Needs Assessment Form

Name Samantha DiMatteo Date: February 14, 2015

Statement of Purpose: I am investigating the issue with seating during community lunch as it relates to comfort of
students and sanitary eating conditions so that I may better understand why students are eating on the floor and work to
improve the lunch environment.

Task Date Complete

Purpose: Limited availability for seating during community lunch is forcing students to eat while sitting on 02/08/2015
the floors in the hallways.
Stakeholders: Students, teachers, administrators, and janitorial staff. 02/08/2015
Relevant Facts: All 1216 members of the student body eat lunch from 10:35-11:35. The seating in the 02/09/2015
cafeteria only accounts for approximately 350 students; the remaining students either eat on the floor in
the hallways or on the gym bleachers.
Data Collection Method: Stakeholders were given a survey in which they could rate the different 02/11/2015
components of community lunch and offer their own suggestions for improvement.
Data Collection Plan: The survey consisted of questions that were of a check mark format and one 02/11/2015
question that required an open-ended response.
Data Analysis Plan: Youth and adult surveys were tabulated. Results were compared graphically. 02/11/2015

Strengths: The structure of the survey lent itself to a collection of data that was quick and easy to 02/11/2015
evaluate. No one was reluctant to take the survey because participation in only took a few moments.
Weaknesses: Twenty adults and one-hundred students were surveyed (accounting for about a third of the 02/11/2015
population), a survey of the entire population of stakeholders would provide more conclusive data.
Short-term Goals: Open up the auxiliary gym and other areas of the school for additional seating. Allow students 02/12/2015
to leave campus for lunch. Allow students to eat outside (weather permitting). More hand sanitizing stations.
Long-term Goals: Provide more tables and seating for students. 02/12/2015

Action Plan: Request that the school provides additional tables and benches. 02/13/2015

Summary: The majority of staff and students surveyed agreed that there was not enough seating during 02/14/2015
lunch and that they would rather eat lunch at a table opposed to the floor. Ordering more tables, providing
additional sanitation stations, and hiring additional janitorial staff will provide community lunch with a better
environment.
