Are you in the Line of Fire?

10 key points to be aware of to reduce the risk

Line of Fire: The zone within a work area where there is risk of injury from
machinery and equipment

1
2

Tensioned
lines and
spring
loaded
devices

Working
at height
and
dropped
objects

Have all snap back zones been identified and controlled?

Be aware and stay clear of coiled or spring loaded devices, strapping/banding and lines under tension
Keep your hands and body clear of rigging equipment such as cables, chains, lifting slings and shackles etc
Always consider the potential for pipe movement when cutting or unbolting
Springs can release tremendous energy if compression or tension is suddenly released

When working at height, always ensure that the correct personal protective equipment (PPE) is used
Tools and portable equipment used at height shall be adequately secured to either the user or workplace and stored safely
Ensure areas below work at height activities have been adequately cordoned off, with the correct barriers in place, taking into
consideration the dropped object cone of exposure

Never exceed safe working load (SWL) during lifting operations

Lifting
operations

Pressurised
equipment

Electrical
equipment

Never walk under a suspended load

Identify all entrapment and crush areas
Be aware of sudden load movement and loads swinging
Ensure effective barriers are in place to prevent unauthorised access

Always verify zero energy before breaking containment

Even after verification of zero energy, keep your body out of the line of fire
Ensure all compressed gas cylinders are secured upright with the correct caps fitted
Be aware of the potential danger when working with pressurised tools

Ensure sufficient isolations are in place when working on electrical equipment

Avoid working near energised electrical equipment
Always stand aside and out of danger when switching a breaker into position

Objects
with roll
or fall
potential

Secure all loads that have the potential to roll before lifting, loading, unloading or transporting
Pay close attention when moving loads with all forms of transport
What will you do if the load shifts? Stay out of harms way!
Secure objects that can roll such as tools, cylinders and poles

Projectiles

Be aware of tasks that generate flying debris such as grinding, chipping, abrasive blasting and hydro-blasting
Before starting these tasks, consider flying debris then select the correct personal protective equipment (PPE)
As the operator, it is your responsibility to know where the debris is going and that no bystanders are in harms way
Ensure that sufficient barriers are in place to contain projectiles

When practicable, position your tools to direct the projectiles in a safe direction

Machinery
and tools

Understand that many machines and processes start automatically without regard for anyone who happens to be in the way
Be aware that equipment can rotate unexpectedly or continue to coast when de-energised
Regularly inspect safety devices designed for your tools and equipment
Always pay attention when operating machinery to avoid trapping your hands and fingers

Moving
vehicles
and heavy
equipment

Be aware of tools slipping. Use correct tool for the task

Never operate equipment with missing guards

If a vehicle operator doesnt have a clear view when manoeuvring equipment, ensure that there is a banksman or flagsman present
When working near moving, heavy equipment, remain in the operators full view. If you cant see the operator, they wont be able to see you

10

Are there adequate site controls in place where there are moving vehicles and equipment?

Pushing and
pulling

When working on or around equipment that is remote controlled or could potentially start up, always lock and tag it out

When pulling or pushing with a spanner, remain focused and anticipate the possibility of the fastener suddenly loosening or spanner slipping

Be aware of your body position

when pulling/pushing, such as a
trolley cart or spanner

Never pull equipment or tools towards your face or body. Position yourself so the tool or equipment would not hit you if it slips
Always cut away from your body
