Nursing Assessment

European, Female mid 60s, 65kg


Works full time as taxi driver, mixed shifts.
Married with children
Lives with husband in own home
Usually active and fit
ETOH 4 wines per week.
Ex smoker - 1994
No cultural or spiritual affiliations
Past Medical/Surgical History:
Mild Osteoarthritis
Mild Asthma not treated, no exacerbations within last year
Dyslipidemia (Raised cholesterol) not treated
Family History:
Brother CABG @ 70yrs
Sister MI @ 60yrs
Mother angina
Referrals to Cardiac Multidisciplinary team
Occupational Therapist to provide guidelines for returning to work,
driving and normal daily activities
Physiotherapist to provide walking plan of gradual return to normal
activity
Social Worker due to inability to work, assess eligibility for benefits
Cardiac Rehabilitation Nurse Specialists provide education about
MI, angioplasty results, risk factor management, recovery,
medications, AAP, answer questions and outpatients cardiac
rehabilitation programme.

A 29-year old male who suffered multiple pelvic and rib fractures and
internal injuries in a motor vehicle accident four weeks ago has been
discharged from a skilled nursing facility to continue his rehabilitation at
his mothers home. He tells you that he needs some stronger pain
medication because he is experiencing almost continuous pain which he
rates at 5-7 on a 0 to 10 scale. He reports that when he moves, the pain
intensifies to 9. He faces another round of surgery because his hip is not
healing properly. His pain medicines include Percocet and ibuprofen
which he has been advised to take as needed. His mother takes you
aside to express her concerns. She says her son has had a drinking
problem in the past and shes certain hes used marijuana. Furthermore,
she says hes had a lot of visitors since hes been home and he jokes and
smiles a lot when theyre around. Shes not convinced her son hurts as
much as he says he does and shes worried hes becoming addicted to
the Percocet because he takes several pills a day.
1. How would you respond to this patients request for stronger pain
medicine?
2. Do you think he is showing signs of addiction?
3. What would you say to his mother?
