Satisfactory Satisfactory J.J. : I practice proper and standard care for all of patients. First of all, Safety and quality of care is very important for patient care. As a student nurse I follow the protocol and policies by being in full uniform, being on time, and giving meds on time and follow assessments care. 1. 2 Act in a responsible, ethical and accountable manner. Satisfactory Satisfactory J.J. : I show responsibility, ethical and accountability by being on time for clinical, doing medication research prior to clinical, doing concept map prior to clinical, and by giving appropriate patient care. I also participate in discussion with clinical groups and in class. I help my colleagues with ADLs, vital signs and medication. 1. 3 Determine strengths and limitations and when to consult and seek assistance from other professionals. Satisfactory Satisfactory J.J. : I review my strength and weakness every week through K4Y and try to improve the following week in clinical. When I have any concerns I talk to my clinical teacher to make sure that i am giving proper care for my patient and also for my knowledge. For example, when one of patient had high Blood pressure even when i give the blood pressure medication and I talked with Zaynab to make sure whether the results were normal or abnormal. 1. 4 Use evidence informed practice and a theory-based approach to care. I used health assessments (Jarvis) and potter and perry for information prior to clinical. I use Mosby drug guide for my research for medication, prior to clinical. I use theory based information both in class and in clinical. 1. 5 Ensure client safety and a safe environment. Satisfactory Satisfactory J.J. : I create safe environment for all my patient in clinical. I ensure that by making sure that the bed is in the lower position, side rails are up when my patients are in bed, have assistance device near by, call be near by, and even walking with my patient to make sure they don't injure them self. I also make sure the the side table is near by the patient and have their belongings near by such as: phone and bible. I also make sure that I wear gloves and use hand hygiene before entering the room. 1. 6 Ask relevant questions to clarify understanding of the plan of care and whenever uncertain. Satisfactory Satisfactory J.J. : I ask relevant question to clarify my understanding for better care for my patient. For example, one of my patient had low temp was low so i told me primary nurse because I was concerned. My nurse told me to put on couple of warm blankets, and when i re-checked the vitals in the afternoon the temp was normal. 1. 7 Identify ethical and unsafe situations and seek appropriate assistance to resolve these. Satisfactory Satisfactory J.J. : I had an interesting experience in week 12 of clinical with my patient, and I talked to my clinical teacher and she told me to talk to patient about it. I ended up talking to my patient and clarify everything. Another example, is when I let my primary nurse know that I am not allowed to give IV and insulin medication, so that the nurse will give the meds on time and the patient will be safe. 1. 8 Demonstrate understanding about the role of the student nurse. Satisfactory Satisfactory J.J. : As a student nurse in clinical for first time I practice along side my primary nurse every week. I practice my vital signs, head to toe assessment, medication, and ADLs. I also had the chance to do g-tube feeding, inhaling medication, rectal medication and dressing. As i progressed learning I was able to do change the dressing for stage 2 coccyx ulcer by myself. Teacher comments for COMPETENT AND SAFE PRACTICE WITHIN A PROFESSIONAL, LEGAL, AND ETHICAL FRAMEWORK : Jisha was present and prompt for all clinical dates. Jisha ensured she complied by the guidelines stated in the handbook. She was prompt, in uniform and prepared to the best of her abilities. Jisha submitted assignments in a timely manner and completed K4Y on time. Jisha took responsibility for her own actions and asked questions accordingly. She understands her role and responsibility as a student nurse with respect to practice limitations, documentation and reporting. When able, Jisha used theory to guide her learning. She ensured a save environment for her patients by ensuring the bed was always in the lowest position, call bell was within reach and the patient had their belongings within reach. When Jisha provided care she used evidence-based researched to provide the care (i.e. following the rights and routes of medication administration, changing dressings, etc). 2. ACCURATE AND RELEVANT COMMUNICATION 2. 1 Participate in team sharing and discussions. Satisfactory Satisfactory J.J. : I participate in group discussion and team sharing at clinical. I participated both in class and at clinical. In post confreres I participate by answering question and discussing about the day, and how collaborative care impact each one of our patient. During clinical when i am unsure of some something such as abnormal vital signs i consult with my teacher before entering it into meditech. Another example, when we had post conference we discuss what other collaboration can be used for each of our patient. 2. 2 Document clearly, accurately, concisely in a timely manner using written and electronic methods. Satisfactory Satisfactory J.J. : I had some trouble with time management prior to midterm and I improved and managed my time ever since. I also document vital signs, sign medication, document, head to toe assessment, and ADLs clearly and accurately. I am more comfortable with documenting all the information to meditech than I was in week 6. I also let my primary nurse know of the vital signs when there is a shift change. 2. 3 Report relevant information to the appropriate personnel. Satisfactory Satisfactory J.J. : I report relevant information to the appropriate personnel such as my clinical teacher and my primary nurse. In week 11 my primary nurse wanted to make sure the respiration count, so we both counted the respiration together. Also in week 12 there was a shift change for my primary nurse and I had to discuss my afternoon vitals with the new primary nurse during shift change. 2. 4 Utilize communication techniques with the client and inter-professional health care team. Satisfactory Satisfactory J.J. : I utilize communication techniques with the client and inter-professional health care team. In week 12 of clinical I had to talk to clinical teacher the proper way to talk to my client, and with my teachers help I knew how to talk to my client and i ended my day with a good conversation with my client. I also talk with my primary nurse during shift report and I let the nurse know who my patient is and everything I am assigned to do. I also greet and introduce myself to my patient every morning and check on them though out the day. 2. 5 Maintain privacy and confidentiality according to the Personal Health Information Protection Act (PHIPA) Satisfactory Satisfactory J.J. : I maintain privacy and confidentiality by making sure that I don't talk about my patients personal information with others outside of clinical. I also don't leave any of my patients information lying around. I also make sure when i do any procedures I close the curtnes. I also don't share my patients information with my friends in other clinical groups. 2. 6 Use technology to retrieve information including research, data and other information. Satisfactory Satisfactory J.J. : I use technology to retrieve information including research, data and other information. I use technology prior to clinical to look up medication and also my patients problems and abbreviation. I also check my patients history at clinical, and also double check all the medication. I also do research for my classes as well as clinical to make sure i have the right information. Teacher comments for ACCURATE AND RELEVANT COMMUNICATION : Jisha has done a good job at communication throughout this clinical. Jisha worked well with others and actively participated in group discussions, sharing her experiences and thoughts. She understands the importance of communication and documentation. She documents accurately and thoroughly, with minimal assistance. She communicates well with staff and patients, often demonstrating great patience when the patient does not understand. Jisha ensures all of her findings, particularly things out of norm (i.e. vital, blood work results, patients current attitude, etc) is documented and shared with the appropriate persons. She is cognizant of her surroundings and the importance of maintaining patient privacy and confidentiality (i.e. not sharing information about patients in front of other, discussing patients in the elevators or hallways). She used technology to find research to support her care and research medications. 3. CARING RELATIONAL PRACTICE 3. 1 Utilize effective communication techniques. Satisfactory Satisfactory J.J. : I utilize effective communication techniques with clients. I always talk to client and make sure they are comfortable. When i take vital signs I always let them know of their blood pressure. I also encourage my patients for example in week 12, my client was totally capable of walking, however, my client preferred to stay in bed. I told my client that walking and movement is good for the body. I demonstrate therapeutic care through good communication. I listen to my patients and I also talk to them while I do any procedures. For example, one of my patient wanted to walk and I walked with him to make sure that he doesn't fall. I make sure that i always introduce my self to my patients and say good bye when i leave. 3. 3 Apply knowledge of therapeutic use of self and identify its effect on client well-being. For example, one of my client wanted me to use shower gel and location that his family provided him with, and I used what he wanted me to use. 3. 4 Use self-awareness to identify the effects that beliefs, values, and personal experiences have on relational practice. I had few clients that prayed and read religious books, while i check on my patients, knowing that i try not to disturb my clients quality time. I also had a client that taught me a word in his language and i used it few times when I checked on him and he was very happy. 3. 5 Interact with clients and members of the inter-professional health care team; and, consult appropriately. The first thing I do when I enter my clients room is introduce my self by saying my name and my title. I interact with my clients through out the day to get to know them as well as taking care of them. I also interact with other health care team members such as: physiotherapist, family and nurses. One of my client had to work with the physiotherapist and I had the chance to talk to the physio and give his vital signs. I had the chance to see some of the family members and while doing any procedures such as vital signs I talk to my clients and family. I work with nurses every week and I let them know what I am assigned to do for that day. 3. 6 Apply knowledge of practices and systems that support the diversity of clients and the interprofessional health care team. Satisfactory Satisfactory J.J. : I apply knowledge of practice and systems that support the diversity of clients and the inter- professional health care team. For example, one of my patient wanted to take a bath and she wasn't comfortable with another nursing student helping me because other student was a guy. Also, one of my patient was not comfortable with eating the chicken soup because she wasn't sure if it was halal, so I talk to me primary nurse and resolved it. 3. 7 Apply knowledge of clients cultural, religious, and other beliefs and values as these influence clients choices and decisions. Satisfactory Satisfactory J.J. : I apply knowledge of client's culture, religion, and other beliefs and values as these influence clients' choice and decisions. For example, when i check on my patients i have seen few of my patients read religious books and i respect it and give them the privacy they need. I also had couple of patients that preferred to wear scarfs on their head, because of their culture and religion. I also helped my clients to put on the scarf. 3. 8 Provide effective client education. Satisfactory Satisfactory J.J. : I provide effective client education. For example, one of my client was fully capable of walking, but every time i asked him would you like to sit on a chair or take a walk. My client preferred to stay in bed. I did talk to him, and told him that moving and walking is good for his body and that he will gain more strength. My patient was willing to walk and go to the patient shower room and even took the shower by himself. Teacher comments for CARING RELATIONAL PRACTICE : Jisha has been a caring professional. She has always respected the culture of others and has an appreciation of how that can effect ones choice. When providing care Jisha is aware of her own values and beliefs and makes sure they do not interfere with the care she is providing. As she had the opportunity to work with patients with different cultural backgrounds, experiencing difference diagnosis, Jisha was able to appreciate how difference patients required different methods of care. For example: An aphasic patient who was still able to communicate using physical signs, or a patient who was only able to communicate with her eyes, etc. When caring for a patient who was aphasic, had left sided paralysis and had only 5% movement to her right side, Jisha made sure she continued to engage the patient and explained to her everything she was doing. She treated every patient with respect, despite the level of care they required (independent versus total care). In addition, she provided effective patient education during feeding, medications, diagnosis, the importance of ambulation, etc. 4. LEADERSHIP AND ADVOCACY 4. 1 Recognize conflicts and strategies for resolution. Satisfactory Satisfactory J.J. : There was an incident were one of my colleague's patient had a dressing for necrosis. My friend actually did the dressing with the help of Zaynab, and the family was present (all the nursing students was watching the dressing). After the dressing was complete the family wanted the dressing to be removed and the Doctor debride necrotic tissue. Also, the family wanted the patient to be transferred to another hospital and also wanted maggot treatment. The doctor, wound nurse, and even my clinical teacher tried to teach the family about the risk of the necrosis and how it can lead to infection and the patients life can be on risk. That amputating below knee will be one of the best choice for the patient, however it was the families decision. 4. 2 Demonstrate leadership skills that foster group work and a positive learning environment. Satisfactory Satisfactory J.J. : One of my patient was behaving inappropriately, and I reported that to Zaynab. After sharing it with Zaynab I talk to my patient and explained that I am a nurse providing care and it is just therapeutic relationship. I felt confident after the discussion with my patient, because I clarified what was happening before with my patient and me. Also, there was an understanding. I also help my colleagues when they need help which include: ADLs and re-positioning the patient. 4. 3 Identify opportunities to improve health care service and nursing interventions by advocating for client, self, others, and quality practice. Satisfactory Satisfactory J.J. : I find opportunities to improve health care service and nursing interventions by advocating for client, self, others, and quality practice. One of my patient wanted to whether the food had any halal in it. I first checked the menu that my patient received. Since, the menu doesnt explain if the food is halal or not, I contacted my primary nurse, because I wanted to make sure that my patients culture preference is being valued. However, my primary nurse was not sure if the soup was halal, so we told the client and my primary nurse made sure that she will mention it to the people who take the order for food. Another example will be attending the infection control in service for the better outcomes for patient safety. 4. 4 Provide feedback to peers and accept feedback from peers and members of the interprofessional health care team. Satisfactory Satisfactory J.J. : I provide feedback to peers and accept feedback from peers and members of the inter- professional health care team. During this semester we were required to do peer/self review, I wrote three of my strength and weakness. Of of my wrote their opinion on my performance as well. I also get feedback from Zaynab through K4Y, it indicate things i am doing good and things i need to improve on. I gladly accept the comments improved in clinical. 4. 5 Support clients rights for self-determination and choice. Satisfactory Satisfactory J.J. : I support clients' rights for self- determination. For example, I had a client who refused to let my primary nurse and me to take vital signs in the morning, because he was sure that another nurse took it. We tried to teach him and told him that it was just to make sure everything is normal, but he still refused. However, when we checked to see the vitals there was no reports. However, when i went in to check his vitals in the afternoon he let me do it. Another example is when one of my patient wanted to take a walk and I supported his decision by assisting him while walking. 4. 6 Respond appropriately to unsafe, unacceptable, and unprofessional behaviours. Satisfactory Satisfactory J.J. : I respond to unsafe, unacceptable, and unprofessional behaviours appropriately. Like mentioned before I had a patient that was behaving inappropriately and I first reported it to my teacher and talked with my patient and make sure that there is an understanding, and that my patient understand that it is just therapeutic relationship. Also, I have seen patients that are not positioned properly on the bed, so I ask one of my friends to help me, so that I make sure my patient is safe. 4. 7 Consult and collaborate when carrying out health care and nursing practices that are in the best interests of the public and protect the public through collaboration and consultation. Satisfactory Satisfactory J.J. : I consult and collaborate with my primary nurse. I share my patients vital signs with the primary nurse and I also inform what as a student nurse i can do in the unit. In week 13 we had a presentation with the pharmacist on how the pharmacy and pharmacist collaborate with other health professional. I also had a opportunity to talk to the physiotherapist, because one of my patient had collaborative care with physio. Teacher comments for LEADERSHIP AND ADVOCACY : Jisha has demonstrated good leadership skills. She advocates for clients when she feels she needs too (i.e. with medications, nutrition, wound care, etc). Jisha supports the choices a client makes when she is positive they have enough information to make an informed decision. When caring for patients who require close contact (i.e. helping with peri-care, have a strong cough), Jisha protects herself, others in the organization and the public by ensuring she wears gloves, wears a mask, and completes hand hygiene appropriately. Jisha is always supportive of her peers. She provides assistance when they require it. Jisha also provides feedback to others in a non-judgemental way. She accepts feedback graciously, and attempts to make changes to her practice accordingly. 5. PLANS OF CARE: MEET CLIENTS NEEDS AND MUTUALLY AGREED EXPECTED OUTCOMES 5. 1 Incorporate the unique needs and expected outcomes into individual clients plans of care. Satisfactory Satisfactory J.J. : I incorporate the unique needs and expected outcomes into individual clients' plans of care. For example, I make a concept map prior to clinical and when I do my head to toe assessment I refer it to my interventions. I also look up my patients past history and admitting diagnosis, so that i can use the information while i care for my patient. For example, one of my patient had respiratory problems, so few of my intervention was auscultating lungs and checking for shortness of breath and respiration, and i use concept map as my guide. 5. 2 Develop theory-based plans of care that are holistic and client centered and are articulated competently in writing and orally. When i do head to toe assessment i prioritize and do the assessment based on my patients past history, admitting diagnosis, and medication. I do research prior to clinical that helps me while i take care of my patient. I document all my findings accurately and communicate with my patient while doing any procedure. I also communicate my finding with my primary nurse. 5. 3 Use knowledge, skill and judgment to assess clients, prioritize needs and outcomes. For example, when I receive patient assignment I research both past medical history and admitting diagnosis, so that I have a clear idea of how I can prioritize my assessments and care for my patient. I also make the connection between the medications and health conditions. 5. 4 Provide information that contributes to the evaluation and refinement of plans of care. Satisfactory Satisfactory J.J. : I provide information that contributes to evaluation and refinements of plans of care. Communication is a key for the better outcome for the patient. For example, in week 13 my patient had aphasia and dysphasia, so I used both verbal and non-verbal communication. The patient was able to point and say few words me. I also talk to my patients and encourage them while i do assessments on them, so they can also participate in the care. 5. 5 Safely and competently carry out nursing assessments and nursing interventions that are theorybased and within skill abilities. Satisfactory Satisfactory J.J. : I do research prior to clinical that helps me through out my clinical day. I use concept map and assessment check list for my patient care. When i do research i look for information that nurses are allowed to do and practice. Potter and Perry, Mosby, Jarvis are some of the reading and research I do to help with my clinical. I made changes to the way i practice. For example, week 6 was the fist clinical day for me and the way i did my assessments were different I try to do so much and had trouble prioritizing, but now I look at my concept map and the patients history and know which assessment i should start with and why i have that intervention for. 5. 6 Carry out nursing care that reflects the practice decision-making framework. Satisfactory Satisfactory J.J. : According to Potter and Perry decision making is the result of critical thinking that focuses on problem solving. I focus on my clients diagnosis and initiate my care based on the findings. When I do any procedures I constantly think of prioritizing and how I should do it and what I should start with. For example, when I am at clinical I check vital signs twice and one of my patient had respiratory problem and I knew that his respiration is high because of his condition. 5. 7 Complete nursing care responsibilities and nursing interventions in an organized and timely manner. Satisfactory Satisfactory J.J. : I compete nursing care responsibilities and nursing interventions in an organized and timely manner. Although, I had some issues with time management prior to mid-term, I focused on my time management and completed all my assessments on time accurately and with care. I also document all my findings after any care. I also helped some of my friends with ADLs as well. For example, one of my friend had a patient with Parkinson's disease and i helped her while she give a bath for her patient. Teacher comments for PLANS OF CARE: MEET CLIENTS NEEDS AND MUTUALLY AGREED EXPECTED OUTCOMES : Jisha has worked exceptionally well at following a care plan and meeting the patients needs. Jisha is aware that all patients are different and they have different needs. She ensures her care is patient specific. She completes accurate and thorough head to toe assessments. Her assessments are specific to the patient and she often focuses on the system related to their clinical diagnosis. Jisha is aware of her limitations as a student nurse and only implements nursing interventions which she knows and understands how too. When there are any changes to the patient, she reports this to the primary nurse or myself so the care plan can be changed accordingly. She has improved over the term in completing nursing responsibilities and interventions in a time manner. She administered medication confidently via all routes except for intravenous. Overtime, Jisha has also improved on creating concept maps which are specifically related to the patient's diagnosis and their current health state. 6. COLLABORATIVE PRACTICE: INTERPROFESSIONAL HEALTH CARE TEAM 6. 1 Work with clients and members of the inter-professional health care team to assess clients, determine health needs, and to achieve mutually agreed expected outcomes. I worked with one of my colleague while checking vital signs and assessments. I helped few of my colleagues with ADLs and with re-positioning as well. In week 13 my patients temp was low, so I talked to my primary nurse, and I applied couple of warm blankets on my patients . 6. 2 Contribute as a member of the inter-professional health care team to respond to the changing needs and expected outcomes of clients. Satisfactory Satisfactory J.J. : In week 13 my client needed dextrose 50% injection because the clients blood sugar was low, and my primary nurse explained the protocol and I watched her while she give the IV and i also timed her. After the IV my primary nurse checked the blood sugar, and it was elevated. 6. 3 Seek out assistance and consult with members of the inter-professional health care team. Satisfactory Satisfactory J.J. : In week 12, Zaynab checked all my medications with me, and i had the chance to administer it by my self and one of my colleague watched me while giving the medication. After administering the medication I informed Zaynab and made sure both of us signed the document. I also ask other nursing students when I need any help with my clients such as: moving the patient and complete bed bath. 6. 4 Use teamwork, consensus building, and conflict resolution skills. I usually help with bath/shower, mobility, and vitals. Another example was when one of my patient was being inappropriate with me, so i talked to my clinical teacher and had a talk with my patient and confronted him and made sure that there was an understanding between us. 6. 5 Describe effective, collaborative, and consultative strategies to meet clients needs within a changing environment. Satisfactory Satisfactory J.J. : I make concept map every week to help me guide how I treat my patient and to prioritize the treatment. In the the morning i look my patients chart and make changes changes to the assignments. I also participate in the end of shift report to know more information on my patient and how it effect the treatment for the better outcome of the client. 6. 6 Interact with members of the health care team respecting their unique role and competencies. In week 13, we had the pharmacist talk to us about what they do and how they collaborative other health care members. We also had in- service with the infection control to how to limit infection around the hospital and patient to patient. I also got the opportunity to work with the physiotherapist, because one of my patient needed help with mobility. She actively contributed as a member of the healthcare team by caring out all nursing interventions within her limits. Jisha has good team work skills. She worked will with her peers and with other members. She assisted other nurses as they required assistance. She has knowledge of and appreciates the various members of the healthcare team. Although, she was not able to collaborate directly with them, Jisha was able to discuss which members of the healthcare team she would consult in her patients care and why. 7. Every week in clinical i get a primary nurse along with my clinical teacher and i communicate with both when I am I need help with anything or when I find any abnormal results in my patient. I also let my primary nurse know what I am allowed to do as a first year student. For example, I had a patient with low temperature and I was concerned, so i talk to my primary nurse and she told me i just have to put extra layers of blankets. 7. 2 Participate in self-reflection to identify strengths, areas for improvement. For example, I got to do a coccyx dressing for stage 2 ulcer by my self in week 13, and also i got to see the insertion of foley catheter in clinical setting. I perform safe practice, legal, and ethical framework for all my clients and also interprofessional team. I was able to promote therapeutic care for all my clients, and also respect my clients culture and religious belief. All in all, I was able to improve every week of clinical and learn and find learning opportunity to further expand my knowledge and also for my clients. Teacher Final Summary with Strategies for Continued Growth As the semester progressed Jisha became a more competent and confident student nurse. She continued to adhere to the guidelines set by Humber College and the practice facility. She demonstrated professionalism with her patients, her peers, and other staff. She competently completed head to assessments and documented her findings into Meditech. She advocated for her patients and demonstrated leadership by supporting her peers. Jisha accepted feedback provided to her and made changes to her practice whenever she was able too. Jisha administered medications through a variety of routes (PO, SC injections, inhalation, nebulizers, rectal and Peg-Tube). When administering medications, Jisha was prepared and was almost always able to make the connections between the medication and the patient's diagnosis and past medical history. To maintain her success, in the upcoming semester, Jisha needs to continue to practice her head to toe assessments skills. This will maintain her competence and help hone her skills. She should explore other resources (i.e. peer-reviewed journals, videos, and text-books) to broaden her knowledge base, deepen her understanding of concepts, and ensure her practice is evidence-based. Jisha should maintain familiarity with the CNO practice standards to ensure her practice is up to par. She should also ensure she seizes new opportunities and continue to challenge herself, as this will further increase her confidence and competence enabling her to provide better patient care. Furthermore, it is important for Jisha to engage in on-going self reflection to recognize her strengths and weakness, and seek assistance accordingly. Lastly, Jisha needs to ensure when administering medications it is critical to understand why this patient is receiving this particular medication, and what the nurse should monitor pre and post (i.e lab work). It has been a pleasure having Jisha as a student nurse for this clinical rotation.
