Overview

Welcome to this new and exciting block which offers a challenging ranges of
disciplines that will make up your study of medical professionalism. As we know, doctor is a
profession that place humanity as its central priority. We have to assure that this key principle
will always be our foundation of thought, emotion and action of every doctors in daily practice.
This fundamental principle is called professionalism.
Pelita Harapan University realize the significant of developing and maintaining the high
standard of behavior for every medical graduate, mostly in encountering globalization. The
relationship between doctors and society, the doctor-patient relationship, and the environments
in which doctors undertake thier training and their practice, have all changed. Therefore, Pelita
Harapan University has implemented the course, Doctor Professionalism Ethics and Society
(DPES), to ensure that our graduate not only mastered a large body of medical knowledge and
clinical skills, but also a competent professionals who have a social, cultural and ethical
awareness, who share, interpret, and transfer medical school experiences and knowledge into
effective actions to better themselves and others.
Doctor Professionalism Ethics and Society block will be started at semester 4th and
ended at semester 7th. Each semester has a specific theme of discussion. This block
comprises of lectures, group discussion, role modelling, and case based learning as well.
This should be a great block for you with full exposure to all material that will
enlightened you as a medical professional to be, so keep busy and good luck!

CHAPTER II

Standard of Medical Competency


1. Ethics, morals, medico-legal aspects and professionalism, and patient safety
Core competencies
a. Behave professionally in medical practice, and support health policies
b. Demonstrate concern for moral and ethical aspects, and understanding of ethical
and medico-legal issues in medical practice
c. Apply patient safety procedures

Graduate doctors are able to:


1. Have a professional attitude
Behave appropriately, based on the Indonesian Code of Medical Ethics
Maintain patient confidentiality and trust
Show trustworthiness and respect in doctor patient relationships
Show empathy through a holistic approach
Consider the financial problems and other obstacles to the provision of health
services, and their implications
Behave ethically in patient management, based on professional standards
Recognize alternative options in facing a difficult ethical problem
Analyse systematically and maintain a chosen ethical course of action in patient
management

2. Behave professionally in cooperating with others


Respect everyone regardless of his/her social status
Acknowledge that each person has a valued role and contribution regardless of
their social status
Participate in activities which require collaboration with other health professionals
Recognize and try to mediate in conflict
Respond constructively in receiving feedback from others
Consider ethical and moral aspects in relation to other health professionals, and
act professionally

Recognize and respond appropriately when a colleague has behaved
unprofessionally

3. Play a role as a member of a professional health service team


Participate in patient management and apply professional values
Work effectively in different health teams
Respect the participation of other health professionals
Be a good manager in private practice and in public health care services
Realize that the medical profession has a role in the community, and is able to
contribute to change
Deal appropriately with unprofessional behaviour by other health professionals

4. Conduct medical practice appropriately in the multicultural society of Indonesia


Respect patients and colleagues personal characteristics, life style, and culture
Understand the heterogeneity of perceptions due to differences in age, gender,
sexual orientation, ethnic group, disability, and socio-economic status

5. Comply with the medico-legal aspects of medical practice


Understand and accept legal responsibilities which relate to:
Human rights
Medical prescriptions
Physical and sexual abuse
The Indonesian Code of Medical Ethics
Issuing health, illness, and death certificates
Legal processes in a court
Understand Medical Practice Law No. 29, 2004
Understand the role of the Indonesian Medical Council as an institution that
regulates medical practice
Identify, state, and analyse the ethical aspects of health policies

6. Prioritise patient safety in medical practice
Implement a standard for patient safety:
Patients rights
Education of the patient and his/her family
Patient safety and service sustainability
Evaluate work improvement methods, and implement a patient safety
improvement program
Provide a leadership role in improving patient safety
Provide staff training in patient safety
Encourage staff communication as a key for achieving patient safety

7. Implement seven steps for patient safety:


Build an understanding of the value of patient safety
Lead and support staff
Integrate risk management activities
Develop a report system
Involve and communicate with the patient
Share and learn from experiences about patient safety
Prevent harm by implementation of a patient safety system

2. Effective Communication
Core competencies
Able to explore and exchange information verbally and non-verbally with patients of any
age, family members, communities, colleagues and other professionals.

The graduate is able to:


Communicate with the patient and members of his/her family.
1. Establish rapport with the patient and his/her family.
Greet the patient
Make the patient feel comfortable
Show empathy and trustworthiness
Listen actively (be attentive, and give enough time to the patient to express his/her
complaint and to explore his / her problem)

Summarize the patients problem, worries and hopes
Always establish and maintain the patients dignity, confidentiality and privacy
Treat the patient as an equal and ask for his/her agreement in deciding on therapy
and medical procedures

2. Collect information
Able to use open-ended and closed questions in acquiring information (move from
open to closed questions)
Ask for clarification from the patient about any of his/her statements which cannot
be clearly understood
Use clinical reasoning in exploring the history of the present illness, family history,
and history of past illness
Conduct data exploration logically and efficiently
Not give education and explanation prematurely while collecting information

3. Understand the patients perspective


Respect the patients belief in everything related to his/her illness
Explore the patients needs, worries, and hopes
React professionally to the patients expression of emotion (anger, fright, shame,
sadness, confusion, euphoria) and to the patient with communication problems
e.g. deaf, mute, psychological disorder
Respond professionally to the patients verbal and non-verbal language
Be sensitive to local biological, psychological, social, cultural factors and norms in
order to implement and maintain holistic therapy, and to maintain a professional
doctor-patient relationship
Use polite and understandable language (including local language) which is
appropriate to the patients age and level of education, in asking questions,
summarizing information, and explaining the diagnosis, the chosen management
and the prognosis

4. Give explanations and information
Anticipate and reduce the patients feeling of fright and stress before physical
examination
Warn the patient when physical examination and clinical procedures may cause
pain or discomfort
Give correct, clear, complete and honest explanations regarding the aims, goals,
benefits, and risks of diagnostic procedures and medical procedures (therapy,
surgery, prognosis, referral) before they are undertaken
Answer questions honestly, and provide consultation or referral for difficult
problems
Provide education or health promotion for the patient and his/her family
Reconfirm that the patient has understood all of the information and choices about
potential procedures
Give enough time for the patient to reconsider and consult before agreeing to a
procedure
Deliver bad news professionally in accordance with medical ethics
Confirm the availability of the health services that have been chosen and agreed

ASSESSMENT
Attendance requirements and prerequisites to sit the examinations:
80 % attendance for all activities
Exemptions permitted with Doctors certificate, event of Family crisis, disaster or
extraordinary reason with prior permission from tutor. Further criteria according to UPH
criteria for exemption

Assessment of Student Performance


A. Tutorial performance
1. Formative: weekly discussions with tutor in the group

2. Formative: mid-block discussions with tutor in the group and written


assessment summary provided to the student.

During this encounter, the tutor will identify any issues of concern with the

Passing grade at UPH is 60.00% (C) and will be determined through recognized
Standards Setting processes. Relationship to the Grading system and GPA is as
indicated in the table.
