properly by applying Loco hand brake and placing 4 wedges
under wheels of the loco.
2. Whether entry is made regarding securing of loco in loco log Yes/No
book and also in the loco stabling register at Station/lobby.
3. Whether speed guns are available in the lobby for conducting Yes/No
ambush check of over-speeding by LIs.
4. Whether LIs are supplied with portable breathalyzer equipment Yes/No
for conducting ambush checks.
5. Whether LIs have conducted ambush check on any LP/ALP with Yes/No
portable breathalyzer in last one month.
6. Whether Ambush checks are being conducted on mobile phones Yes/No
carried by LPs/ALPs on duty, to check whether the mobile
phones are in switched Off condition and a record of such
checks are maintained.
7. Whether LI has found & counseled any LP using mobile phone Yes/No
on run or during rest hours in Running rooms. Any record of
check and course is maintained.
8. Whether Loco Inspectors are monitoring their nominated Loco Yes/No
Pilots as per the periodicity i.e. C grade once in a month, B
grade once in two months, A grade once in three months.
9. Whether any Loco Pilots are overdue for monitoring by their Yes/No
nominated LI. Entry in LIs diary to be cross checked.
10. Whether LIs are mentioning the weak areas of Loco Pilots, their Yes/No
Bio-data, Family profile, Health condition etc in their diary.
11. Whether LIs are monitoring their nominated LPs in freight trains Yes/No
from outstation to home station.
12. Whether each Loco Inspectors diary is checked and signed by Yes/No
power officer every month, in detail.
13. Whether LIs are checking that his LPs do not leave their HQ Yes/No
during Rest. Any record of check and counseling is maintianed.
14. Whether Loco Inspectors have done any ambush check in a Yes/No
month for observance of speed restriction of 40/30 Kmph by
LPs for Coaching/Freight trains while passing signal at caution.
15. Whether LIs have done any ambush check in a month, to check Yes/No
LPs of loaded goods trains are stopping the train at 500 Mts
before Stop signal at danger in gradient 1/150 and steeper.
16. Whether Crew are booked next day morning at 8.00 hrs after Yes/No
availing Leave.
17. Whether LI is accompanying LP in the first trip, after absence Yes/No
from duty for more than 15 days due to Leave/Sick/Absent
18. Whether feedback of Signal defects and tracks defects Yes/No
mentioned in the register are given to lobby within 24 hours, by
Engineering and S&T department.
19. Whether calibration certificate of BA test equipment is available Yes/No
in lobby.
20. Whether BA test equipment (working/spare) is tested every day Yes/No
in the morning at 8.00 hrs and whether record of the same is
available.
21. Whether BA test equipment is tested as per the procedure Yes/No
prescribed.
22. Whether details of staff found positive while conducting BA test Yes/No
is available in the lobby with action taken on such cases.
23. Whether LP is having all required tools and manuals (with Yes/No
amendment slip) in his line box.
24. Whether the checking of tools is being done once in three Yes/No
months and same entered in LPs competency certificate by
Chief Crew controller.
25. Whether Crew are being given relief after completion of 12 Yes/No
hours of duty. Bad cases and trend brought to notice of Power
Officer by CCC.
26. Whether all Crew are being brought back (Crew turnaround) to Yes/No
HQ within 60 hours. Bad cases analysis to be done.
27. Whether there are any cases of crew turnaround more than 72 Yes/No

28. Whether SPM data check is being done 100% for coaching train Yes/No
and 25% for Freight trains as per ACTM Vol-III Para 30508.
29. Whether LPs/ALPs of coaching trains are coming to duty/lobby Yes/No

31. Whether any LP/ALP is booked overdue for PME. Yes/No

40. Whether LI has repeatedly counseled his LPs, to strictly follow Yes/No
signals and only signals, and not to get guided by any other
observation or presumption.
41. Whether SOBs are issued and acknowledged by all LPs/ALPs Yes/No
regarding new /shifted signals
42. Whether LIs have monitored his LPs in sections other than his Yes/No
Division/Railway, if applicable
