METHOD STATEMENT
1.Supervisor must ensure forklift to be use has current thorough examination report
2. Supervisor must carry out toolbox talk before works start, covering task to be carried out, personnel
involve, risk, work areas and exclusion zones.
4. Before lifting, operator should check ground conditions to ensure its suitable for them to drive in, the
operator should then lift the load approximately 150mm (6inches) to check the load position, weight and
balance.
5. If the operator is unable to see clearly the load final position, a banksman should be used to signal to the
operator when landing the load
6. Before lifting the load, the operator should step out and inspect the load- check for warning signs,
centre of gravity and loose materials

3. Equipment failure 1. Ensure forklift has been adequately maintained, with current certificate
