the gingival model.

4. Validate
At this stage you check all the settings for the brackets and validate their positioning with the
validation tools so the bracket transfers can be designed in the Bracket Transfer module of
Appliance Designer Validation allows for a reliable exchange and review process of indirect
bonding cases between locations (e.g. a clinic and a lab). Reviewing a validated case without
making any changes will keep the validation. Making any change a validated indirect bonding
project will require a new validation to be done for the project to be processed further.

229

Click at the Validate button and Done at the bottom of the screen to finish the procedure.
It is possible to validate the brackets with the help of the 2D cross-section option. Control the
position of the brackets on the teeth as well as in relation to the antagonist.

230

Click Show prescription table to view all the bracket placement prescriptions. This information
can be added to the report. Note that the tip (FA) and torque (FA) fields give you an indication
of the patients malocclusion in absolute terms with reference to the chosen reference plane
(typically occlusion plane).

Note: Validating the model set will save with the brackets positioned in the way you
did in the Ortho Analyzer ready for the transfer to the Appliance Designer.

8.1 Appliance Designer Glossary

Warning: It is the responsibility of the Appliance manufacturer to ensure that the
design of the 3D appliance is suitable for Orthodontic purposes.

Part: a part is an individual element of an appliance; it consists of components and

features and will typically be manufactured individually. For example, one appliance can
be made of two separate parts: one for the upper jaw and one for the lower which will
be mounted together.
Component: a component is a building block for the appliance.

The main component types are:

Shell: wrap surface

with a fixed offset
distance.

Bar: a 3D model based

on a 2D profile, allows to
apply predefined
parameters and
geometries.

Combine models: adding or subtracting 2 models geometries

Remove undercuts: fills undercuts to simulate e.g. waxing on models
Load model: load any STL to use it as visual guide, or combine it with the appliance.
Model Hollowing: allows to hollow a 3D model in order to save material for rapid prototyping.
Create Bracket transfer master model: allows you to create a master model for either
thermoforming or continuous digital printing of directly printed trays. With this feature it is
possible to design brackets for printing.
Create Post Processing: this special step allows to ensure that some selected features (see
below) are being applied to a specific model in a modeling template
Features: indicate the specific design parameters applied to a component.
233

Additional features, which can be applied to any active 3D model (i.e. not just to components)
are:

Additional Features:

Modify model: a sculpting toolkit to add, remove material, or smooth the models surface.
Attachments: allows to place an STL object from the library to the active 3D model.
ID tags: places a unique ID tag on the active 3d model to distinguish it.
Cut model: allows you to make a cut on your designed model.
Drill Compensation: ensures that the drill used in CNC milling manufacturing process is able
to access the sharpest areas of the designed part. It needs to be adjusted with relation to the
actual drilling tool which will be applied.
Workflows: sequence of components and features used to create an appliance or part.
Template: a template allows to reapply a given workflow to another patient model set. A
detailed modeling template is easy to create using the components and features presented
above.

234

Warning: Please assure that the required material thickness and fitting properties
are applied, when creating the appliance model, according to the specific material
and manufacturing machine requirements you will use.

2D spline on models
This option allows you to control the height of the modeled bar by positioning on a 2D plane,
typically an occlusion plane.
To place the central spline in 2D mode
select the plane from the Plane dropdown menu (you have to create the
desired planes before this. For more
information see the Additional Plane
Views section).
Adjust the plane position in the Offset
edit box or use the dedicated control
points to rotate the tilt of the 2D plane.
Snap to grid aligns each control point
to the nearest grid line.
Symmetric spline allows you to place
control points of the spline
symmetrically. If a sagittal plane has
been defined, this will be used a
reference for symmetry. If there is no
sagittal plane, the mid-line of the grid is
used as a reference for symmetry.
Place the spline of the plane and click
Next. To redo the spline click Clear
spline. Otherwise, right-click the spline
to edit it.

247

Note: It is possible to Undo/Redo the last action on any stage of a bar creation by

8.3.1.6 Bracket Transfer Master Model

The Bracket transfer master model option allows you

to create a master model for either thermoforming
or continuous digital printing of directly printed
trays. With this feature it is possible to design
brackets for printing.
Watch also video:
How to create a bracket transfer media in Appliance
Designer
Follow this procedure:

After you have selected this option the New component properties form appears:

261

Choose the source model and click OK

It is possible to create three types of Master model: with markers and containers.

Master model with markers

Markers are small hedges or fences on the tooth surface, which indicate where the real physical
bracket should be placed after the master model has been printed. This method still require
you to protect the delicate features of the brackets with silicone before thermoforming, but
doesnt have the same accuracy requirements for the printer as the container method.
1. Indicate the parameters for the markers:
Inner surface offset - indicates the parameters of the offset
from the inner surface. The values can vary due to the purpose
of extra or less friction. The values can be set in the measure
from -0.1 to 0.1 mm.
Wall width - indicates the wall width of the markers. The values
can be set in the measure from 0.5 to 1mm.
Wall height - indicates the height of the wall of the markers.
The values can be set in the measure from 0.5 to 1mm.

262

Master model with markers

Master model with containers

Containers are a bracket representation that protects the delicate and overhanging features on
the brackets. That means you dont have to protect them with silicone drops before vacuum
pressing.
1. Adjust the settings for the brackets containers by indicating
the following parameters:
Inner surface offset - indicates the offset of the size of the
containers. The choice for the higher or lower parameters
depends on your desire to increase or avoid friction. The values
can be set in the measure from -0,1to 0,1 mm.
Block-out angle - indicates the difference between the walls of
a tooth and the walls of the brackets. Allows you to
accommodate friction issues. The values can be set in the
measure from -20 to 20 degrees
Retention amount - indicates the amount of material removed
perpendicular to the insertion direction of the bracket onto the
tooth. This creates a gripping effect from either the
thermoformed or directly printed tray created on the basis of the
master model. The values can be set in the measure from - 5 to
5 mm.
Prolongation length - allows you to extend the base of the
bracket into the tooth. This removes the cavities between the
bracket base representation and the tooth surface, which can
263

create problems during printing of the master model. The values

can be set in the measure from 0 to 10mm.

Master model with containers

Click Done to create a master model with markers/containers.

2. Build a Shell on the Master model with containers by right-clicking the model and selecting
Create "Shell".
If you use a printer with short-term biocompatible materials, it is possible to design and print a
transfer tray on the basis of the master model with containers and hence skip the
thermoforming.
Indicate the insertion direction and position the spline:

9.6.4 Questionnaires
The Questionnaires are used for a better presentation of the customized
analysis and the implementation of the scoring systems.
The Questionnaire is a powerful feature that allows you to present your customized analyses in
a user-friendly manner by arranging them in a logical sequence of Steps.
Each step can contain the following items:

Imported pictures to illustrate e.g. a measurement

The written instructions
A question with multiple choices, each answer having its own individual score
Analysis objects (including any custom analysis object available)
A screen capture button
Notes to be filled by the user

The default model and state of the patient model can also be preset for each step.
Steps can be organized freely into Groups.
Finally, it is possible to enter weights and scores for the individual steps and groups, thereby
making it easy to compute indexes e.g. the PAR Index.
The Edit Primitives-Questionnaire window allows you to customize the already existing
questionnaire or create a new one. In this section, we go through the settings of the PAR Index
scoring system included as an example.

Warnings and Precautions

Important Information and Legal Disclaimer
Furthermore, the software does not contain any functionality attempting to check whether any
such real-life implementation attempt might result in damage, e.g. to the patients teeth or
jaw. Any treatment attempt to implement on a patient a tooth movement or other manipulation
visualized by way of the software should be planned and made only by an orthodontic
professional having with due diligence assessed whether the visualized manipulation can
actually be implemented without causing damage to the patient.
The risks pertaining to any such treatment attempt shall be assumed entirely by the person
carrying out the treatment.
3Shape A/S disclaims any and all liability for the results of any treatment attempts made on
the basis of the visualizations generated within the Ortho Analyzer software.
Importing third party models
Warning: Please assure that any scan models imported into the system are
representing the specific patients dentition in adequate quality.
Scanning
Caution: Beware that the scanner is a precision mechanical device sensitive to
temperature and power variations as well as vibrations.
Positioning the model in the scanner
Warning: The front teeth should always face the inside of the scanner. There should
be approximately 1cm of blue clay underneath the model, holding it firmly in place.
Preparing impressions before scanning
356

Warning: Since 3Shapes R-series lab scanners use optical technology (i.e. laser
light or LED source), the impressions surface must not be too wet/reflective. It is
therefore recommended to wipe excess liquid from the alginate impressions surface
using a tissue, or to use a spray on a dry surface in the case of too shiny silicone
materials. A spray can also be applied to the material used for the bite registration
(wax or silicone).
ScanSuite
Caution! We strongly recommend that both the scanner power supply and scanserver PC power cable are plugged into a UPS unit (rather than directly into the
mains outlet) to prevent work losses and damages to the equipment.
Occlusion control
Warning: Please assure that the occlusion is correct after scanning by using the
miscellaneous available occlusion-adjustment tools.

Warning: If the models are intersecting, after clicking the OK button you will get the warning.

Creating the patient and model set

Warning: Please assure that the physical plaster- or impression models matches the
patient information when scanning the models and creating the record in the system
2D and 3D measurements
Warning: Please ensure to position measurement points on the 3D model and on the
2D cross section carefully by optimizing zooming and viewing positions.

Warning: Please ensure to position analysis points on the 3D model correctly

according to the analysis protocol by optimizing zooming and viewing positions.
Segmentation
Warning: Please make sure to validate separation boundaries and tooth axes
sufficiently when segmenting teeth by using the visual inspection tools available,
notably the overlay of DICOM or X-ray 2D images.

357

Warning: Please ensure that the quality of the Cone Beam DICOM data is sufficient
and correctly imported to allow for proper alignment with the surface scan.
Collision detection
Warning: Please use the available tools for collision detection and distance maps
during the virtual setup process.
Design properties
Warning: Please assure that the required material thickness and fitting properties
are applied when creating the appliance model - according to the specific material
and manufacturing machine requirements you will use.

Warning: It is the responsibility of the appliance manufacturer to ensure that the

equipment and materials used are suitable for producing appliances for Orthodontic
purposes.
Generic Design Guidelines
Warning: It is the responsibility of the Appliance manufacturer to ensure that the
design of the 3D appliance is suitable for Orthodontic purposes.
