A progress report detailing current work to date on the study, significant changes, and
additional administrations needed (reapplications only).
PROPOSAL OUTLINE
Proposals must incorporate all of the sections listed below. Any additional information that
might aid in the review of a particular request should be included. Proposals must not
exceed three pages in length. A separate application form must be submitted for each
independent project/study.
ABSTRACT State the purpose of the study and clearly specify the test

Progress reports are required by researchers who are reapplying for the discount or
requesting an extension of a current discount. Please include your original proposal with an
additional section describing the progress you have made to date, as well as the number of
additional administrations needed.

MHS Inc. retains all rights to our published assessments. Although MHS staff may provide
assistance in the interpretation of the results, we do not share the scoring keys of our
assessments with researchers. For information related to test construction, reliability and
validity, please refer to the technical manual.
MHS does not grant permission under any circumstances for full versions of our
instruments to be included as a part of a dissertation, in the appendices, or included in any
document derived from the use of our instrument in your study or for any other purpose.
Misuse to this end is a violation of MHS copyright. Full versions are available on a
temporary basis for review board approval purposes only under the requirement that they be
destroyed shortly thereafter. You may include up to six items from the assessment for
illustrative purposes with the appropriate permissions. See Part B of the application to
apply for permission.
The instrument(s) used in the project cannot be copied or reproduced, in whole or in part, or
distributed to individuals outside of the designated research team for any reason. The
instrument(s) used in the project cannot be translated, modified, or used to develop another
psychometric assessment tool without the expressed permission of MHS, please contact us
before applying should you need to make this request.
DATA COLLECTION
For researchers who hold the equivalent of a masters degree or higher in psychology, an
additional discount may be available to you if you agree to participate in a data collection
project for one of our unpublished instruments.
MHS Inc. is looking for assistance collecting data for a number of nationally-standardized,
norm-referenced instruments. We are looking for data from professionals who work with
individuals in clinical and non-clinical environments, of varying ethnicities, ages, etc.
Participation in a data collection project can involve a sample other than the one described
in this application. Please indicate your interest to be a data collection site on Part C of the

If you feel that you do qualify, you can request to have your qualifications reviewed. You
will need to provide evidence such as a transcript indicating that you have completed a
required course (e.g., a graduate level psychology course in tests and measurement), and
include an official detailed course description (from the course calendar or university
website). You may be asked to provide additional information.
You may also take the required course at your institution. Postgraduate psychological tests
and measurements courses are available at many institutions, with some offering the
courses online.
What if I need to use the assessment in another language?
Many of our assessments are available online in multiple languages; others are only
available in paper format. Paper format tests are available via licence to copy; separate
administration fees apply. When taking the assessment online, respondents can simply
select a language from the drop-down menu. Please call our client services department to
determine whether the assessment is available in the needed language and to determine
what administration options are available.
If the language I need is not available, can I translate the assessment myself?
It is against MHS policy to translate the assessment without the express permission of
MHS. Requests to translate an assessment should be made to MHS prior to applying for the
discount as this option is not available for some assessments. A separate administration
procedure applies to translations, please contact permissions@mhs.com for details.
What is to be included in my research summary? How will this information be used?
All those granted the research discounts are required to provide a report of their results to
MHS upon study completion. This report can be structured much like your proposal
(including title, investigators, abstract, method, and analysis) with the addition of a results
section that describes the results of the study and relates these to the research hypotheses.
Where applicable, indicate whether the research has been submitted or accepted for
publication, and include citation information. The report need not exceed 3 pages in length,
nor does it need to include the data itself.
Research summaries are used to contribute to the research and general community. MHS
may share the details of your summaries with interested students, researchers, clinicians
and clients. You may also find your research featured in one of our publications or
webcasts. Please keep us up to date on any publications you have made using our tools and
well be sure to include your citations in our documents.
Ive used the tool and I now need help with the analysis. What can I do?
A supervisor who endorses the application agrees to provide assistance in the
administration of the assessment and the interpretation of the results, and should therefore
be your first point of contact for issues arising from your study.
Please contact our Research and Development Department at r&d@mhs.com if you have
any technical questions regarding scoring or interpretation. Although we may be able to
answer questions of a general nature, the responsibility for research design, statistical
analysis and interpretation of the results rests with you and your supervisor.

Request the submission of a new application should the researcher request additional
materials or an extension outside of the finish date specified above, or use of the
materials for a new purpose.
Access the data in our database and use this data for ongoing research purposes
royalty-free. Any data accessed will not be connected to any specific researcher or
account holder. All names or other identifiable information will be removed prior to
our use of the data.
To report on the results of your study, royalty-free as described in your submitted
research summary.
