Important Information about the Anthem Online Provider Directory

This online directory was developed to help Anthem Blue Cross and Blue Shield
members easily find participating physicians, hospitals, and other health care providers.
While we try to provide you with accurate data, the information contained in this directory
may change as participating providers join or leave the network, move, or retire. This
online directory does not constitute a recommendation of any of the listed providers.

Prior to receiving services, you should always verify that the provider you choose is still
participating with Anthem and accepting new patients. In addition, some benefit plans
require that you obtain a referral from your primary care physician before receiving
services from another physician or specialist. In order to receive benefits or to receive the
highest level of benefits - check your schedule of benefits, your group administrative
contract or call the Customer Service number on the back of your ID card to verify your
benefits.
Your participating provider's agreement for providing covered services may include
financial incentives or risk sharing relationships which are based on utilization and quality
of services. If you have questions regarding such incentives or risk sharing relationships,
please contact Anthem Blue Cross and Blue Shield or your provider.
If you receive services from a network hospital, the hospital may arrange with physicians
and other providers to assist in your care. Anthem encourages its network hospitals to
hold those hospital-based providers to Anthem's standards for member billing. However, if
a hospital does not require its hospital-based providers to comply with Anthem's
standards, Anthem cannot influence the manner or amount that you may be billed by the
hospital-based provider.
