Report of Accident on Site (Date)

We regret to inform you of an accident which occurred on the project at (time) on

(date). The incident was investigated by our Safety Officer, a copy of whose
report is attached for your information.

(Optional) The relevant authorities have been informed.

Yours etc.

Note: The Safety Officers report should contain recommendations for

remedial action including further training etc.
