AFFIDAVIT
I, TAMTAILER P. NANDU, Filipino, of legal age, a resident of
Kasalamatan Village, Jolo, Sulu after having been duly sworn to an
oath in accordance with law hereby depose and say that:

1. I was the OIC, Schools Division Superintendent of Dapitan

City Schools Division in 2012-2013;

2. Sometime in December, 2012, I received a check in the

amount of Two Hundred Seventeen Thousand Five Hundred
Fifty Four and 75/100 (Php. 217,554.75) as budget for the
Dapitan City Division for ZPRAA 2012 from the Sports Fund of
the Regional Office ;

3. That I gave the said check to Mr. Germanico C. Malacat,

Education Program Supervisor I and In-Charge for Sports in
the Division to cover the expenses on the said ZPRAA Meet
with an instruction for him to liquidate as it is an Standard
Operating Procedure in handling government funds ;

4. That it only came to my knowledge that said funds was not

liquidated when I was already transferred to Isabela City
Division sometime in 2014;

5. I am executing this affidavit in good faith and to attest to the

truth and veracity of the foregoing statements and as
necessary for clearance from Dapitan City Division.

IN WITNESS WHEREOF, I have hereunto set my hand this 12th
