Introduction :
On 30th December 2016, 1 area of leakage was noted on nozzle N3 weld N3W1 (see a drawing N: 806094 ) of
equipment 00-V-051A, anomaly report IA-17-ANOM-003 was raised to capture the external condition, in addition
profile RT was completed (IA-17-RT-001). Following these reports internal visual inspection of Nozzle N3 and other
nozzle of similar design was required to further determine the damage mechanism for this leakage. A Close &
General visual inspection was carried out from the internal of the vessel as detailed below.

It should be noted that due to product residual (NORM levels detected) access was restricted from M2; M4; M7 and
the inspection was carried out from the open man way only (see a drawing N: 806094 item 2). No Access is possible
in M3 and M5 due to small diameter 16.

The following nozzles and welds were accessible (limits of inspection shown on diagrams in following pages):
From M6: Nozzle N3 and Welds G1 & G2 (see a drawing N: 806094 item 3)
From M1: Nozzle N9 and N2 Weld W1 & W2 only (see a drawing N: 806094 item 1)

Results:
Man way M6:
Severe Internal corrosion was noted in the nozzle N3, we note that nozzle has a dissimilar metal (carbon steel to
duplex steel) weld N3W1, corrosion of the Carbon Steel material is found around the full nozzle circumference HAZ
(Heat Affected Zone) of weld and nozzle neck ( a band 60 mm x 360deg down from weld N3W1), the maximum
accessible measurement depth of the corrosion at the HAZ was 6 mm located in the area of the leak, the corrosion
groove adjacent to the weld root measured 2-4mm for the full circumference. The root of weld N3W1 is present with
indications of minor contamination.

The shell from man way M6 to the weld G06 was not cleaned but appear to be in good condition no severe corrosion
was noted.

Pitting was noted on the manway M6 face with maximum depth= 2 mm.

Man way M1:


The neck of nozzle N9 and weld N9W1 (at top shell) showed initiation of corrosion with depths up to 0.5mm.
Corrosion grooving in the HAZ area adjacent to weld N9W1 was present with a depth estimated 0.5-1.0mm (it
should be noted that accurate measurements were possible due to geometry of the Nozzle. The root of weld N9W1
is present and no significant deterioration was observed.

The surface of Nozzle N2 was prepared by wire brushing but some products were still present during the inspection.
The neck nozzle N2 and weld N2W1(at bottom shell) showed slight surface corrosion with wall loss less than
<0.5mm. No depth of Corrosion grooving in the HAZ area adjacent to weld N2W1 could be measured although the
initiation of the grooving was visible The root of weld N2W1 is present and no significant deterioration was
observed.

2 isolated pits were noted near to nozzle N9 see diagram, dimensions are 25mm wide by 12mm long by 4.0mm
deep and 23mm long by 25mm wide and 4.0mm deep

Slight surface corrosion with wall loss less than <0.5mm was noted at: East side and South side of the main wet gas
inlet header, it should be noted that the surface was not cleaned and corrosion product debris were present.

Pitting was noted with maximum depth= 2 mm on the face of man way M1.

M4, M2, and M7: restricted access due to product residual, the inspection was carried out from the opening man
way the general condition of the vessel appear to be in good order, but residual product was present on the bottom
of the headers.
Conclusion/Recommendation:
Further review by Integrity team is required to confirm deterioration mechanisms and repair opinions.
