Course This course utilizes three major units of study to address the Canby School District and State of Oregon Common
Descriptio Curriculum Goals for Health Education, and Career Related learning Standards.

This is a wellness course designed for ninth grade students to learn healthful behaviors that will enhance their current lives and
prepare them for the demands and stressors of post-high school education, work and family life. The course includes study on the
concepts of wellness and the skills needed for healthful behaviors in mental, physical and social health.

This course is a graduation requirement at Canby High School.

Text Glencoe Health textbook textbooks will be provided for each student to use while in class when needed.
Students are not to take textbooks out of the classroom; however, textbooks are available for individual students
to check out in the library with picture id

Hours
Classroom The classroom is an environment for supporting learning and student success. As such, any action that inhibits
either will not be tolerated. Some further guidelines which dictate classroom policy are:

1. Practice Respectful Behavior

2. Come to class prepared and geared up to learn.
3. Your best effort is required each day
4. No drinks in class except water/sports drink
5. All work is to be turned in on time.
6. Maintain a classroom environment that allows all students to succeed.
If student cannot rise to the expectations the following steps will be taken:
Verbal warning; student/teacher conference; third party contact as needed.

Attendanc Attendance is essential for student success. Any attendance issues will be addressed following the school
attendance policy (see student handbook).

If tardiness/absenteeism becomes an issue, parent contact will be attempted and a referral sent to the Student
Center.
If a student is absent, it is the students responsibility to make-up missed material. Each student is responsible
for all course content whether the student was present for class or not.
Organizati It is a great benefit for students to be able to look back at previous work. Thus, materials will be referenced
throughout the course and students are expected to keep all course materials neat and organized.

Supplies Students will need a section in their binder for Wellness, Notebook paper and pen or pencil.
Some projects will require extra materials depending on what the student would like to produce.

Grading Learning activities, worksheets, quizzes, projects, homework, class presentations, discussions and unit tests will be assigned
points based on quantity and quality of workmanship. Assessments are based on national health standards. Grades will reflect
students progress in respect to goals, objectives, scope and sequence described in the planned course statement. The final
exam will be worth 15% of the total points. The total points at the end of the trimester will be assigned a letter grade. Progress
reports shall be given to each student at the sixth week grading periods of the trimester. The teacher will take into consideration
special needs in assigning grades.

At any time the student will be able to check their grade with the weekly grade reports posted in the classroom using student ID
numbers only or from Synergys student view.
Participation All students will receive 10 points per day for proper class participation. Students who are not present will forfeit the participation
points. In the event of an excused absence, see the instructor for make-up work in this area.

Make-up Following an absence, it is the students responsibility to find out what was missed and to plan to make it up. To make up
missed work, arrange a time with your teacher to complete the needed assignments. Any assignment, quiz or test missed due to

an absence will be marked 0.

Late Work Late work is accepted due to absence but must be turned in within a week equal to the number of days missed.
Retake Students can take test retakes. Student will receive the average between the two tests.

Grades Grades will be determined by the following scale:

A+ 99 100% B+ 89% C+ 79% D+ 69%


A 92 98% B 82 88% C 72 78% D 62 68%
A- 90 91% B- 80 81% C- 70 71% D- 60 61%
F Below 60%

Oregon University System:


The Oregon University System (OUS) has recently notified all high schools that only courses in which a student has received a C-
or higher will count toward the college prep credit requirement. This requirement begins with the graduating class of 2005. It is
highly recommended that a student with a D grade in a required college entrance course meet with a counselor to discuss if the
course will have to be re-taken.

Monitorin Students grades will be posted in the classroom by student ID number. Grades will also be accessible via the
StudentVUE and ParentVUE portals online. If there are any questions about a students grade, feel free to contact

participating in class, finishing assignments and doing the study guide that correlates to the unit test for each
