DUTIES AND RESPONSIBILITIES:


Circulating Nurse
• Knows the operations to be performed.
• In- charge of the operating room assigned.
• Receives endorsements from ward nurse regarding patients' pre-operative medications, consent
for the procedure to be performed, surgical checklist, and other concerns required for the
procedures.
• Greets and identifies the patient, and oversees for the appropriate attire before being brought to the
operating room
• Orients the patient of the OR physical set-ups and certain procedures.
• Assists as patient is transferred to the OR table.
• Applies safety straps to secure the patient's arms at the side of the table and gives adequate and
modest drapes to the patient.
• Attaches the patient to the necessary equipments such as cardiac monitor, BP apparatus,pulse
oximeter.
• Checks patient's vital signs and watches for any unusualities.
• Checks the patency of the IV line/s , fluid level, and regulates it to the prescribed rate/s.
• Stays with the patient and establishes a relaxed and calm environment and attends to the special
needs of the patient before induction is started.
• Assists the anesthesiologist in induction of the anesthesia, takes note of the time of the induction
and the amount of drugs being given whether it is general, spinal, epidural, or spinal-epidural.
• Assists during intubation while takes note of the level of the endotracheal tube, listens heart sounds
for equality, places airway at mouth, and secures tube in place using adhesives tapes.
• Fastens the back of the OR gowns of the scrub nurse and the surgeons.
• Performs surgical skin preparations at surgical site and its surrounding areas accordingly.
• Inserts Foley bag catheter with Closed Drainage Unit whenever the patient undergoes either spinal,
epidural, spinal-epidural or for longer duration type of surgeries.
• Makes initial counting of sponges, sharps, instruments and any items at the sterile field with the
scrub nurse aloud as the scrub nurse touches each items.
• Records each item (count) of each type of item at a designated forms.
• Informs the surgeon, anesthesiologist, and all members of the surgical team of the results of the
count.
• Assists the scrub nurse in anticipating and providing sterile supplies and instruments needed in the
procedure,
• Ensures sterility, appropriate temperature, humidity, lightning safety, and availability of equipments
and supplies.
• Coordinates the surgical team with the personnel of other department such as x-ray and laboratory
of any procedures that might be needing them.
• Keep watches over the aseptic technique before, during, and after the procedure.
• Obtains blood for transfusion and makes sure to check the blood type, compatibility, bag number,
serial number, expiry date, and watches for any reactions during the transfusion.
• Keeps an eye at all instruments and materials being used during the operation and be accountable
for them.
• Makes sure all specimen for studies are appropriately labeled and promptly send to the laboratory
and all request forms are made accordingly.
• Makes sure all necessary papers are properly signed by the surgeon and anesthesiologist for
proper and legal documentation.
• Documents the patient's pre-operative and intra-operative vital signs, names and amounts of drugs
administered by the anesthesiologist or the surgeons (for local anesthesia), and records all
pertinent data.
• Transports the patient to the recovery room and does an endorsement to the RR nurse.
• And in some instances, the circulating nurse does the post-operative vital signs monitoring in the
recovery room every 15 minutes for two hours or as needed by the patient.

DUTIES AND RESPONSIBILITIES:


Scrub Nurse
• Gathers all equipments for the procedure and prepares all the supplies and instruments using a
sterile technique.
• Responsible for establishing and maintaining the integrity, safety, and efficiency of the sterile field
throughout the entire surgical procedure.
• Does aseptic technique according to the accepted practice.
• Does proper gowning and gloving following the basic aseptic technique.
• Adequately drapes the tables and areas necessary for the procedure.
• Meticulous in accounting all instruments and materials before, during, and after the operation
together with the circulating nurse.
• Serves gowns and gloves to the sterile surgical team.
• Assists the surgeons in draping the patient accordingly and aseptically.
• Anticipates the needs of the surgeon systematically, serving instruments properly, and have
ligatures ready as if the scrub nurse knows the procedure just like the surgeon.
• Maintains sterility at all times.
• Assists in after care of the patient after the procedure.
• Returns all unopened instruments at each designated cabinet while consciously rechecks its
sterility's expiration date.
• Cleans, decontaminates, disinfects, packs, put all labels to each items, and sterilizes all
instruments being used.
• Cleans, decontaminates, and prepares the OR theater for the next case.

DUTIES AND RESPONSIBILITIES:


Recovery Room/ Post Anesthesia Care Unit Nurse
• Total management of immediate post-operative patients.
• Receives endorsements from the circulating nurse regarding the operation and present status of
the patient.
• Attaches the patient to the monitoring apparatus, O2 machine, and ensures patency of all IV lines
and arterial lines.
• Does vital signs monitoring, pain and neurological assessments every 15 minutes and documents
accordingly.
• Assesses fluid intake and output hourly.
• Carries out all doctors' order conscientiously.
• Administers prescribed post-operative medications by rechecking doctors' order properly.
• Provides comfort and safe environment.
• Does a necessary nursing procedures such as suctioning, positioning, chest-tapping, and
reinforcing health teachings to the patient and to the patient's direct relatives when patient cannot
comprehends so.
• Updates the surgeon and anesthesiologist of any untoward event post-operatively.
• Attends cleverly all patients' needs.
• Informs the resident on duty or resident in-charge once patient is due for evaluation.
OTHER DUTIES AND RESPONSIBILITIES OF AN O.R. NURSE:
• Be accountable of all OR items.
• Be responsible in cleaning and packing instruments , and some supplies for sterilization.
• Takes call for reservations and bookings of OR cases according to the number of staff nurses and
availability of Sterile Equipments needed and OR rooms.
• Does dump dusting and environment care of the whole OR theater.
• Checks oxygen and nitrous level, and other supplies every after a surgery.
• Does weekly requisition of OR items from the hospital's property custodian.
