I. INTRODUCTION

The institutional plan aims to improve and develop the different key areas of
education to attain quality education. Thus, the school has to do its best to continue, support
and implement the appropriate national policies, strategies, programs and projects of the
Department of Education.

ALTERNATIVE DELIVERY MODES IMPLEMENTATION


Republic Act No. 9155 hereby declared the policy of the State to protect and
promote the right of all citizens to quality basic education and to make such education
accessible to all by providing all Filipino children a free and compulsory education in the
elementary level and free education in the high school level. For that, the school implemented
the alternate delivery mode to the students.

For the past few years, the drop-out rate is a problem of the school. It is the aim of
the school to reduce the dropout rate. To achieve this, special activities and programs conducted
by the school and the attention and support given by parents. Monthly reports on attendance for
each section, list of students for close monitoring quarterly, and accomplishment report monthly
were submitted. Also, consistent home visitations for the students at risk of dropping out were
done by the advisers.

MONITORING AND SUPERVISION OF SCHOOL HEAD

To quality assured the achievement of the ultimate goal of the Department of Education
which is academic achievement, supervisory plan and supervisory report was prepared and
submitted by the school head monthly. One hundred (100) percent (%) of them were observed.

PROGRAMS AND PROJECTS

Health and Nutritional Status

For the current school year, there are 76 students under the wasted category and only 6
students belong to severely wasted. Also, there are more male students with nutritional status
than those of female students. Thus, only a small portion of the total population suffers
nutritional status problem.

The most common problem that the students suffer is the visual impairment having 67
students and the least they suffer are hearing impairment and autism spectrum.

Violence Against Women and Children

BuNaHis observed its kick-off ceremony for the campaign to end Violence Against
Women and Children last November 28. Both women and teachers posed to support the
program.

Different activities were done; film viewing, essay writing, poster making, slogan
making, movie making and symposium.

Appreciation of students work was done on this culminating activity held on the
BuNaHis gymnasium. Students were appreciated on different activities with VAWC campaign
period. Mr. Maglinao, school ESP coordinator, gave the closing remarks.
DRRMC
In celebration of this years National Disaster Consciousness Month with the theme
Kahandaan at pagtugon sa Sakuna, Tungkulin ng Bawat Isa,our school has organized an
informative bulletin board to let everyone be informed on what to do, before and after disaster.
The bulletin board contains necessary information which will raise the awareness on how to
prepare during disasters like earthquake, fire and the like.

On the other hand, the SWT (School Watching Team) identified the hazard and risks
within the school campus. The assessment of the surrounding and the classroom is better than
last years assessment. This proved that the suggestions relevant to safety by Bureau of Fire
protection and City Disaster Risk Reduction Officers were followed.

National Reading Month Celebration


The more you read, the more things you will know. The more that you learn, the more
places you will go Dr. Seuss.

This is the line that fits the festive celebration of our school-English Department as it
celebrates National Reading Month in November.

English Department brought BuNaHis community to the magical world of reading as


they celebrated National Reading Month. The department held programs to culminate the
celebration and to achieve its goals which are to deepen the appreciation for books, exhibit the
writing and visual art skills, expand students vocabulary, encourage accurate word usage,
develop skill in oral language and all expressive mechanism (voice and bodily expression)
promote teachers, students and community relations and camaraderie and gear up the students
with values and meaningful life lessons.

MAPEH Day

School is a place where students are nurtured toward becoming responsible and
productive members of society. School provides a means of molding students into capable
professionals in their fields, Intramurals allow students to develop psychomotor skills and fine-
tune motor skills with coordination, movement, strength, dexterity, grace, speed and to teach
value of sportsmanship, the ability to accept defeat gracefully is a sign of maturity and a requisite
skill for success.

Our school conducted the MAPEH Day to facilitate the Sports Festival. Activities were
HAtaw Sayaw, palaro ng Lahi, Tawag ng Tanghalan and the much-awaited contest, Mr. and Ms.
MAPEH Day 2016.

School-Based Immunization Program

The School-based Immunization Program was conducted through the provision of


Measles Rebella (MR) and Tetanus Diptheria (TD) vaccines to all grade 7 students. The
Department of Health (DOH) used vaccines approved by the world Health organization (WHO)
which are safe, effective and use worldwide.

Barangay Health Workers (BHW) collaborated with the Lipa City Health officers for the
implementation and conduct of the vaccination.
Nutrition Month Celebration
In response to DepEd Memorandum No. 100, S. 2016, BuNaHis conducted the week-
long celebration of Nutrition Month 2016.

Activities were Poster-Making, Tiangge and Parade of Dishes.

The theme of the said celebration is First 100 Days Ni Baby Pahalagahan Para Sa
Malusog na Kinabukasan.

School-Based Management

The school already attained level II (Progressive) as the result of the validation conducted
by SDO Validation and Monitoring Team. It means that the school intensifies mobilization of
resources and maximizes effort to achieve desired learning outcomes. Also, it introduces and
sustains continuous improvement process. Moreover, the school encourages the maximum
participation of all stakeholders.

School to School Partnership

In line with DepEd Order 83, s. 2012 entitled Implementing Guidelines on the Revised
School-Based Management (SBM) Framework, Assessment Process and Tool (APAT), Bulacnin
National High School cultivated partnership with Sto. Toribio Elementary School. With this, the
former as the Leader School guided the latter to attain and achieve its vision, mission and goals.
Upon assessing the SIP, the current problems of the partner school were found to be high
drop-out rates and poor achievement in English and Math. These needs were necessary in
formulating the Work and Financial Plan and Action Plan.
There were many accomplishments of the said partnership. The teachers of both the
partner and the leader school crafted the instructional materials suited for each of the grade level.
The monitoring, evaluation and consultation of the instructional materials made were conducted.
The leader school concluded that the instructional materials crafted can help in the differentiation
of instruction. The leader school facilitated the Learning Action Cell (LAC) sessions of the
teachers from the partner school on October 24-26. These LAC sessions aimed to foster
productive learner-teacher relation thru understanding of different pedagogical approaches,
differentiated instruction and e-learning.
For the month of November, Bulacnin National High School and Sto. Toribio
Elementary School faculty and staff worked collaboratively to craft instructional materials for
teaching and learning purposes. Educational materials and supplies were delivered. Also, The
teachers utilized the said materials for all the pupils of partner school across all subject areas and
grade levels.
Meanwhile, the learning modules were distributed. The compiled/downloaded reading
materials were given to the all pupils. Materials were also utilized to help the Pupils at risk of
Dropping out. Lastly, the compiled/downloaded Math worksheets were used for Grade 3 and
Grade 6 pupils.

STAKEHOLDERS PARTICIPATION

For school year 2016, in terms of co-curricular activities, 73.08% of the stakeholders
participated. In terms of extra-curricular activities, 73.74% of the stake holders of the
stakeholders attended the activities. Finally, 36.38 % of the stakeholders attended other major
activities.
Stakeholders contributed Php 10,000 in MTAP classes, Php 15 000 in Intramurals and
another Php 15 000 in Scouting. Similarly, the stakeholders also contributed Php 15 000 in
MHCC. In addition, they also donated Php 30 000 in drug symposium, Php 2000 in Fire and
Earthquake Drill and Php 5 000 in Cell Group.
SM City Lipa is the most active stakeholder since 2012.
