HOW TO SUBMIT AN ENQUIRY

1. Before approaching the FSB, individuals who believe that they are entitled to unclaimed
benefits should directly contact the respective retirement fund in order to establish
whether they have any benefits attributable to them. The details of the fund can be
obtained from the members employer at the time.

2. If the enquirer wishes to complain about the non-payment of a benefit that was due and
payable during the last three years, , the Pension Fund Adjudicator should be approached
for assistance:

3. If the Pension Funds Adjudicator cannot assist, a member can call the FSBs Call Centre on
0800 20 2087 or 0800 110 443 for assistance with the contact details of the retirement fund.
Please refer to the attached flip book prepared by our Consumer Education
Department.

4. Should individuals experience difficulties in contacting the retirement fund directly, they
may submit the attached enquiry form together with the following supporting documents,
as applicable:

Copy of the enquirers Identity Document;


Copy of the deceased or the members Identity Document;
A summary of the members work history
Members payslip indicating that contributions were made to a retirement fund;
Any correspondence between the fund, the administrators of the fund and the
member, such as Benefit Statements, Increase Letters or Member Booklets;
Copy of the members divorce order and settlement agreement;
Copy of the members death certificate or disability finding or report.

Once the documents are received, the FSB will contact the fund with the enquiry and
provide them with 30 days to respond to the query.

NB: The FSB does not regulate or maintain oversight over the following funds, and any
queries regarding these should be directed directly to each:

SUBMITTING AN ENQUIRY FOR SOMEONE ELSE

Individuals are welcome to assist family and friends with lodging an enquiry with the FSB
regarding an outstanding benefit owing to them. Simply assist them with completing the
attached form and submitting it to us as indicated above.

PLEASE NOTE THAT THE FSB PROVIDES THIS ENQUIRY SERVICE FOR FREE.

It has come to our attention that there are individuals assisting members or beneficiaries to
claim their benefits for a fee, despite them making use of our services for free. The member or
beneficiary is then charged an amount (in some cases even a percentage of their total
benefit) for a service that could have been rendered by the FSB for free.

In order to protect members interest, we are cautious of promoting such engagement with
agents/representatives who may exploit members by charging fees for our free assistance.
We also do not want to promote a business practice which may be considered as
undesirable.

If you intend acting as the agent of a member or beneficiary, please provide us with proof in
the form of an affidavit signed by the member or beneficiary (or their guardian in the case of
minors) in which it is confirmed that they are aware and have been informed of the above
details, but that despite this, the person still wishes to use your services, as an agent, to claim
any unclaimed benefit s/he may be entitled to receive from the fund. You should also provide
details, in the form of an affidavit, of the rates you charge clients for this service and proof of
your FAIS accreditation to provide such financial and intermediary services to the public.

NB! BY LODGING THIS ENQUIRY YOU AGREE TO THE FOLLOWING:

You wish the FSB to submit an enquiry to the fund on your behalf;
The FSB will at all times respect your privacy and keep your personal information
confidential;
The FSB endeavours, as far as it is under its control, to ensure compliance with the
provisions of the Protection of Personal Information Act (POPI) when dealing with
your personal information;
The information submitted by you to the FSB will be used solely for the purposes of
lodging such an enquiry on your behalf;
Should your enquiry not fall within the jurisdiction of this offices mandate, you give
consent to the FSB to forward any information submitted by you to another entity
with the appropriate jurisdiction;
Where your enquiry falls under the FSBs jurisdiction, you give consent that the FSB
may share any information submitted by you with any of the relevant parties
involved in the enquiry to find out important information about your case. This
consent will also include details of minor children (where applicable) i.e. birth
certificates of minors or any similar document, where they are beneficiaries with
regards to death benefit claims;
You have the right to object to the sharing of your personal information with other
parties. Should this be the case, then the FSB will not be able to conduct an enquiry
on your behalf and your file will be closed. By submitting the enquiry, the confirm
that you do not object to the sharing of personal information with other parties.
