INTRODUCTION

Glaucoma or Increased Intraocular Pressure (IOP) is the result of inadequate drainage


of aqueous humor from the anterior chamber of the eye. It is a condition that causes
damage to your eyes optic nerve and gets worse over time. The increased pressure
causes atrophy of the optic nerve and if untreated, blindness. Glaucoma tends to be
inherited and may not show up until later in life.

There two types of glaucoma first is the open-angle and the second one is the closed
angle( or narrow angle). Chronic open-angle is the most common type, accounting for
90% of all glaucoma cases. It develops slowly, may be associated with diabetes and
myopia and may develop in both eyes simultaneously.

Narrow-angle glaucoma is the less common form and may be associated with eye
trauma, various inflammatory processes and papillary dilation after the instillation of
mydriatic drops. Acute angle-closure glaucoma is manifested by sudden excruciating
pain in or around the eyes, blurred vision and ocular redness.This condition constitute a
medical emergency because blindness may suddenly ensure.

PRESENT HEALTH HISTORY

Patient M.A.P was admitted with a diagnosis of Hyper mature Cataract with Narrow
Angle (Glaucoma OD). Prior to admission, the patient said that he is having a viual
disturbance like he was seeing a white blockage on his vision when he is reading on
something since august 2016. He ignored it until his vision became more blurry so he
decided to seek medical attention. The patient came here in the city to have checkup of
his eyes and then he was scheduled for trabeculectomy.

PAST HEALTH HISTORY

Based on the patients verbalization, he was hospitalized in 1964 to done


appendectomy and he was admitted again on year 1998 because of an particular
accident( Politics matter).The patient said that he was fully immunized, has a history of
hypertension, no allergies on food and no other childhood illnesses.

FAMILY HISTORY

Patient M.A.P is the only child of his parents, both deceased. He had a history of
hypertension that he got from his late mother. His wife is a retired midwife also from
cotabato no particular disease and they had 4 children, 3 females and one male, luckily
no history of any diseases.
