Exactly 2 months ago I got an inflammation in my left eyes.

Then I went to the hospital


next to my house to get medication, in Pekalongan. My mom said that the hospital is
always crowded. So, I decided to come there around 5.30 am in the morning.
Surprisingly, before 6 am, there had been almost 200 people had lined up / queued. We
waited for the locket to open so we could come into the poly room to get examined by
the doctor. When I talked to a patient beside me, he told me that he even had been
waiting in front of the hospital since 2 am in the morning. And he got examined at
around 11 am in the morning. He waited there for 9 hours. I thought this is ridiculous.
Ladies and gentlemen, many people said that this phenomena was one of some
impacts of BPJS. From that experience, I attempt to learn more about BPJS which is
actually the implementation of universal coverage.
Dear audiences, I am Fathia. I am a medical student, Im passionate to be a doctor
inshaAllah. Today Im going to share at once engage you all here to critize how health
coverage works in Indonesia. Welcome to my speech.
Ladies and gentlemen, back to my previous statement, what Universal Coverage
means? We can simply understand the meaning of universal coverage is that all people
and communities can use health services they need without any financial hardship. I
emphasize one more time, all people. Whether theyre rich, theyre poor, theyre black
or theyre white, theyre urban or villagers. They diserve the same quality of medication.
Why? Because maintaining health is a basic human right for everyone, even for the
sinners or criminals.
The implementation of universal coverage in our country is BPJS. The concept of BPJS
itself is like a cross-subsidy. Which means a sick person will be helped by some healthy
people. In short way, the role of the governement here is as the manager which manage
the funds from the premium then distribute it at once provide health services across the
country.
Ladies and gentlemen, after 3 years managed by the government, I highlight that BPJS
needs to be evaluated and criticized by us. Why us? Because we are medical students,
this case is clearly related to our future profession. Many of us may take a role in
governmental position a decade later, or even less than a decade. So hopefully by this
brain storming, we can give solution also recommendation to the stakeholder for a
better universal coverage.
Lets go to the first issue. Challenges in services and infrastructures. BPJS began on 1st
January 2014, but the implementation has not gone smoothly yet, with many workers at
hospitals poorly informed about the programs details. Hospitals are afraid they will lose
money by not being repaid like in the past. As a result, the quality of treatment has been
compromised while the patients safety is also a concern.

The second issue is a problematic payments. The World Bank estimates the insurance
plan will cost $13-$16 billion each year once it is implemented, while Jakarta has said it
will double its spending to 16 trillion rupiah in 2014 to cover the poor. The members of
Indonesian Medical Association says the amount the government is paying for the poor
is too low, and this could absolutely decrease the quality of healthcare, relating to the
first issue.

The third is about the doctors. Our former president Susilo Bambang Yudhoyono ever
said that we must aware to the potential problem coming from universal coverage,
especially that doctors will not be adequately paid and they wont be able to provide the
maximum quality of care. In the other hand, the Health Ministry says that Indonesia
needs more than 12,000 new doctors to meet its goal of 40 doctors per 100,000 people.

Ladies and gentlemen, we can take a lesson from our neighbouring country, Thailand.
When they implement Universal coverage, precisely the quality of healthcare is
dramatically decreasing. Furthermore, the patients are becoming lazy to maintain their
health. They become spoiled and asked for an excessive spending. As the result, the
burden that governement need to handle is getting bigger. Actually this also happened
in Indonesia.

I fundamentally believe and support BPJS as the form of universal coverage. I believe
that BPJS brings hope of better health and protection of the poor. But somehow, as
medical students, I would like to suggest to the government to be more concerning, as
soon as possible, to improve the services and facilities of healthcare system in
Indonesia. I truly believe that right now our government is still working on it. And Im
optimistic Indonesia can have better uinversal coverage as soon as possible. Besides,
ladies and gentlemen, as a medical student I also want to encourage you all as my
friends, my partners, to be more active in socializing the way of healthy life in society, so
the people will not always suspend BPJS all the time. I absolutely believe that by these
2 combination way, we can achieve what we want, that is universal accessible
healthcare for everyone.
Ladies and gentlemen, we are not living in the movies of Harry potter. Where students
go to Hogwarts School of Witchcraft and Wizadry, and where the impossible is possible.
If we had magic wand, we could right away make up the best healthcare system or style
to enhance improving the health quality of people. But thats impossible. This is the
reason why I believe in making a good universal coverage needs process, support,
awarness, and evaluation from who? From me and you, as the one who soon wearing
