Guidance notes for submitting supporting documentation

STEP 2 Send supporting documentation to support your eligibility application form.

Each year, many eligibility applicants are deemed not eligible to apply due to small errors and
omissions in their documentation. Reading this guidance will ensure that you understand what
information you need to provide.

Important information for applicants

All supporting documents (without the guidance notes) must be submitted in the order specified below:
1. Eligibility Checklist, with your name and Oriel Pin number recorded (this will be emailed to you
after submitting your online eligibility application form)
2. One passport-sized photo stapled to the top right-hand corner of the first page of the Eligibility
Checklist
3. Photocopy of the page(s) of your passport or EEA identity card containing your personal details;
including nationality, photograph, date of birth, signature and date of expiry.
4. Original and signed Deans Statement
5. Academic transcripts
6. Medical school degree certificate or, if you have already qualified and dont have your
certificate, an official letter from your Dean confirming your qualification (Please refer to FP/AFP
2017 Applicants Handbook)
7. International English Language Testing System (IELTS) certificate for the academic version of the
test (if required)
8. Evidence of any name change (if applicable)
9. Evidence that you will comply with the General Medical Councils (GMC) requirements for
provisional registration
10. Evidence of your right to work in the UK.

Supporting documentation must be received by the UKFPO's Eligibility Office from 11 July to 10
August 2016. Eligibility documentation received outside this period will not be considered under any
circumstances.

Original completed Deans Statement must be submitted; a photocopy of this document will not be
accepted. Photocopies of all other documents should be submitted as the originals will not be
returned. Photocopies should be in colour.

The UKFPO reserves the right to remove any applicants from the national application process if new
information becomes available which affects their eligibility status.

Information supplied on the Eligibility Application Form and your resulting eligibility status may be
shared with other organisations involved in the application process.

Applicants who qualified from medical school before 2 August 2015 must undergo an assessment of
their clinical skills in October 2016, prior to being granted full eligibility. Go to

Applicants with refugee status

Please contact eligibility@foundationprogramme.nhs.uk if further guidance is required. It is understood
that all documents may not be readily available from the medical school in your home country.

Instructions on submitting eligibility documentation

Please read the instructions below before submitting your documents.

1. Your name. You must complete all documentation using the same name, in the same format each
time it is shown. For example, if your name is written as John Smith on the Eligibility Form, then it
must appear that way in all future documentation and correspondence, not as Jonathan Smith, John
A Smith, JA Smith, Smith John, etc. The name you record on your online Eligibility Application Form
must match the name on your passport.

2. Passport. All applicants must provide a photocopy of the page(s) of their passport containing their
personal details; including nationality, photograph, date of birth, signature and date of expiry. EEA
Identity cards would also be considered valid.

3. One passport-sized photo.

This photo must have been taken within the last 12 months and will be used to identify
you when you sit the Situational Judgement Test (SJT)
Your face must not be obscured in the photo (for example, you should not be wearing
sunglasses or any facial covering). Guidance about suitable photographs is available at
www.gov.uk/photos-for-passports .

4. FP/AFP 2017 Dean's Statement.

You must send the original form. Photocopies are not accepted.
You must get your medical school Dean to complete and sign the Deans Statement.
It is your responsibility to ensure your Dean has completed the Deans Statement
correctly.

5. Academic transcripts of all of your undergraduate university grades.

Photocopies of the original transcripts are required.
If transcripts are not in English, an official English translation must also be provided.

6. Medical degree certificate or official letter from your Dean confirming your qualification.
If you have completed medical school:
A photocopy of your medical degree certificate for your primary medical qualification is
required (please do not submit original certificates as they will not be returned).
If your degree certificate is not in English, an official English translation must also be
provided.
If your degree certificate has not yet been issued, you must provide an official letter on
headed paper from your Dean that confirms the date and year when your degree
certificate will be conferred/ issued. A sample Deans letter outlining the information it
must contain is available in the FP/AFP 2017 Applicants Handbook, Appendix 1.
Failure to comply with these requirements will result in your Eligibility Application being
deemed incomplete.

You do not need to supply any documents in this section.
Please tick not applicable on the Eligibility Checklist.

7. IELTS certificate for the academic version of the test.

A photocopy of a valid pass certificate from the academic version of the International
English Language Testing System (IELTS) is required showing that your score is 7.5 or above
in each of the domains: speaking, listening, reading and writing. IELTS information online
will be accepted as long as a clear screenshot is provided. IELTS offer a checking service.
Applicants can provide a reference number and link to this service.
The expectation is for an IELTS certificate to be submitted within 3 weeks of the exam.
An IELTS certificate is not required if your Dean confirms in the Dean's Statement that your
primary medical qualification (including the language of instruction, examinations and at
least 75% of clinical contact with patients) has been undertaken solely in English.
The UKFPO will not accept English language evidence issued by Universities listed here
http://www.gmc-uk.org/doctors/registration_applications/23567.asp . If your University is
one of those listed, you will be required to submit a valid IELTS certificate with your
application.
The IELTS certificate must be dated no earlier than 12 August 2014.
Please note the requirements for satisfactory IELTS are set at a higher level than the
requirements stipulated by the GMC.
GMC registration does not exempt applicants from meeting the UKFPO English Language
requirement.

8. Evidence of any name change.

If the names given on your passport, Eligibility Application Form, medical school degree
certificate and Deans Statement do not match, you must provide documentation
confirming an official name change (for example, a photocopy of your marriage certificate
or divorce decree).

9. Evidence that you can comply with the General Medical Councils (GMC) requirements for
provisional registration.
Before you can start work as a foundation doctor, you must have provisional registration
with a licence to practise from the GMC.
In order to obtain provisional registration, many applicants qualifying outside the UK are
required to pass the Professional Language Assessment Board (PLAB) examinations, PLAB
1 and PLAB 2.
Go to the GMC website for more information on the requirements for gaining provisional
registration.

Important dates for the PLAB examination in relation to your Eligibility Application:
In order to take PLAB 1, you must have qualified from medical school.
The latest PLAB 1 examination date that would allow sufficient time to take PLAB 2 and get
GMC provisional registration by the start of the Foundation Programme is 16 March 2017;
bookings must be made by 9 March 2017 (subject to availability).
If you need to undertake PLAB 1 and PLAB 2, your date of qualification from medical
school must be no later than 9 March 2017.
You should not assume that there will be places available on the PLAB examination dates.

 The evidence required to support your Eligibility Online Application

If you have not yet qualified from medical school:

You must either provide evidence that you are not required to take the PLAB examinations,
or a valid Deans Statement showing that you will qualify from medical school by 9 March
2017.

If you have already qualified from medical school:

You must submit evidence related to your GMC registration status by submitting ONE of the
following documents:
A photocopy of the letter from the GMC confirming that you already have provisional
registration with a licence to practise.
Evidence that you are not required to take the PLAB examinations.
A photocopy of the email/letter from the GMC showing that you have successfully passed
either PLAB 1 or PLAB 2.
A photocopy of the confirmation email/letter from the GMC showing that you have booked
a place to take the PLAB 1 examination no later than 16 March 2017.
If you are not able to provide one of the above documents when you submit your
supporting documentation to the Eligibility Office, you have until 12 January 2017 to provide
this information.

If you are not required to undertake the PLAB examinations to gain provisional registration with
the GMC:
You must provide proof of this.
Refer to the GMC website www.gmc-uk.org for guidance regarding who needs to undertake
the PLAB examinations.

10. Evidence of your right to work in the UK.

You must have the right to work in the UK as a doctor in training to be considered fully
eligible for allocation to the UK Foundation Programme.
Proof that you have the right to work in the UK must remain valid until at least 2 August
2017.
If your visa page states family member of EEA National, you must provide the relevant
marriage certificate.
For those seeking to exercise their EC rights, you will be required to provide your partners
passport, marriage documents and evidence of exercising your EC rights on the appropriate
documentation.
You have until 12 January 2017 to provide proof of your right to work in the UK to the
Eligibility Office.

IMPORTANT INFORMATION REGARDING RIGHT TO WORK:

For the UK Foundation Programme commencing in August 2017 (FP/AFP 2017), we expect there will be
more fully eligible applicants than there are vacancies. If this is the case, applications from individuals
who are not able to provide evidence of their right to work in the UK by 12 January 2017 will not be
considered.

If you cannot provide evidence of your right to work in the UK by 12 January 2017, you may still submit
your eligibility documentation for consideration. If you are deemed eligible to apply for FP 2017 subject to
providing evidence of your right to work, you may go on to submit an FP 2017 application when the

submission period opens on 03 October 2016. However, you should bear in mind that you will be required
to sit a Situational Judgement Test (SJT) in the UK on 02 December 2016 or 09 January 2017 and it is very
likely your application will not be considered under the Home Office resident labour market test. The
resident labour market test dictates that applicants with the right to work must be considered first, and it
is anticipated there will be sufficient fully eligible applicants to fill all available vacancies.
