ENERGY STAR CLOTHES WASHER REBATE APPLICATION 

Please verify that the following requirements have been met before submitting this form 
Rebate Requirements 
1.  Applicant must be a current residential member of an electric co‐op participating in Wabash Valley Power's 
ENERGY STAR Clothes Washer Rebate Program 
2.  Member must use an electric water heater in the home where this clothes washer will be used 
3.  Rebate amount is $50 
4.  Only units purchased on or after April 15, 2010 are eligible 
5.  The ENERGY STAR qualified clothes washer must be installed and operable in applicant's owner‐occupied 
residence located on co‐op lines 
6.  Rebates are for either replacement of an existing clothes washer or first‐time clothes washer purchase 
7.  One rebate per member primary residence; one rebate per member secondary residence on co‐op lines 
8.  Applicant must agree to allow verification of:  clothes washer installation, ENERGY STAR qualification and that 
an electric water heater is used in the home 
9.  All fields must be completed on the form to receive a rebate. Incomplete forms will not be processed 
10.  A copy of the invoice for the ENERGY STAR Clothes Washer showing the manufacturer and model number 
must be attached to this form.  Also, a copy of the Energy Guide label showing the ENERGY STAR logo must be 
attached to this form 
11.  Please allow 6 to 8 weeks to receive your rebate.  Rebate may be a check or bill credit at the sole discretion of 
your local electric cooperative 
12.  Wabash Valley Power Association and its member electric cooperatives reserve the right to change or 
terminate this program at any time without notice 
13.  Completed and signed rebate forms with a copy of the proof of purchase and Energy Guide can be mailed or 
dropped off at your electric cooperative:
