Introduction
The Scott County, Indiana, outbreak of human immunodeficiency virus (HIV) and hepatitis C
virus (HCV) is the single largest known outbreak among persons who inject drugs (PWID) in the
United States. As of February 2017, 215 HIV cases had been reported to the Indiana State
Department of Health (ISDH) in Scott County. Of these, 204 (95%) reported injection drug use
and 204 (95%) were co-infected with HCV. Substantial increases in the rate of chronic HCV
infection have also been noted in Scott County (316 to 1,001 cases per 100,000 population from
2014 to 2015), although the full contribution of injection drugs is unknown due to the
magnitude of cases and fact that not all could be prioritized for investigation. The response to
the outbreak has been as large and complex as the outbreak itself, being among persons who
reported injecting oxymorphone (Opana) and sharing needles, and involving numerous federal,
state, and local agencies and partners.

What Local Health Departments Can Do


Local public health staff are critical to the identification of outbreaks. Important
activities to implement, maintain or enhance:

Promptly report new HIV cases to the ISDH Division of HIV/STD/Viral Hepatitis
Promptly report new HCV cases to the ISDH Epidemiology Resource Center
Look for possible clusters of HIV and/or HCV: case numbers above the baseline rate,
disproportionately more cases in a specific demographic group, or common risk factors
Become familiar with local data on injection drug use and related infections to identify
clusters
Request access to the ESSENCE system to monitor emergency department chief
complaints due to drug overdose
Consider enhanced testing for HIV and HCV, especially in jails, prisons, drug treatment
centers, and emergency departments

Data Methodology
Data were acquired from several Indiana State Department of Health divisions. If the number
of cases in a county was less than five, case counts and corresponding rates were suppressed
to protect patient privacy and were reported as zero. Rates in this report represent cases per
one hundred thousand persons by county and were calculated by dividing the case count by
the county population estimate (U.S. Census Bureau) for the corresponding year(s). Rates
calculated from counts less than 20 were considered unstable and are indicated by a grey bar
in the bar charts and a "U" in the maps. Time series charts illustrate change in a variable over
a period of time. Linear trend lines were added to the time series chart to indicate an
increasing or decreasing trend. Trend lines were calculated on the suppressed datasets by
identifying the slope () and y-intercept () of each dataset.

for patients age 15 to 74 with a primary diagnosis of abscess and/or cellulitis of the trunk,
buttocks, or extremities (International Classification of Diseases, Ninth Revision [ICD- 9CM])
codes 682.2682.7 and 682.9) and a primary or secondary diagnosis of opiate, cocaine, or
amphetamine dependence/abuse (ICD-9-CM codes 304.0, 305.5, 304.2, 305.6, 304.4, 305.7).

where the case was investigated. Year is determined based on first positive laboratory specimen
date. Rates are per 100,000 population. Data include those who were incarcerated in the
Indiana

Non-fatal Emergency Department Visits due to Opioid Overdoses
Opioid overdoses that present to the emergency department can be used to monitor trends in morbidity and
aid overdose prevention planning efforts. Prevention strategies include distribution of naloxone (a medication
that reverses the effects of opioids), enhancing access to treatment, including medication-assisted treatment,
and implementing harm reduction services, including comprehensive syringe services programs.

Data Summary: From 2011 to 2015, the rate of non-fatal emergency department visits due to opioid
overdoses increased from 28.5 (n=1,856) to 45.0 (n=2,977) occurrences per 100,000 population.

Soft Tissue Infections Associated with Drug Abuse
The risk of skin and soft tissue infections might be increased by poor injection site hygiene, syringe reuse,
intramuscular or subcutaneous routes of injection, and contaminated drugs. Prevention strategies include
preventing initiation of injection drug use, increasing access to substance abuse treatment programs, promoting
hygienic injection practices such as access to alcohol swabs, clean needles, syringes, and other injection
equipment, and increasing education about and medical access for the treatment of soft tissue infections.
Data Summary: From 2011 to 2015, the rate of soft tissue infections associated with drug abuse in Indiana
increased from 2.4 (n=153) to 5.1 (n=338) occurences per 100,000 population.

Since 1999, a national trend of increasing drug overdose deaths has occurred. Recent rate increases were
driven by synthetic opioids, including illicitly manufactured fentanyl and heroin. Prevention strategies
include distribution of naloxone (a medication that reverses the effects of opioids), enhancing access to
treatment, including medication-assisted treatment, and implementing harm reduction services, including
comprehensive syringe services programs.
Data Summary: From 2013 to 2015, the rate of deaths from drug poisonings due to opioids in Indiana
increased from 2.6 (n=168) to 4.1 (n=274) deaths per 100,000 population.

Hepatitis C, Acute
The Hepatitis C virus (HCV) is a blood-borne virus primarily transmitted by an HCV-infected person
sharing needles, syringes, or other injection equipment. HCV occurs in the first several months after
infection and results in illness from very mild or no symptoms to a serious acute HCV infection requiring
hospitalization. Many people living with HCV do not have symptoms and do not know they are infected.
Chronic HCV infection results when a person is not able to clear the virus after acute infection.
Data Summary: From 2011 to 2015, the rate of acute hepatitis C virus infection in Indiana increased
from 1.3 (n=84) to 2.1 (n=140) cases per 100,000 population.

or by sharing needles, syringes, and other injection equipment. Sharing injecting equipment is considered
high risk for transmitting HIV because the drug materials may have blood in them, which can carry HIV. Use
of drugs can reduce inhibitions and increase sexual risk behaviors, which may result in chlamydia, gonorrhea,
or other STDs. There is no cure for HIV, but treatment with antiviral therapy greatly extends the life
expectancy of people living with HIV.
Data Summary: From 2011 to 2015, the rate of newly diagnosed HIV/AIDS infection in Indiana increased
slightly from 8.0 (n=518) to 9.4 (n=621) cases per 100,000 population.

Chlamydia is a common sexually transmitted disease (STD) that is transmitted to both men and women
through unprotected anal, vaginal, or oral sex. Most of those infected do not show symptoms. The
infection is caused by bacteria that are effectively treated with antibiotics. Untreated chlamydia can
increase your chances of getting or giving HIV.
Data Summary: From 2011 to 2015, the rate of chlamydia infection in Indiana remained relatively
stable, 426.6 (n=27,802) and 436.4 (n=28,886) cases per 100,000 population, respectively.

Endocarditis
Endocarditis, which is inflammation of the inner part of the heart, can be caused by bacteria that travel
to the heart. One of the ways a person develops endocarditis is by injecting drugs using a needle
contaminated with bacteria. Endocarditis might develop slowly over time, but a person can be treated
once the infection is identified. The infection can cause tissue damage and make a person more likely to
develop endocarditis again.

Data Summary: From 2011 to 2015, the rate of emergency department visits in Indiana due to
endocarditis increased slightly from 7.4 (n=484) to 11.7 (n=776) cases per 100,000 population.
