Medina Care Plan: Improve the Quality of Life Among the People of Medina

Healthy People 2020 Goal

A Healthy People 2020 goal is to improve health-related quality of life and well-being for

all individuals. According to Healthy People 2020, the health-related quality of life is a multi-

dimensional concept that includes domains related to physical, mental, emotional, and social

functioning. It goes beyond direct measures of population health, life expectancy, and causes of

death, and focuses on the impact health status has on quality of life (Office of Disease

Prevention and Health Promotion, 2016).

Short Term Goal

Within the Medina community there will be an educational program formed for all

members of the community. This program will detail important information regarding

chronic health diseases and what steps the people of Medina, Ohio can take in order to

prevent these diseases, and improve the quality of life.

Long Term Goal

Throughout the Medina community this educational program can be implemented into

the schools in the Medina area, and the Medina Recreation Center. With the

implementation of this program, evidence will a decrease in chronic health disease in the

Medina area and an increase in the overall quality of life.

Outcome Criteria

Within three months, an educational program will be organized and ran by local

community members looking more towards medical professionals. Volunteers must have

a medical background, to provide adequate information to the community. These medical

professionals will be at the table to answer any type of questions that the community

member were to have, and present a power point on how to prevent chronic diseases.

This program will first be offered at the Medina Recreation Center three times a week. A

booth or table will be set up to offer this information for community members who are

entering the Recreation Center. This program will be advertised at local businesses, and

gyms in order to spread the information. Within the first three months the attendance rate

is aimed at 5% of the population for the information tables period. The goal of this

educational program is to spread the word about chronic disease and how it can impact

your overall quality of life.


By six months there will be a forty-five-minute presentation put on at the Medina

Performing Art Center. This presentation will be offered twice a month for six months. A

sign in sheet will be placed at the front door in order to obtain some brief demographics

for future use including age, race, sex, do they see a PCP, and do they attend a gym

regularly. This presentation will include a panel of doctors and other medical

professionals. They will present a powerpoint to the community about chronic diseases

and how they affect your overall quality of life. This presentation will also offer advice

on how to improve your overall quality of life. Topics for the presentations will be based

on questions/comments made during the tables in the Media Recreation Center.

Following the presentation, audience members will be able to ask the panel of

professionals questions they may have about the presentation or any other specific

questions they may have. For this presentation the attendance rate is aimed to be around

10% of the population, growing from the 5% attendance rate during the information

tables. The presentation will be adjusted in order to meet the needs of the community

based on the answers from the sign in sheet, and questions from the audience.
After eighteen months of educating the population about the importance of prevention of

chronic disease to improve the overall quality of life within the Medina community will

see an increase in visits to primary health providers. With the information community

members received at both the information tables and the presentation, physician offices

will be contacted regarding an increase or decrease in the number of patients seen for

routine checkups. Along with contacting physicians, local gym owners will be contacted

regarding an increase or decrease in the attendance rate. Overall the numbers should

increase following the presentation.


By twenty-four-months there will be a decrease in chronic health diseases in the Medina

Community. Evidence will show that there is an increase in community members visiting

their primary care providers, and increase in community members attending local gyms.

When asked, community members will be able to educate others as to what leads to

chronic health diseases and how it can lead to their quality of life. The overall goal is to

improve the quality of lives of the Medina community members.

Nursing Diagnosis

Ineffective health maintenance among the people of Medina, Ohio related to the

increased rate of chronic health disease leading to a decreased quality of life.

As Evidence By

As stated by the Centers for Disease Control and Prevention, weight that is higher than

what is considered as a healthy weight for a given height is described as overweight or

obese. Body Mass Index, or BMI, is used as a screening tool for overweight or obesity

(2016).
According to the CDC, chronic diseases are responsible for 7 out of every 10 deaths each

year (2016).
According to The State of Obesity Project, Ohio has an adult obesity rate of 29.8%,

increasing from 20.6% in 2000. Ohio ranks 26th out of the 50 states when it comes to

obesity (2016).

A statement in the Community Health Needs Assessment for Medina County says, the

leading chronic diseases of Medina County are coronary heart disease, stroke, and

hypertension. Having a hypertension rate of 20.6% (2013).


The Community Health Needs Assessment for Medina County states that it did not

meet the standards of Healthy People 2020 of reducing coronary heart disease mortality

rates (2013).

Interventions and Rationales

Intervention: Surveying and speaking with local physician offices and local gyms in order

to obtain an estimate number of community members that see a primary health care

provider and also attend a local gym.


Rationale: In order for this educational program to gather information for the presentation

and table information current data needs to be collected. Data that would be gathered

would include number of patients that are seen regularly, number of patients that

currently have a chronic disease, along with other information. No names of patients

would be given out in the survey, along with the physicians who had taken the survey.

With contacting the local gyms, data can be gathered based on the number of community

members attending gyms, taking that step in order to reduce the risk of chronic diseases.
Intervention: An educational program will be provided to the community members of

Medina, Ohio. This program will be run by medical professionals from the Medina area

and centered in the Medina Recreation Center.


Rationale: Providing information to the community members of Medina can beneficial in

many ways. According to the CDC, half of all Americans have at least one chronic health

condition which could have been prevented if those people would have been educated

about chronic health conditions (2013). Education is the key to help with the prevention

of chronic health conditions. With this educational program, the community will now

have the material and education to take the steps in order to improve their quality of life.

The target of this program is to speak to at least 5% of the Medina population. The

population of Medina is approximately 27,000 (The City of Medina Ohio, 2016).


Intervention: In order for the community members of Medina to fully understand what it

is like to live with a chronic health condition, it is important that they tell stories from

those living with conditions including hypertension, diabetes, coronary heart disease.
Rationale: To fully understand what it is like to live with a chronic health condition and

how it can affect the quality of life. Stories give first-hand experience on what it is like to

live day by day with a chronic disease such as COPD. Complications that can arise from

COPD include being unable to work, limited physical activity, having increased

confusion, being admitted to the hospital for overnight stays, among many other things.

All of these complications not only affect the person with COPD but it also affects the

family members. The family member might need to take off work in order to take the

affect member to the doctors or hospital. The family will now only have one income, and

that can lead to financial problems. People might think that chronic health conditions

only affects the person with the disease, but that is far from the truth.
Intervention: To present the educational program to the Medina City School Board in

order to make this content apart of the curriculum into the health classes within the

school system.
Rationale: The addition of this content into the health classes will not only promote a

healthy lifestyle but also help reduce the risk of chronic health conditions. According to

Healthy People 2020, promoting well-being emphasizes a persons physical, mental, and

social resources and enhances protective factors and conditions that foster health. (Office

of Disease Prevention and Health Promotion,, 2016). A safe and healthy school and home

environment will thus encourage students to reduce the risk of chronic health conditions.

Evaluation

Three-Month Evaluation

Based on surveys that were collected from local physicians and gym owners, information

for the informational table was used. Volunteer medical professionals were able to manage the

informational table three times a week for the first three months of this program. With at least 1

physician at each table session. Based on a count that was collected at the informational tables

the attendance rate reached 4% of the total population of Medina. The goal was to reach 5% of

the total population. A way to help reach more community member of Medina is by adding

additional times to the informational tables and not having a table three days a week for one

hour.

Six-Month Evaluation

Based on data collected during the informational tables, and questions asked from local

community members during the informational tables, a forty-five-minute presentation was

created to be presented to the community. Three physicians from three different medical

practices, and two registered nurses were selected to be a part of the panel of professionals for

questions following the presentation. This presentation was held at the Medina Center of

Preforming Arts, and had a total attendance rate of over 10% of the population. The goal for the

six-month period had an attendance rate of 10%. This presentation was presented twice a month

for a total of six months. Each presentation was different and had a guest speaker who was a

local community member. This community member shared their story on what it is like to live

with a chronic disease and how it does not only affect them, but also their family.

Eighteen Month Evaluation

After eighteen months of educating the people of Medina about the importance of

reducing the risk of chronic conditions will see an increase in primary prevention including

screenings and education. A post survey was sent out to local physicians and gym owners. This

survey is to now see if the educational program that has now been running for nine months has

had an impact on this people of Medina. Following the collection of the survey, data does show

that more people of Medina are seeing their primary care providers to reduce the risk of chronic

health conditions. Along with increasing visits to primary health providers there was an increase

in community members going to a local gym.

Twenty- Four Month Evaluation

At the one year mark following the implantation of the educational program to reduce

chronic health conditions shows that chronic health conditions have decreased in the city of

Medina. With the decrease in chronic health conditions there is an increase in the overall quality

of life. People in Medina are living longer, and living a better quality of life. Everyone should be

educated on the importance of chronic health conditions and how they and their families can

reduce the risk. When local community members are asked what are risks for chronic health

conditions they are able to answer promptly and correctly. The long term goal of the educational

program is to present this educational program to the Medina City School Board in order to take

this information and add it to the curriculum.
