Several key project deliverables for 2015 include:
Development and piloting of an electronic triage and referral tool (eSRF) to
improve access to services
Piloting a Cooperative Staffing service model for individuals and families who ar
e served by more than one County program
Piloting a welcome center at Northern Service Center to improve access to se
rvices and overall customer service
Improving internal communication about service integration and EDO

Identifying outcomes and measures to determine the success of pilots and integrati
on efforts
In 2016, areas of development are likely to include:
Expanded e-SRF functionality and deployment of Feedback loop established
o Client Index connection
o Establish requirements for community resource integration (e.g., MNHelp.info)
Assess referral sources for fit with technical and business requirements Ensure
accuracy of referral information and develop a process to maintain a resource
database
o Participate and contribute resources to state systems modernization triage and

milestones and demonstrate progress toward integrated service delivery vision

Portfolio Program Leader Duties
The Portfolio Program Leader will work on the project from the date of hire
through 2016 or as funds are made available. It is anticipated that the work will be
more time intensive the first few months of the project.
Planning/Control
1. Develop an overall schedule for initiative phase completion that effectively all
ocates the available resources to fulfill deliverables
2. Support work groups in developing detailed work plans, which identify and se
quence the activities needed to successfully complete deliverables
3. Coordinate multiple work streams/interdependencies to maintain critical path
work/prevent overlap and ensure alignment with goals/outcomes
4. Identify, track, manage and mitigate risk; escalate these issues when necessa
ry to ensure minimal impact to quality, budget, and timeline
5. Where project control is in jeopardy, create contingency plans with appropriat
e input from
management and key stakeholders to revise project schedule, scope or budg
et in a timely manner
6. Ensure deliverables meet expectations with respect to quality, budget, delive
ry timelines, and alignment with overall initiative strategy
7. Communicate the overall schedule to management and stakeholders that will
be affected by the project activities; revise the schedule as required
Implementation
1. Maintain an understanding of work groups plans, status, issues and risk mitig
ation strategies
2. Use existing forms and records to document initiative activities; recommend
alternatives if applicable
3. Monitor the progress of the initiative and make adjustments as necessary to e
nsure the succesful completion of project deliverables

Staffing Qualifications
1. Bachelors Degree; Masters degree preferred with 710 years of exp
erience
2. Preferred experience working with health and human services,
education and non-profits
3. Understanding of organizational development, complex systems ana
lysis, group
facilitation and information systems requirements analysis
4. Understanding of the relationship between process design and infor
mation capture/retrieval
5. Proven process improvement or reengineering experience; as well as
experience with change management and large group dynamics
6. High level of consultative skills to work with a diverse team of huma
n services/corrections/public health/IT professionals
7. Proven leadership skills and ability to engage stakeholders with a go
al of gaining consensus
8. Strong organizational/business acumen to influence appropriate deci
sions for diverse and undefined issues
9. Excellent oral, written and interpersonal communication skills

Comfortable working with a multicultural perspective and enga
ging work group
participants and stakeholders in ensuring culturally responsive prod

Payment Source Dakota County Community Services


Rates
Contract Not To Exceed Amount: The Fiscal Agent will pay an amount not to exceed
$100,000 for contract term. Travel: Professional time for travel will not be
reimbursed separately. Billing 1. Contractor must provide monthly invoices to
designated Community Services Administration staff. 2. Contractor must comply
with the Countys billing procedures. 3. The County will impose a 10% penalty for
invoices received 30 days after the date that invoiced services were performed.
The County will impose a 20% penalty for invoices received 60 days after the date
that invoiced services were performed. The County will refuse payment on invoices
received 90 days after the date that invoiced services were performed. All bills for a
calendar year must be submitted to Dakota County by February 15 of the following
calendar year in order to receive reimbursement.
4. Dakota County will pay for the actual costs of providing interpreter services to
non-English speaking participants who are an open Dakota County case. The
Contractor must receive prior written authorization of interpreter services costs
form County staff prior to using those services. Unless specifically prior authorized
by the County, the Contractor must access interpreters from those agencies under
contract with the County to provide interpreter service.
