TEST 1 MATHEMATICS FORM 5 ( CHAPTER 2 4 )

ANSWER ALL THE QUESTIONS.

1. A ball is thrown up, from the balcony of the vertical flat. Its position during its thrown is

presented by the equation y=x 2+5 x +12 where y metres is the height above the sea

level and x metres is its horizontal distance from the flat.

(a) Complete Table in the answer space for the equation y=x 2+5 x +12 by writing down

the
values of y when x = 1 and x = 5. [2 markah/marks]
(b) For this part of question, use the graph paper. By using a scale of 2 cm to 1 unit on the x-axis

and 2 cm to 2 units on the y-axis, draw the graph of y=x 2+5 x +12 for 0 x 6

[4 markah/marks]
(c) By using the graph drawn in (b), find
(i) the value of x that which the ball reaches its greatest height.
(ii) use this value of x in the given equation to calculate the greatest height reached.
