1. SMMARY / AREAS OF CONCERN:

1.1 Raw plate material inspection
As per ITP step 09(Incoming raw material quality control and assurance-Raw material inspection, including
material identification, appearance and dimension inspection), GQIS inspector carride out inpection for raw
plate material, the detail inspection scope as below table, all inspection data found in conformity with ITP &
specification and traceability, the surface hasnt found any indication of damage. The result is satisfactory.

1.2 Witness impact testing

As per ITP step 08(Incoming raw material quality control and assurance-steel plate and coil material
properties retest), GQIS inspector witness impact testing, before testing, GQIS inspector reviewed impact test
equipment calibration certificate and valid time and confirmed that are satisfactory, inspection the sampling
dimension and confirm that are satisfactory and traceable, confirm the impact test process in accordance with
EN ISO 100045-1, the test result in conformity with standard of BS EN10025-2 and BS EN10025-4.

1.3 Witness chemical composition analysis

As per ITP step 08(Incoming raw material quality control and assurance-steel plate and coil material
properties retest), GQIS inspector witness chemical composition analysis, before testing, GQIS inspector
reviewed chemical composition analysis equipment calibration certificate and valid time and confirmed that
are satisfactory, inspection the sampling dimension and confirm that are satisfactory and traceable, confirm
the chemical composition analysis process in accordance with EN ISO 14284-1996, the test result in
conformity with standard of BS EN10025-2 and BS EN10025-4.

1.4 Inspection ancillary installation for pile 3568-C05-068

As per ITP step 15(production and installation of ancillary), on 30th August, 2016 GQIS inspector witness
hydrostatic testing for flexible hose, the result is acceptable. But when assebling the flexible hose the factory
welding socket with pile first, after threaded flexible hose with socket again, in this case need perform
hydrostatic test again, also that fabrication process is not in line with drawing requirement(drawing require
fixible hose end fitting to be threaded into full socket prior to welding to plate/pile shell), after consult with
factory they state they would manufacturing strictly according to the drawing and specification in the future.
After threaded flexible hose with socket found the hose more longer than drawing requrient, the extre length
about 240mm, this maybe affect the medium fluid speed, factory state they will send this batch of hose to
vendor to modified the flexible hose according to drawing.
